The COVID-19 pandemic caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has caused more than 620 million diseases and 6.5 million deaths worldwide as of September 2022, according to World Health Organization (WHO). Today we know that, its effect on the respiratory and nervous systems. Vessels (endothelium), myocardium, kidneys, gastrointestinal tract and other organs are also affected. However, the impact of COVID-19 on the reproductive system and subsequent changes in the menstrual cycle insufficiently explored. The goal of the review is to study the impact of the coronavirus pandemic on the female reproductive system, in particular on the menstrual cycle of adolescent girls. The review includes clinical studies aimed at studying the impact of coronavirus infection (COVID-19) on reproductive function in adolescent girls. We searched for data in PubMed, Google Scholar and Cochrane Library search engines. Searching PubMed, Google Scholar, and Cochrane Library databases using keywords, 514 studies were found. Further, 42 articles were screened. During the selection, 30 articles were examined, selected by title and abstract of studies over the past few years. A full-text analysis of the selected studies was performed, as a result of which 11 studies were eligible for a qualitative synthesis

The frequency of detection of stromal gastrointestinal tumors in children and adolescents is about 0.4% of the total number of diagnosed GISTs. These tumors can occur throughout the gastrointestinal tract. During the instrumental research, misinterpretation of tumor origin is possible. For this reason, the consistent use of a variety of non-invasive diagnostic methods is extremely important in the preoperative period. We present a case report of a rare GIST detection in pediatrics, which at the stage of noninvasive diagnostic studies was determined as a pelvic tumor. It was possible to accurately determine its origin only after diagnostic laparoscopy. We also provide a description of instrumental and laboratory research methods for the differential diagnosis of the abdominal and pelvic cavities neoplasms

Ovarian tumors are an important and urgent problem of modern gynecology. This is due to the difficulties in diagnosing neoplasms due to the absence of characteristic complaints and frequent asymptomatic course. Pediatric surgeons, pediatric gynecologists should conduct differential diagnostics between ovarian tumors and tumors of the abdominal cavity and retroperitoneal space in order to timely diagnose and treat neoplasms and prevent possible medical error. The article presents an indicative clinical case of the occurrence of giant mucinous ovarian cystadenoma in a teenage girl requiring differential diagnosis with formations of a different localization. Patient F., 16 years old, was admitted to the hospital in the direction of one of the regions of the Russian Federation. The reason for the patient's treatment to a gynecologist at the place of residence was pain in the lower abdomen for a month and an increase in the size of the abdomen. The patient was sent to the hospital to clarify the diagnosis and further treatment. Radical surgical treatment was performed: removal of the cyst of the left ovary using video endoscopic technologies

Perimenopause is a period in a woman’s life characterized by estrogen deficiency, leading to atrophic changes in the reproductive system, which are directly correlated with the incidence of the cervical cancer. Using the example of this case report, we would like to demonstrate that a competent examination helps to verify the diagnosis, before using laboratory and instrumental diagnostic methods

Total prolapse of the uterus is a common pathology in older women who have given birth. To date, there is no single ‘gold standard’ in the surgical management of women with total uterine prolapse. Using the example of this case report, we would like to demonstrate that the operation of choice should be considered – vaginal extirpation of the uterus without appendages. This is due to the lower frequency of intraoperative complications and the peculiarities of the course of the postoperative period, as well as early activation and rehabilitation

The article describes a clinical case of giant fibroadenoma in a 13-year-old girl, whose size was 20 cm, as well as a brief review of the scientific literature on benign neoplasms of the mammary glands in young patients. Giant fibroadenomas account for 0.5 to 2% of all fibroadenomas. In children and adolescents, ultrasound of the mammary glands is the main method of diagnosing fibroadenoma. Fibroadenoma needs to be differentiated from a phylloid tumor, which is clinically no different from fibroadenoma, but at the same time it has a high risk of malignancy. To make a final diagnosis, a puncture fine needle aspiration biopsy is additionally perform

The aim is to clarify the timing of recurrence of endometriomas removed and the dependence of the ovarian reserve status on the number of ovarian resections performed, as well as to assess the awareness of patients about the impact of surgical treatment of endometrioid ovarian cysts (EOC) on reproductive function and therapeutic effectiveness of in vitro fertilization (IVF) technology. Material and methods. In 93 infertile patients with EOC and values of anti-muller hormone (AMH) < 1.2 ng/ml, the dependence of the degree of reduction of this hormone on the number of operations performed with relapses of endometrioma was analyzed. Using standard research methods in the same patients, the prevalence of infertility factors identified in the International Classification of Diseases of the 10th revision was studied in parallel, and women's awareness of the impact of surgical treatment of EOC on reproductive potential was clarified. Results. Other forms of endometriosis were found in 71% of women with EOC, and the observed infertility, which necessitated the appointment of IVF, was mainly associated with confirmed male and tubal factors. Among the patients operated on for EOC with a reduced level of AMH, the majority underwent repeated ovarian resections due to relapses of endometriomas that occurred within two and a half years after their removal. The degree of decrease in the level of AMH was directly dependent on the number of operations performed. During surgical treatment of EOC, only 7.5% of patients received relevant information about their frequent recurrence and possible consequences for the reproductive function of ovarian resection performed (especially multiple). Conclusions. Frequent recurrence of EOC, which forces repeated resections of the ovaries, is an obvious factor that, due to the reduction of the ovarian reserve, prevents the successful use of IVF technology with its own oocytes in situations where this method of infertility treatment is in demand. When planning the removal of EOC in women of reproductive age, surgeons should necessarily refer patients to reproductologists for advice on issues related to the implementation of reproductive plans both in the near and long term

Bacterial vaginosis is a clinical condition in which the normal vaginal microflora, predominantly lactobacilli, is replaced by other microorganisms, mainly anaerobes. The goal of treating bacterial vaginosis is to restore the normal microflora of the vagina, to delay the growth of microorganisms that are not characteristic of this vaginal microcenosis. The purpose of the study was to evaluate the effectiveness of various methods of combined two-stage treatment of bacterial vaginosis in women of reproductive age. Material and methods. The study included 50 patients aged 18 to 45 years (mean age 29.6 ± 4.5 years) with bacterial vaginosis. The first group consisted of 25 patients who received Clindamycin 2% cream for 7 days, dose vaginally at night. For the second stage of therapy in this 1 group, acidophilus bacteria + vitamin-mineral-protein-peptide complex Superlimfly (Acylact Duo) were used, suppository time per day at night intravaginally for 10 days. The second group consisted of 25 patients who received Clindacimin cream for 7 days, dose vaginally at night. For the second stage of therapy, a preparation containing a lyophilized culture of lactobacilli L. casei rhamnosus Doderleini was used, at least 1 × 108 CFU, capsule in the vagina at night for 14 days. Results. The effectiveness of BV therapy was 49% (n = 46). The duration of remission was 24.1 ± 2.1 days (n = 24) in the group of women receiving adjuvant therapy with Acilact Duo dosed cream, and 19.9 ± 2 3. days (n = 22) in the comparison group. In all patients, the average vaginal pH before therapy was 5.5 ± 0.4. In group1 participants, after therapy, the pH was 4.54 ± 0.143 , in group 2 patients, the average vaginal pH was 4.976 ± 0.241. Among the patients of both groups, vaginal dysbiosis of varying severity was detected before treatment (normocenosis was not detected). After therapy, in patients of group, normocenosis of the vagina was achieved in 88% (22/25), in participants of group 2, normocenosis was noted in 72% (18/25). At visit (in 45 days) in women of the group 1 84% (21/25), in the participants of the group 2 – normocenosis was noted in 64% (16/25). Conclusions. Acidophilic bacteria + vitamin-mineral-protein-peptide complex Superlymfly (in which dosed Acylact Duo) quickly restores the optimal acidity of the vagina (pH value) and promotes the growth of the number of beneficial lactobacilli, as a result of which it creates an optimal and natural microflora that prevents the appearance of dysbiosis in patients of reproductive age

Acidophilic bacteria + vitamin-mineral-protein-peptide complex Superlymfly (in which dosed Acylact Duo) quickly restores the optimal acidity of the vagina (pH value) and promotes the growth of the number of beneficial lactobacilli, as a result of which it creates an optimal and natural microflora that prevents the appearance of dysbiosis in patients of reproductive age","PeriodicalId":11400,"journal":{"name":"Effective Pharmacotherapy","volume":"5 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-04-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87968239","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Cardiovascular disease (CVD) is the main cause of death in patients with diabetes mellitus (DM). Cardiovascular risk in patients with type 2 diabetes is 2–4 times higher, and CVD occurs 15 years earlier than in people without diabetes. Timely detection and effective control of risk factors are of decisive importance for the prevention of complications of type 2 diabetes. The aim of the study: to identify cardiovascular risk factors and analyze the effectiveness of their control in patients with type 2 DM and diabetic foot syndrome (DFS). Material and methods. 80 patients with type 2 diabetes without associated CVD. Mean age 64.6 ± 9.2 years. The duration of type 2 diabetes was 10.0 (4.0; 15.0) years. Conducted a clinical examination, laboratory (clinical and biochemical blood tests) and instrumental (electrocardiography, ultrasound of the brachiocephalic arteries and arteries of the lower extremities, echocardiography, volumetric sphygmography) studies. Results. The frequency of detection of hypertension was 98.8%, dyslipidemia 91.0%. Target values of blood pressure (BP) were noted in 32.0%, and target values of low-density lipoprotein cholesterol (LDL-C) in 26.0% of the examined. 80.0% of patients received antihypertensive therapy, and 39.7% of patients received lipid-lowering therapy. Metformin was the most commonly prescribed oral hypoglycemic drug in the structure of drug therapy for type 2 diabetes. Insulin was significantly more frequently prescribed to patients with DFS. The target level of glycated hemoglobin (HbA1c) was observed in 55.0% of patients. Patients with DFS had significantly higher systolic and pulse BP, and reached the target BP values three times less often. Conclusion. Patients with type 2 diabetes are characterized by insufficiently effective control of glycemia and major cardiovascular risk factors. Thus, the target HbA1c levels were noted in every second patient, the target BP level in every third patient, and the target values of LDL-C only in every fourth patient.
