"Effect of different Tooth Surface Finish on Marginal Adaptation of Laminate Veneers”-an In Vitro Study "不同牙齿表面处理对层压板贴面边缘适应性的影响"--体外研究

Aim: Assessment of marginal adaptation of laminate veneers after using three different diamond grit abrasives on natural teeth in sequence blue, blue and yellow, blue yellow and polishing tips in three groups. Methodology: Natural sound incisors were collected from Ain shams oral and maxillofacial surgery undergraduate clinic. Specimens were cleaned using ultrasonic scaler and low speed polishing tips. Teeth were then prepared using silicone index for standardization purposes to receive a butt joint veneer preparation; then secondary impression was taken for all the specimen then designed on EXOCADGMBH and milled using a ROLAND 5 axis milling machine. Afterwards lithium disilicate e.max processed in a Programat IVOCLARVIVADENT (P710) porcelain furnace. Veneers were then seated on the die using glycerin and placed on the measuring grid of digital microscope for measurement and statistically tabulated. Results: statistically significant difference between three tested groups showing improved marginal adaptability. Blue showed the highest marginal gap, yellow was second lowest and white polish group was lowest. Conclusion: Fine Polishing showed improved vertical marginal gap distance and marginal adaptation of laminate veneers.

EFFECT OF COMMONLY CONSUMED BEVERAGES ON COLOR STABILITY OF HYBRID CERAMICS: AN IN VITRO STUDY 常见饮料对混合陶瓷颜色稳定性的影响:体外研究

Background : In everyday life, many different beverages and meals are ingested in varied amounts, acidity and colors. Each beverage has distinct influence on the oral cavity structures including the color stability of esthetic dental restorations. Objective : The current study aims to assess the color change of polymer infiltrated ceramic (Vita Enamic) after being immersed in different types of beverages, like coffee, tea and cola in comparison to distilled water as control over a 28-day test period which equals to 28 months of beverages’ consumption. Materials and methods : A total of 40 specimens were sliced from Vita Enamic blocks as each slice measures about 0.5 mm thick. Four groups were tested based on the immersion solutions. All samples were thermocycled at 15000 cycles between 5 and 55 degrees then samples were kept in 20 ml of each solution (Water, coffee, tea and cola) for at 37 ℃ for 28 days for each group. The color parameters (L-a-b) of the specimens were recorded before immersion and at the end of the 7 th (D1), 14 th (D2), 21 st (D3) and 28 th (D4) days after immersion. Color measurements were statistically analyzed with a significance threshold of P<0.05. Results : There was significant difference between four groups where (p<0.001). The differences in the color change (∆E) after staining from the highest to the lowest through all storage periods were as follows: tea > coffee > cola > water. Conclusion: Regarding the limitations of the present study, commonly consumed staining beverages influenced the color stability of the polymer infiltrated ceramics. Tea and coffee beverages produced the greatest color change beyond the acceptance range.

Morphometric analysis of the inferior alveolar canal and its foramina in Egyptian children utilizing cone beam computed tomography: An observational retrospective study 利用锥形束计算机断层扫描对埃及儿童的下牙槽及其孔进行形态分析:一项观察性回顾研究

Aim: This study assessed the effect of age, gender, and arch side on the location, length, and diameter of the inferior alveolar canal and its foramina in a group of Egyptian children using cone beam computed tomography. Material and Methods: Fifty scans of 10-15-year-old children of both genders were evaluated for the location, length, and diameter of the inferior alveolar canal, and the location and diameter of the mandibular and mental foramina on both sides of the jaw. Data was collected and analyzed using independent t test for comparing gender and age categories, and Paired t test for comparing arch sides. Results: Age significantly affected the position of the inferior alveolar canal to the superior, P= 0.029 and inferior, P=0.034 mandibular borders, position of the mandibular foramen to the sigmoid, P=0.008, and the size of the inferior alveolar canal, P=0.012. Gender only significantly affected the position of the mandibular foramen to the anterior, P=0.035, and posterior, P=0.013 mandibular borders. The arch side significantly affected the position of the inferior alveolar canal to the superior border, P=0.025, and position of the mandibular foramen to the gonion, P=0.006. Conclusions: Cone-beam computed tomography offers valuable insights into the dimensions and location of the inferior alveolar canal and its foramina for clinicians. Such information is crucial for procedures like mandibular block anesthesia, surgical extraction of supernumerary teeth, odontome removal, and managing severe mandibular fractures resulting from traumatic injuries.

Shear bond strength and SEM Evaluation of glass ceramics bonded to immediately sealed dentine using universal adhesive 使用通用粘合剂将玻璃陶瓷粘接到即刻密封的牙本质上的剪切粘接强度和 SEM 评估

Immediate dentin sealing (IDS) has been proposed as an alternative to delayed dentin sealing (DDS), a procedure in which adhesive resin is applied immediately to freshly cut dentin following tooth preparation. Materials and methods: The occlusal surfaces of 50 freshly extracted human teeth were prepared flat to expose the midcoronal dentin surface. Then, all prepared teeth were allocated randomly according to dentin sealing protocol into two main groups; Group (DDS): Delayed Dentin Sealing and Group (IDS) Immediate Dentin Sealing. For (DDS) group, the prepared occlusal surfaces were covered directly by a layer of provisional restoration. After 1week, provisional restorations were removed and dentin was cleaned. For (IDS) group, universal dentine bonding adhesive was applied immediately to freshly cut dentine, then light cured, and temporarization was performed as in DDS group. Fifty glass ceramic discs were fabricated from Lithium disilicate block. After ceramic surface treatment, discs were bonded to dentine of both groups. Then, all specimens were thermo-cycled before being subjected to shear bond strength test. Scanning electron microscope (SEM) was used to examine dentin-cement-ceramic junction and mode of failure. Results: Specimens with IDS protocol revealed higher shear bond strength than DDS protocol. SEM revealed that Adhesive failure was recorded as the most common pattern in IDS group, while in DDS group showed predominantly mixed failure at the dentine side. Conclusion : The findings of current study suggest that, immediate dentine sealing with universal adhesive proved to be a reliable strategy for bonding glass ceramic restorations on prepared teeth with exposed dentin.

EFFECT OF DIFFERENT SHADES ON THE SURFACE ROUGHNESS OF RESIN NANO-CERAMIC: IN VITRO STUDY 不同色调对树脂纳米陶瓷表面粗糙度的影响:体外研究

Objective : Clinicians use different shades of restorative materials to mimic the natural teeth color. However, the effect of these shades on the restoration properties was unclear. Thus, the aim of the present study was to assess the effect of different shades of resin nano-ceramic on surface roughness before and after aging. Materials and methods : Twenty rectangular specimens of 1±0.05 mm thickness were obtained from Lava Ultimate resin nano-ceramic blocks with two different shades (B1 and C2); n = 10. All specimens were polished and subjected to accelerated aging using autoclave for 6 hours. The surface roughness was assessed using digital microscope before and after aging. Surface morphology analysis was performed using a scanning electron microscope (SEM). After testing for normality, the collected data revealed parametric distribution and were expressed as mean and standard deviation (SD). Data were statistically analyzed using independent t-test and Paired t-test at a level of significance (P ≤ 0.05). Results : Specimens with C2 shade showed statistically insignificant higher surface roughness compared to B1 shade before and after aging. Aging caused significant changes in the surface roughness in both groups, with changes in the surface microstructure. Conclusions : The shade of resin nano-ceramic does not affect the surface roughness. Aging represents an important factor that can affect surface roughness.

Surface morphology analysis was performed using a scanning electron microscope (SEM). After testing for normality, the collected data revealed parametric distribution and were expressed as mean and standard deviation (SD). Data were statistically analyzed using independent t-test and Paired t-test at a level of significance (P ≤ 0.05). Results : Specimens with C2 shade showed statistically insignificant higher surface roughness compared to B1 shade before and after aging. Aging caused significant changes in the surface roughness in both groups, with changes in the surface microstructure. Conclusions : The shade of resin nano-ceramic does not affect the surface roughness. Objective: The aim of this study is to evaluate the effect of different pontic framework designs, constructed from two different materials: monolithic zirconia and Polyether ether ketone (PEEK), on the fracture resistance of implant supported fixed partial prosthesis. Material and Methods: The study conducted assess the effect of pontic design on the fracture resistance of a four units implant supported fixed partial denture; the pontic constructed replaces the second premolar and first molar with two different designs: modified ridge lap and sanitary designs. A total of forty implants were used in this research; the implants are embedded in poly-urethane foam blocks (20 pounds per cubic foot (20 PCF)) to resemble supporting bone structure. Specimens are divided according to material of construction into two groups, monolithic zirconia group (n=10) and PEEK group (n=10). Then, the specimens are sub-divided into two subgroups according to pontic design modified ridge lap group (n=5) and sanitary group (n=5). All specimens are subjected to loading test in a computerized universal testing machine until fracture occurs. Results: According to statistical results, there is a significant difference between the zirconia and PEEK in different pontic designs. The monolithic Zirconia displayed higher results the PEEK upon testing p≤0.05. All the tested materials were within the acceptable range for functioning under mastication. Conclusions: Zirconia and PEEK are materials of choice when designing a long span bridges due to their high fracture strength values. Aside from the pontic design, the restorative material has a great impact on the fracture resistance, behavior of the material and stress distribution.

Aim of study : to evaluate retention & surface roughness of PEKK & Acetal Resin frameworks in Kennedy class IV RPD. Materials and methods: Twelve partial denture frameworks were constructed for this study, acrylic resin (AR) model of Kennedy class IV maxillary arch was constructed & RPD design was established. Abutment preparations were made; digital scanning & RPD framework was virtually designed in 3D format. The STL file was transferred to 5-axis milling machine for milling the PEKK blanks producing PEKK frameworks. For acetal resin frameworks, the STL file was used to print an AR 3D printed framework which was flasked, after which the acetal resin frameworks were produced by injection molding technique. A Universal testing machine was used to evaluate retention of RPD frameworks at different simulated time intervals. The surface roughness of the RPD frameworks were evaluated using 3D optic profilometer at a fixed selected area in all frameworks. Results: The PEKK partial denture framework showed statistically insignificant higher mean values compared to acetal resin at baseline, 3 & 6 months. A significant difference was noticed at the 1, 1.5, 2, 2.5 & 3 year cycles. There was an insignificant difference between the surface roughness of the PEKK & acetal resin frameworks. Conclusion: Within the limitations of this study, it can be concluded that the PEKK may provide significantly better retention compared to acetal resin in Kennedy Class IV partial denture frameworks. However, both materials showed an insignificant difference regarding their surface roughness.

Assessment of Oral Health Status in a Group of Egyptian Children Undergoing Hemodialysis: An Observational Study 评估一组接受血液透析的埃及儿童的口腔健康状况:观察研究

Aim: The study aimed to assess the oral health status of a group of Egyptian children with chronic kidney disease undergoing hemodialysis. Methodology: A total of 69 children were included in this study. Clinical examination included the following parameters: caries status, periodontal health, oral hygiene, presence of any developmental defects of enamel, oral mucosa, and soft tissues. Results: Within the results, 71% of the children were caries-free and the mean values of DMF and def indices for caries-affected children were 0.53±1.23 and 1.48±1.77 respectively. The mean values of simplified oral hygiene and gingival indices were 1.15±0.79 and 0.54±0.5 respectively. Also, 62.32% of studied children were affected with variable presentations of enamel developmental defects. No soft tissue abnormalities were found in studied children except for mucosal pallor in 46.38% of children. Conclusion: It was concluded that chronic kidney disease and hemodialysis have a negative impact on the oral health of children.
