: Background: Vitiligo is the most common chronic acquired depigmenting disorder, resulting from the loss of melanocytes from the basal epidermal layer . Ecad is the major adhesion molecule mediating melanocyte–keratinocyte interactions and is essential for both the establishment of epithelial structures during development and numerous dynamic pro-cesses such as differentiation, polarity, proliferation, and migration during morphogenesis and homeostasis. The aim of this work : is to estimate the level of E- cadherin in serum of vitiligo patients before and after NB-UVB therapy as compared to normal control persons, to evaluate the role of E-cadherin in the pathogenesis of this disease . Patients and methods : The level of Ecadherin was detected in 30 venous blood samples of vitiligo patients before (group I) and after treatment with NB-UVB ( group II) and 30 healthy controls using ELISA. Informe consent was obtained from the participants in this study after ethical committee approval from Dermatology Department . Results: Serum level of E- cadherin was significantly higher in vitiligo patients before exposure to narrow band UVB compared to healthy controls and became lower in vitiligo patients after exposure to narrow band UVB but still higher than control. No detected relation was found between serum level of E- cadherin and patients’ age, skin type, symmetry ,vitiligo type ,last time of new lesions and VIDA (p-value >0.05). In addition, Serum Level of E-cadherin was significantly higher in Vitiligo patients with negative as compared with positive family history; with a statistically significant (p-value= 0.045),and slight negative significant linear correlation between serum level of E-cadherin and percentage of vitiligo in studied Vitiligo patients; (p=0.045). Coclusion :E- cadherin may play arole in the pathogenesis of vitiligo. Recommendations: On the basis of our findings in this study and in conjunction with that from previous studies, we suggest that: 1) Additional studies on large number of cases in association with assessment of E-cadherin to evaluate its exact role in vitiligo pathogenesis. 2) Study gene polymorphism of E-cadherin gene and association with vitiligo. 3) Performing experimental study to evaluate the effect of E- cadherin up-regulation on vitiligo patients’ response to NB-UVB and other modalities of therapy.

: Background: Cytokine storm causes ARDS, multiple organ failure and post covid-19 pulmonary sequale so anti-inflammatory drugs as tocilizumab and methylprednisolone are widely used to treat covid-19 patients. Methods: prospective observational study on 60 covid-19 patients >18 years old. Diagnosis of covid-19 infection was based on World Health Organization (WHO) criteria and confirmed by nasal and pharyngeal swab for real time reverse transcriptase polymerase chain reaction analysis. 30 hypoxemic covid-19 who received tocilizumab and 30 hypoxemic covid-19 patients who received high dose of methylprednisolone.then Comparison of both groups regarding duration of admission, length of Oxygen need, resolution of CT findings and inflammatory marker levels. Results: There was no significant difference ( p-value >0.05) between the two treatment protocols as regards CT findings at time of admission and after a month from discharge. On admission, Higher mean of CRP, D-dimer and a lower mean of procalcitonine were noted among the group treated with Tocilizumab protocol while significantly lower mean of ferritin (p-value <0.05) .After one month follow up there was no difference in CRP level between the two groups. Also there was no statistical significant difference between the two groups regarding hospital stay and length of oxygen therapy between the two groups. Conclusion: lower ferritin and higher D-Dimer were noted in tocilizumab treated group while no difference in radiological finding or length of hospital stay between the two groups

: The goal of this study is to evaluate both functional and radiological outcomes in patellar fractures management with both techniques; cannulated screws with tension band and modified tension band wiring. 30 patients, with displaced or mildly comminuted and partial articular patellar fractures, were randomly grouped into two groups, 15 in each group. Group 1: fixed by cannulated screws with tension band and group 2: fixed by modified tension band wiring. By comparison between types of fixation, regarding post-operative complication (loss of knee motion, loss of reduction, delayed or non-union and pain during movement) there was no statistically significant difference between types of fixation, but there was statistically significant increase hardware removal and infection in K-wires method. Cannulated screws with tension band has a better radiological outcomes and better curative effects on patella fracture with low incidence of complications. Modified tension band wiring technique despite being the traditional and most widely accepted method of transverse patella fracture fixation; it has certain distinct disadvantages.

: Background and objectives: Erectile dysfunction (ED) is the inability to achieve or sustain an erection for satisfactory sexual activity Aim of work: evaluate sexual functions of patients with erectile dysfunction and their female partners across different ED therapies . Patient and methods: A total of 100 married couples (the males age range from 51-60 years, the females age range from 38-45 years) were included in the study.. Results: female partners of ED patients who received sildenafil did not demonstrate improvements in frequency of pain during vaginal penetration, confidence of becoming aroused, frequency of satisfaction with arousal and maintenance of lubrication. Concolusion: This study linked treatment-related improvement in men’s erectile function with observed improvements in women partner’s sexual function mainly with Sildenafil.

: Background: Psoriasis is a chronic disease, in which genetic, immunological, and environmental factors play a significant role. The immunological mechanism involved in pathogenesis causes cutaneous inflammation and keratinocytes hyperproliferation. Subcutaeous adipose tissue secretes adipokines which play a role in cutaneous inflammation. Visfatin is an adipokine that induces inflammatory process by increasing cytokines production and activation. It affects inflammatory and immune response through induction of chemotaxis. Aim of this work: To estimate visfatin level in lesional tissues of psoriatic patients in comparison with healthy controls and its correlation with disease severity. Patients and Methods: The expression of visfatin was detected in the skin of 30 psoriasis patients and 30 healthy controls using using ELISA technique. Results: Visfatin expression was significantly higher in psoriasis patients compared to healthy controls. In addition, there was no significant difference between visfatin expression and severity of the disease. Conclusion: Visfatin may play a role in pathogenesis of psoriasis.

: There are numerous techniques for dealing with astigmatism both during and after cataract surgery. Good uncorrected post-operative distance visual acuity can be obtained for high percentage of cataract patients with pre-existing corneal astigmatism. The goal of this study is to determine the keratometric change and resultant post-operative refraction of routine cataract patients, after a clear corneal incision of 2.8mm on the steeper meridian. Place and Duration of Study: This was a general free hospital-based prospective study. The study was started from May 2016 and was completed by May 2018. Methodology : A total of 30 cataract patients were included in this study. Pre-operative keratometric readings were compared with postoperative readings, after a clear corneal incision (CCI) of 2.8mm made during phacoemulsification procedure. Results : Patients were monitored for four weeks post-operatively. A total of thirty eyes underwent phacoemulsfication cataract surgery with clear corneal incision on steep axis had mean residual astigmatism of 0.45 D and SD achieved 0.58.

: Background: Gentamicin (GNT) is a highly effective aminoglycoside antibiotic that is commonly used to treat life-threatening bacterial infections. Aim: The aim of the current study is to determine whether tadalafil can protect against GNT-induced nephrotoxicity. Methods: Twenty-four male Albino rats were used in the study, which were randomly divided into four groups, six animals each. Control untreated group received distilled water (5ml/kg, P.O) for 12 days and on days 6-12, they received (5ml/kg,i.p) normal saline daily, one hour after oral administration of distilled water. Tadalafil group received tadalafil (5mg/kg, P.O) for 12 days and on days 6-12, they received (5ml/kg,i.p) of normal saline daily, one hour after oral administration of tadalafil. Gentamicin group received distilled water (5ml/kg, P.O) and on days 6-12, they received gentamicin (011mg/kg,i.p) one hour after oral administration of distilled water. Gentamicin+Tadalafil group received tadalafil (5mg/kg, P.O) for 12 days and on days 6-12, they received gentamicin (100mg/kg,i.p) one hour after oral administration of tadalafil. Body weight and kidney weight were investigated. Urine volume as well as urinary albumin, creatinine, creatinine clearance and albumin /creatinine ratio (ACR) performed. The results of the present study showed that GNT decreased creatinine clearance and increased the serum levels of renal function parameters. GNT caused a significant increase in renal cortex MDA, NO and urine levels of KIM-1, while it decreased CAT and SOD activities. Gentamicin administration resulted in increased immunohistochemical expression of iNOS enzyme while decreasing eNOS expression. Renal corpuscles and tubules also showed histological and ultrastructural changes. Pretreatment with tadalafil, on the other hand, reversed the effects of GNT administration. In conclusion , findings of the present study indicate that tadalafil reduces GNT-induced kidney damage by inhibition of inflammation, oxidative stress, and apoptosis.

Gentamicin+Tadalafil group received tadalafil (5mg/kg, P.O) for 12 days and on days 6-12, they received gentamicin (100mg/kg,i.p) one hour after oral administration of tadalafil. Body weight and kidney weight were investigated. Urine volume as well as urinary albumin, creatinine, creatinine clearance and albumin /creatinine ratio (ACR) performed. The results of the present study showed that GNT decreased creatinine clearance and increased the serum levels of renal function parameters. GNT caused a significant increase in renal cortex MDA, NO and urine levels of KIM-1, while it decreased CAT and SOD activities. Gentamicin administration resulted in increased immunohistochemical expression of iNOS enzyme while decreasing eNOS expression. Renal corpuscles and tubules also showed histological and ultrastructural changes. Pretreatment with tadalafil, on the other hand, reversed the effects of GNT administration. : Background: In about 80-90 percent of people with rheumatoid arthritis (RA), the hands are affected. causing discomfort, stiffness, deformity, limited range of motion, and muscular weakening that have a significant impact on physical functioning and activities of daily living. The purpose of this study was to assess hand strength in patients with rheumatoid arthritis and see how it correlated with clinical and radiological characteristics. The participants in this study were 50 RA patients as defined by the criteria of ACR/EULAR 2010, another 50 seemingly healthy participants of similar age and sex were used. Hand Strength was assessed using with Jual hand dynamometer and radiological evaluation by Plain hand and wrist radiography were taken (P-A) and lateral. Hand Strength measured with a hand dynamometer was substantially greater in healthy controls. Health assessment questionnaire, score ranged from (0.05) to (4.50) with an average of (1.06 ±0.58). disability of the shoulder, arm and hand score ranged from (5.80) to (85.80) with an average score of (49.25 ±19.28). Cochin Hand Function Scale score among studied RA patients ranged from (0) to (79) with an average of (34.38 ±19.28). Mean HS in right hands were (13.98 vs. 36.34), in left hands were (12.90 vs. 38.22) in RA patients. HS was significantly higher among studied RA patients with no Loco-motor manifestations as compared with RA patients with Loco-motor manifestations in both right and left hands; (p-values= 0.021 & 0.047) in right and left hands respectively.

: Helicobacter pylori is a microaerophile Gram-negative bacillus, resistant to the activity of gastric juice. The bacteria may take the vegetative form (spiral shape) or sporulation form .Liver cirrhosis and H.Pylori infection are two common diseases in our population and reducing the incidence of complications in cirrhotic subjects is an important step in current gastroenterology practice. The aim of the present study is to determine incidence of H. Pylori infection among patients with liver cirrhosis secondary to HCV infection and study the correlation between esophageal varices degree and H. Pylori infection. This study involved 50 patients with liver cirrhosis secondary to Hepatitis C Virus infection were selected from gastroenterology and internal medicine clinics at Beni-Suef general hospital and Beni-Suef university hospital. Our results illustrate that the percent of H. pylori infection among studied patients was 68%, our results illustrate that there is statistically significant association between the grade of varices and the detection of H. pylori; p value is less than 0.001.
