The Abstract: Background : Noninvasive ventilation (NIV) is now seen as first-line treatment in patients with acute exacerbation of chronic obstructive pulmonary disease and respiratory acidosis (AECOPD). Therefore, the identification of the predictive factors of NIV failure could assist clinicians in detecting the patients at greater risk of negative outcomes. One of these factors is the diaphragmatic dysfunction (DD). Although the measurement of trans-diaphragmatic pressure is the gold current standard for assessing diaphragmatic function, ultrasonography (US) of the diaphragm at the bedside is capable of identifying (DD) in several clinical conditions. Objectives: to evaluate the prevalence of diaphragmatic dysfunction (DD) during AECOPD, and its impact on NIV outcome. Setting: Department of Chest, Respiratory intensive care unit (ICU), Beni-Suef University Hospital. Methods: a prospective observational study which was conducted on 41 adult patients with acute exacerbation of chronic obstructive pulmonary disease who were admitted to respiratory ICU of chest department Beni-Suef University hospital for NIV. The cases underwent diaphragmatic assessment by bedside diaphragmatic ultrasound. Diaphragmatic thickness fraction (DTF) was calculated from the following formula: (Diaphragmatic thickness at end inspiration – Diaphragmatic Thickness at end expiration) / Diaphragmatic Thickness at end expiration. NIV outcomes (failure and success groups) was analyzed to find a cutoff point of DTF to predict success of NIV. Results: The prevalence of diaphragmatic dysfunction in all studied patients was 18 patients (43.5%) out of 41 patients most of them were among NIV failure group [12 (70.5%) out of 17 patients]. The cut off point of DTF was below 0.306 to predict failure of NIV . with a P value 0.002. Conclusions : Assessment of DTF by diaphragm ultrasound in B-mode represents an easy to-obtain new index for prediction of success or failure of NIV in AECOPD patients needing NIV.

: Background : Non-alcoholic fatty liver disease (NAFLD) and chronic kidney disease (CKD) are global health issues all over the world, affecting about30% (NAFLD), and15% (CKD) of general people. Even, with absence of other risk factors such as overweight, diabetes and hypertension, there is force correlation between NAFLD and CKD. Objective: The target of our study is to detect the incidence of non-alcoholic steatohepatitis between non-diabetic hemodialysis patients by measuring Controlled Attenuation Parameter (CAP) using transient elastography (TE) (Fibroscan®). Methodology: This was a cross sectional study in Beni-Suef governorate, including 1000 hemodialysis non-diabetic ESRD patients from different hemodialysis units from all the central hospitals in Beni-Suef governorate. Results : According to CAP (dB/M) there were 503(50.3%) S0, 233(23.3%) S1, 100(10.0%) S1-S2, 67(6.7%) S2,10(1.0%) S2-S3 and 87(8.7%) S3. According to liver stiffness grades there were 508(50.8%) F0, 280(28.0%) F1, 57(5.7%) F2,95(9.5%) F3 and 60(6.0%) F4. There were 508(50.8%) F0 and 492(49.2%) F1:F4 with mean 6.14 (± 3.90SD) and range (2.0 – 19.90). Conclusions: In this study we concluded that there is a high incidence of NAFLD among non-diabetic patients on regular Hemodialysis in Beni-Suef governorate, significantly correlated with increasing degree of liver stiffness.

: Background: Retinal vein occlusion is the second most common retinal vascular disease after diabetic retinopathy. Branch retinal venous occlusion (BRVO) occurs the occlusion of one of the branches of the retinal vein. The occlusion occurs mostly at arteriovenous crossing and more frequent in upper temporal retinal veins. Patients and Methods: In the current study, randomized control study was conducted on 2 groups divided into 27 eyes (Ranibizumab alone), 27 eyes (Ranibizumab and Targeted retinal laser). Results: The BRVO is investigated by FFA and OCT-Macula and macular edema is measured by OCT-Macula. And the current result shows highly statistically significant improvement in visual acuity and reducing macular edema (decrease foveal thickness)with P value <0.001 in Ranibizumab alone compared with Ranibizumab and retinal laser.

: The goal of our study is to assess if the use of Storz professional image enhancement system (SPIES) improve carcinoma in situ (CIS) detection. We conducted a prospective study on 35 patients of both sexes who have fulfilled the inclusion criteria from December 2019 to May 2020. Preoperatively, we collected urine samples from all patients amenable for cystoscopy for urine analysis and urine cytology. Contrast enhanced abdomen and pelvic computerized tomography (CT) was done for all patients. Under spinal anaesthesia white light cystoscopy (WLC) was done first, then SPIES camera was used by the same operator to detect any other suspicious lesions. TURBT was done for all lesions and specimens were sent for histopathology assessment. Our results showed that there was statistically significant difference between the SPIES and WLC regarding the number of lesions detected, as the total number of lesions detected by the SPIES was 37 lesions, three of these lesions were CIS, but the WLC detected only 27 lesions, no CIS was detected by WLC ( p value 0.001). Pathological recurrence was confirmed in 6/35 (17.1%) patients at 3 months follow up.

: The goal of this study is to investigate the levels of serum calcium in asymptomatic pregnant women and measuring the prevalence of hypocalcaemia. Healthy pregnant women (n=500) were enrolled consecutively, with singleton pregnancy, in first trimester with the mean gestational age (9.5 ±1) weeks of gestation. Serum Total and Ionized calcium were measured to all women in the study. The prevalence of hypocalcaemia was measured. Nearly two thirds of women (337) had hypocalcaemia (67.4%), Only 163 had normal calcium levels" serum calcium level of 8.6 mg/dL or above". The prevalence of hypocalcaemia was more in young aged women as about two-thirds (64.8%) were in the age group 20 to less than 30 years, with mean age 23.9 ±4.6 years. Also the prevalence of hypocalcaemia was high in parity less than 2, with statistically significant p-value (0.034). No determinants (education, dietary intake of Ca +2 , weight, height, BMI, and Hb levels) found in our study to be in correlation with the high incidence of hypocalcaemia.

Nearly two thirds of women (337) had hypocalcaemia (67.4%), Only 163 had normal calcium levels\" serum calcium level of 8.6 mg/dL or above\".
