Intrahepatic islet transplantation holds promise for T1D treatment, yet faces challenges like inflammatory storms and graft revascularization delays. Endocrine pancreas bioengineering, especially with encapsulation technologies, aims to recreate vascular and endocrine compartments ex vivo to generate a functional device for successful in vivo application. This review explores the synergy of diverse encapsulation techniques and alternative cell sources in crafting a functional vascularized endocrine scaffold.

For people with diabetes, technological development represents an important opportunity to optimize therapeutic outcomes through an approach aimed at improving the quality of physical activity, exercise, and sleep, while combating sedentary behavior. However, this approach requires adequate training of members of the health care team and includes, in addition to knowledge of the dimensions of the aforementioned behavioral variables, an understanding of the dynamics of change associated with them. It is also crucial not to overlook the importance of active patient participation and the selection of appropriate tools and equipment.

We review the evidence for food choices aimed at reducing cardiovascular disease (CVD). Low consumption of red meat, processed meat, and salt and increased intake of plant-based foods – whole grains, fruits, vegetables, legumes, and nuts – are linked with reduced CV risk. The same applies for the replacement of animal and tropical fats with olive oil and polyunsaturated fat rich oils. Moderate consumption of wine, coffee and tea is associated with a reduced CV risk, whereas soft drinks substantially increase CV risk. New data support the replacement of high glycemic index (GI) foods with whole grains and low GI foods, and a moderate consumption of fermented dairy as appropriate choices for the reduction of CVD.

Triglycerides and cardiovascular prevention have become increasingly important, especially in diabetic patients who continue to have a higher cardiovascular risk compared to the general population. Elevated triglycerides, part of the typical “diabetic dyslipidemia”, are often under-recognized but play a crucial role in cardiovascular risk. Treatments focusing on triglycerides as fenofibrate and omega-3 fatty acids, particularly Icosapent Ethyl (IPE), have shown to reduce cardiovascular events in well-selected patients. This review addressed how and why it is important to consider hypertriglyceridemia in cardiovascular prevention among patients with diabetes.

Type 1 diabetes (T1D) is a common and serious disease that affects children, adolescents, and young adults. Diagnosis is often with diabetic ketoacidosis (DKA), which poses serious health risks. Autoantibody studies have shown that presymptomatic stages can be identified before clinical diagnosis. Screening for autoantibodies in the general population could help reduce and prevent DKA. This review focuses on Italian law 130/2023 on population screening of T1D.

Lipodystrophy syndromes are rare, heterogeneous disorders with significant health impacts, compromising quality of life. Characterized by complete or partial adipose tissue deficiency, either congenital or acquired, they are often associated with low serum leptin levels and severe metabolic abnormalities like diabetes mellitus and dyslipidemia. Diagnosis is challenging due to phenotypic variability and low prevalence. Management requires a multidisciplinary approach, including lifestyle changes and may necessitate leptin replacement therapy.

Maintenance of plasma glucose (PG) homeostasis is accomplished thanks to a complex network system. Even a minor fall in PG may activate multiple neuroendocrine actions promoting hierarchical responses, to prevent and ultimately recover hypoglycemia, primarily neuroglycopenia. Among these responses, gastric emptying (GE) plays an important role by coordinated mechanisms which regulate transit and absorption of nutrients through the small intestine. A bidirectional relationship between GE and glycemia has been established: GE may explain the up to 30-40% variance in glycemic response following a carbohydrate-rich meal. In addition, acute and chronic hyperglycemia induce deceleration of GE after meals. Hypoglycemia accelerates GE, in order to speed up carbohydrate absorption, but its role in counterregulation has been poorly investigated, although it was recognized shortly after the initial use of insulin in human.

Heart failure is the leading cause of death worldwide, due to the limited capacity of the adult heart to regenerate. Today, the treatment of heart failure is based on the use of devices, pharmacological therapy of symptoms, and heart transplantation. However, these approaches do not represent a long-term solution and the availability of heart donors is inadequate. For this reason, there is great interest in regenerative medicine to treat heart failure. This review summarizes primary strategies of cardiac regenerative medicine, their present status, and future perspectives and challenges.

La medicina cardiometabolica nell’obesità 肥胖症的心脏代谢医学

The rising prevalence of obesity worldwide has become a significant health issue, particularly as a key risk factor for cardiovascular disease (CVD). Obesity is linked to various cardiac complications, such as coronary artery disease, heart failure, and an increased risk of sudden cardiac events. It also contributes to other cardiovascular risk factors like dyslipidemia, type 2 diabetes, hypertension, and sleep disorders, heightening the risk of CVD. Understanding the pathophysiology connecting obesity and CVD is essential for effective treatment in obese patients. This includes considering adipose tissue dysfunction, chronic inflammation, immune dysregulation, and changes in the gut microbiome. Recent European Society of Cardiology (ESC) guidelines highlight the critical need to diagnose and treat obesity to prevent CVD. The complex interplay between obesity and CVD presents challenges in clinical practice: obesity can hinder precise CVD diagnosis and necessitates careful tailoring of pharmacological treatments and cardiac procedures. It’s imperative for diabetologists to consider the impact of excess weight in improving cardiometabolic outcomes for those with obesity. This article aims to clarify controversial aspects of managing heart disease in patients with overweight or obesity and reviews evidence on cardiometabolic outcomes from current weight management interventions.
