The N0 Neck in Oral Squamous Cell Carcinoma A Comprehensive Review and Update on Diagnosis and Management 口腔鳞状细胞癌的0颈部:诊断和治疗的综合回顾和最新进展

Oral cavity cancer represents one of the most devastating diseases worldwide, as the morbidity and mortality associated with it can be quite significant. Spread to cervical lymph nodes represents a major contributor to patient prognosis, however, despite many technological advances, accurate and consistent identification of involvement of the neck in early stage oral squamous cell carcinoma remains a major challenge. With increasingly sensitive and specific imaging modalities, cytological analysis, as well as sentinel node biopsy, identification of regional cervical metastasis continuously improves. The spread of this disease to regional lymph nodes of the neck often results in dramatic changes in treatment and prognosis, and thus management of the neck in early oral cancer is of paramount importance. A consensus on the ideal treatment modality for the clinically node-negative neck still eludes us, with elective neck dissection at the time of primary treatment being the current gold standard, with radiation and chemotherapy having adjunctive roles. The “wait and see” philosophy also has its advocates, as well as its critics. Advances in genetic analysis of tumours may represent the future in patient-specific intervention. This article represents a review of past, present, as well as future advances in the diagnosis and management of the N0 neck in oral cancer, highlighting the most recent research, techniques, and practices worldwide. keywords:

For almost two hundred years dentistry has gone through many changes. Some of the changes have been good, and some of the good changes, have been dismissed as bad. The dismissal of “The Focal Infection Theory” for example, is now proving to have been very bad. In the early 1900’s, Dr. William Hunter brought forward the idea of dental infections as being the cause of diseases [1]. Others came after Hunter, such as Dr. Frank Billings, where the term, “Focal Infection Theory” was first introduced.

Contact Point Headache in Adolescence: Diagnostic and Therapeutic Challenges 青少年接触点头痛:诊断和治疗的挑战

One of the unusually rare headaches, known as contact point headache, is a new type of headache in the International Classification of Headaches, which is a major therapeutic challenge in addition to diagnosis. This is also one of the reasons that patients have wandered for years without diagnosis and adequate treatment. Unfortunately, chronic headache pain in the school-age population, especially in adolescence and teenagers, is not uncommon. Although the etiological factors are quite different, rhinogenic headaches, especially touch point headaches, are increasingly more common. It was recognized and described by Sluder in 1908, by which it was named, but it still does not receive enough attention.

Preventing Thyroid Diseases with a Plant-Based Diet, While Ensuring Adequate Iodine Status 以植物性饮食预防甲状腺疾病,同时确保足够的碘含量

People with thyroid disorders in the US usually have an autoimmune disease, such as Hashimoto’s thyroiditis or Grave’s disease. Those following a plant-based diet are much less susceptible to auto-immune diseases in general. They also have a lower BMI on average. Since obesity is a significant risk factor for hypothyroidism, this contributes to the lower risk of hypothyroidism among vegans. However even after controlling for BMI and potential demographic confounders, vegans have been shown to experience a 22% lower risk of hypothyroidism, and a 51% risk of hyperthyroidism. Given that they are unlikely to be consuming any seafood or dairy, iodine deficiency is a greater possibility for those following a plant-based diet. Studies show that many vegans have very adequate iodine status, but some are deficient. The median is mildly deficient, but the range can vary greatly. thyroid hormone synthesis and thereby increases TSH stimulation, which can produce goiter. Laboratory tests that detect abnormal thyroid function may be more useful for diagnosing chronic iodine deficiency or excessive iodine intake and for monitoring the effects of iodine supplementation.
