高尿酸血症・痛風の治療ガイドラインも2011年に 第2版の追補がされ約6年が経過した(第1版2002 年,第2版2010年発行).その間,尿酸生成抑制薬 であるフェブキソスタット(2011年5月販売開始) とトピロキソスタット(2013年9月販売開始)が, 開発・上市された.アロプリノール以来約40年 新規開発のなかった尿酸生成抑制薬が本邦で 開発され,従来の尿酸生成抑制薬に加え薬剤選択 の幅が広がったことから,臨床医の尿酸降下薬 の使用実態についてアンケート調査を実施した. アンケート配布数665名のうち186名の有効回答 を得られた.使用している尿酸降下薬としては, フェブキソスタット183名,アロプリノール161名, ベンズブロマロン153名,トピロキソスタット86名 (いずれも累計)と,大多数の医師が複数の薬剤 を使い分けていた.尿酸降下薬の使用基準とし ては,合併症がない場合で8または9mg/dL以上 が 85.5%,合併症がある場合で 7または 9mg /dL 以上が 91.4%を占めた.また,尿酸降下療法の 目標とした血清尿酸値の達成度は, 「ほぼすべての 患者で達成(100%)」および「大体の患者で達成 (75%)」を併せて,85.5%がほぼ目標を達成して いた.今回の結果から,高尿酸血症・痛風の治療 ガイドラインに基づく尿酸降下薬の使用開始が 浸透していることが示された.
