The AA induced isoimmune thyroiditis in the guinea pig using homologous total thyroid extract. They studied carefully the most intimate ultrastructural alterations in basal conditions and after a single TSH stimulation. About 20% of follicles were grossly damaged, with very little signs of function and did not respond to TSH. About 70% of follicles were less damaged, with very rudimental functional aspects and did not respond to TSH. Only 10% of follicular units, with small alterations of epitelial cells exhibited quite slight changes after the tropin. Very small interstitial infiltration is detectable at the immunization time considered.

Study in the electron microscope of certain human thyroid lesions showed that the phenomenon of nucleolar segregation is comparatively frequent in sporadical or diffuse nodular goitres, in familial goitres and cancer of the thyroid. The reorganization and redistribution of the nucleolar material, reflecting deep alterations in the metabolism of ribonucleic acid, are proper to hypofunctional thyropathies. The intensity of segregation increases in proportion to neoplastic transformation of the thyroid cell.

The existence of a specific hormone of the thymus could be demonstrated with the traditional tests: The observation of the consequences of thymectomy. The possibility to suppress these consequences by a specific therapy. This succeeded by means of thymic grafts and by means of injections of an adequate thymus extract. (Several extracts of demonstrated substitutive potency have been prepared.) A presumably pure chemical entity was isolated, which suppresses the consequences of thymectomy as completely as the total extract. There is circumstantial evidence for the assumption, that under normal conditions this substance is secreted into the circulation. Lymphocytes act presumably as its vectors. The most conspicuous features of the physiological role of the thymic are: (1) Its influence upon the lymphocytes. (2) Its interactions with other hormones. (3) Its role in the development of the immunitary reactions. The complete study of the functions of the thymus under all these aspects has been performed thus far only with the presumably pure substance mentioned above. The question, whether the thymus secretes one or several hormones, can not be answered thus far. (A mention is made of the substances obtained from the thymus and active on the growth of malignant tumours.)

40 consecutive pregnancies following treatment of infertile women with gonadotrophins were investigated. The 24-hourly excretion of oestriol, pregnanediol and chorionic gonadotrophin was measured at weekly intervals and correlated with the clinical findings. 23 pregnancies resuited in single babies, 6 in twins, 1 in triplets and 10 in abortion. Twelve single pregnancies were clinically normal and were used as a control group to estimate the mean and 95% confidence limits of the 24-hour excretions, and the mean and 95% confidence limits of the weekly rates of increase of each hormone excreted. These figures were used to determine if threatened abortion, abortion, pre-eclamptic toxaemia or multiple pregnancy could be predicted from the excretion of the hormones. Abnormally low excretion of at least 1 of the steroids occurred before clinical evidence of threatened abortion in 4 of 10 cases, abortion in 6 of 10 cases and pre-eclamptic toxaemia in 3 of 8 cases. The excretion of all steroids appeared to be raised in multiple pregnancies but the difference between the mean of these and the mean for single pregnancies was not significant, probably because of the wide scatter. The usefulness of hormone assays in pregnancy is discussed.

Stimulation tests with ACTH have undergone various modifications due to the advent of different methods for an assessment of adrenocortical function. An attempt was made to review the manifold diagnostic procedures and to compare them with the experience obtained from an intravenous 1-hour test with fluorometric determination of plasma 11-hydroxycorticosteroids. The latter procedure represents the most simple reliable method being widely adequate to the requirements of clinical practice including outpatient performance. The results of the 1-hour ACTH test in adrenocortical diseases as well as in other clinical conditions are described.
