Effect of Diuretic and Stone Removal Mixture on Kidney Stones and its Influence on Renal Function and Urinary Levels 利尿除石合剂对肾结石的影响及其对肾功能和尿水平的影响

Our main objective is to study the effect of diuretic Paishi mixture on renal calculus after extracorporeal shock wave lithotripsy and its effect on renal function and urinary N-acetyl-beta-d-glucosaminidase, beta-2 microglobulin, cystatin C levels. A total of 150 patients with kidney stones undergoing extracorporeal shock wave surgery were randomly divided into research group and control group with 75 subjects in each group. The patients in the control group were treated with routine methods such as anti-infection, pain relief and hemostasis after operation, while the research group was treated with diuretic and stone-dispelling mixture on the basis of the control group. The medication cycle is 7 d. The treatment effects, symptom scores, average time for residual stone removal, renal function-related indicators, urine N-acetyl-beta-d-glucosaminidase, beta-2 microglobulin, cystatin C levels, inflammatory factor levels and the proportion of analgesic drugs were compared between the two groups. The drug effect of the study group was better than that of the control group (p<0.05), the symptom score, average time of residual stone excretion, levels of renal function indicators, blood creatinine and blood urea nitrogen in the study group were lower than those of the control group (p<0.05). The levels of urinary N-acetyl-beta-d-glucosaminidase, beta-2 microglobulin and cystatin C in the study group were better than those in the control group (p<0.05). The indicators of inflammatory factors and usage rate of analgesic drugs in the study group were lower than those in the control group (p<0.05). Diuretic and stone removal mixture can significantly improve the postoperative effect of extracorporeal shock wave lithotripsy, shorten the time of stone removal, increase the levels of renal function, urinary N-acetyl-beta-dglucosaminidase, beta-2 microglobulin, cystatin C, etc., improve the level of inflammatory factors and reduce the usage rate of analgesic drugs.

A Pilot Study on Assessment of Impact of Pharmacist-Delivered Intervention on Compliance and Low-Density Lipoprotein Goals Among Patients with Cardiovascular Disease 评估药师干预对心血管疾病患者依从性和低密度脂蛋白目标影响的初步研究

The objective of this work was to assess the pharmacist provided telephonic patient educational interventions, designed to improve the management of lipid levels and reduce the non-compliance of lipid-lowering medications. This work was a prospective study. The participants were 60 adults (intervention group with 30 and control group with 30) with cardiovascular disease prescribed lipid-lowering drugs to manage dyslipidemia. Patients in the intervention group were advised regarding lipid management and compliance with therapy by a pharmacist through regular face-to-face counseling and telephonic counseling. Control group patients were received standard medical care. The primary outcome measure was the percentage of patients who achieved a goal low-density lipoprotein below 100 mg/dl±5 % and improvement in adherence to the prescribed medications after 2 mo. In the intervention group, a significant difference was observed in the low-density lipoprotein levels and improvement in adherence after 2 mo of pharmacist intervention. The study demonstrated that pharmacists’ individualized telephonic counseling had positive impacts on the management of lipid levels, including improved medication compliance.

A Scoping Review of Oral Chinese Traditional Medicine for Vascular Dementia 中药口服治疗血管性痴呆的综述

Vascular dementia is a serious neurological disease associated with reduced perfusion of the brain. No western drugs have been approved for the treatment of vascular dementia. Oral Chinese drugs are commonly employed for vascular dementia in China. The purpose of this scope review is to review the recent evidence from clinical trials regarding the benefits of traditional oral Chinese medicines and to outline their effects on vascular dementia. Seventeen Chinese herbal medicines used in vascular dementia were systematically searched from 7 databases, such as EMBASE, Cochrane, PubMed, China Sci-Tech journal database, China National Knowledge Infrastructure, Chinese Biomedical Database as well as Wanfang database. Data were obtained from guidelines, consensus, meta-analyses and systematic reviews and randomized controlled trials. A revised Cochrane Risk Assessment Instrument was applied to evaluate the risk of bias with respect to the intention-to-treat effect on eligible randomized controlled trials. Microsoft Excel 2019, GraphPad Prism and RSTUDIO statistics were employed for data integration and process. As the data show, the number of research published increased between 2009 and 2014, while the most studied medicines were TianZhi, Ginkgo biloba leaf and NaoXinTong. A low overall bias risk of intention-to-treat was achieved in only 7 trials. More consistent benefits were observed with Ginkgo biloba leaf and TianZhi for amelioration of mini-mental state examination score deficits and activities of daily living impairments. The overall reporting quality of randomized controlled trials of traditional oral Chinese medicines for vascular dementia was poor and the transparency of these randomized controlled trials must be urgently improved. Oral Chinese drugs may be beneficial to patients with vascular dementia, but they need to be further assessed.

Analysis of Puerarin’s Mechanism in Treating Diabetic Osteoporosis by using Network Pharmacology 葛根素治疗糖尿病骨质疏松的网络药理学机制分析

Puerarin can promote the proliferation of osteoblasts and it can also be used as a scaffold material, but its mechanism is unknown. In this study, we explored the primary molecular mechanism of puerarin in diabetic osteoporosis treatment. We used traditional Chinese medicine systems pharmacology database and analysis platform, PubChem, web-based gene set analysis toolkit, cytoscape and so on to screen the drug targets of puerarin and the disease targets of diabetic osteoporosis. We analyzed the puerarin mechanism based on the protein-protein interaction co-expression network, gene ontology, Kyoto encyclopedia of genes and genomes enrichment, etc., and molecular docking by AutoDock. The main enriched signaling pathways in the Kyoto encyclopedia of genes and genomes were non-small cell lung cancer, endocrine resistance and so on. The main enriched biological processes were positive regulation of cell migration, cytokine-mediated signaling pathway and so on. The main enriched cell components of the cellular component were the integrin complex, platelet alpha granule membrane and so on. The main cellular functions of the molecular function were C-X3-C chemokine binding, nitric oxide synthase regulator activity and so on. The tumor necrosis factor, phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit alpha and prostaglandin-endoperoxide synthase 2 with puerarin docking binding energy were all less than -5 kcal/mol-1. In conclusion puerarin may act through multiple targets and some pathways in diabetic osteoporosis.

Acute glufosinate ammonium poisoning can cause neurological complications and respiratory failure, which are usually delayed and difficult to predict. Serum ammonia level might be an indicator of severe glufosinate poisoning, but it has not been confirmed. We aimed to investigate the potential predictors of respiratory failure after glufosinate poisoning. We conducted a retrospective review of 21 cases of glufosinate poisoning between 2010 and 2019. Patients were assigned to intubated due to respiratory failure and non-intubated groups. The following characteristics were compared between these two groups; age, sex and period from poisoning to hospital arrival, vital signs, Glasgow coma scale, laboratory parameters and electrocardiogram measurements. Furthermore, the outcomes of morbidity and mortality were analyzed. Totally, 12 cases were be intubated and the other 9 were not intubated.

Comparative Analysis of Natural Bioactive Metabolites of the Indigenous Host Plants of Muga Silkworm for Antioxidant and Antibacterial Properties against Swine Diseases 家蚕本地寄主植物天然生物活性代谢物抗氧化和抗菌猪病的比较分析

The present study explores the natural bioactive metabolites, antioxidant and antibacterial activities present in the host plants of Antheraea assamensis (muga silkworm) viz . Machilus bombycina (som), Litsea polyantha (sualu), Litsea salicifolia (dighloti) and Litsea citrata (mejankari). The methanolic extracts of leaves of these plants were screened for their phytochemical analysis and free radical scavenging activity using 2,2-diphenyl-1-picrylhydrazyl as the free radical. Phytochemical analysis revealed the presence of flavonoids, phenolic compounds, carotenoids, saponins, coumarins, terpenoids, tanins, cardiac glucosides and xanthoproteins in the said plants. The results of 2,2-diphenyl-1-picrylhydrazyl scavenging activity revealed Litsea polyantha to exhibit stronger antioxidant efficiency compared to the rest. Besides these, the plant extracts were investigated for their antibacterial assessment against Gram-positive Staphylococcus aureus and Streptococcus suis and Gram-negative Pasteurella multocida and Escherichia coli bacteria found in pig. All the plant extracts were validated to possess antibacterial activity with variable potency. Litsea citrata was the most effective extract retarding microbial growth of both Gram-positive and Gram-negative bacteria, followed by Litsea salicifolia , Litsea polyantha and Machilus bombycina . The present findings underscore the fact that indigenous food plants of muga silkworm are fortified with antioxidant and antibacterial efficiency, which might pave to the development of novel herbal pharmacological compounds for the medication of pig diseases.
