Gestational Diabetes Mellitus (GDM) is a disorder of carbohydrate metabolism that causes hyperglycemia and insulin resistance during pregnancy. Sirtuin 1 (SIRT1) and Peroxisome Proliferator-Activated Receptorγ (PPARγ) are genes that play a role in glucose metabolism. Dysregulation of SIRT 1 and PPARγ is associated with the GDM. Hyperglycemia in GDM induces inflammation and oxidative stress in hepatocytes causing liver damage. Basil extract (Ocimum basilicum L) is one of indonesia medicinal plant has been used as traditional medicice in diabetic disorder. This study aims to investigate the hypoglycemic activity of basil extract by evaluating the expression of SIRT 1 and PPARγ in GDM rats and investigate the potential hepatoprotective effect of basil extract by analyzing the rat’s liver histology. Twenty-four of pregnant rats were divided into four groups; negative control, positive group induced by streptozotocin 40 mg/kg BW intraperitoneally, and two groups of GDM rats treated with 100 mg/kg BW and 200 mg/kg BW of basil extract for 14 days. Blood glucose levels were examined with a blood glucose meter. The expression of SIRT 1 and PPARγ was assessed by the RT-PCR. Histological analysis of the rat`s liver was conducted to determine the percentage of cell damage and tissue edem The data were statistically processed using SPSS. The extract of basil at a dose of 200 mg/kg BW showed an anti-diabetic effect which decreased the glucose level concentration by about two times compared to the untreated rat. It enhanced the expression of SIRT-significantly with a value of p= 0.035 (p <0.05). Basil extract-treated group showed a trend of increasing PPARγ expression, but not statistically significant. In addition, HE staining on the liver showed a tendency to improve in the group given basil extract. This Study concluded that basil extract could increase SIRT 1 and potential to be an anti-hyperglycemic therapy with a hepatoprotective effect.

Clindamycin peel-off mask film, an effective formulation for C. acnes treatment: characterization and microbiological activity 克林霉素剥脱膜,一个有效的配方痤疮治疗:表征和微生物活性

Clindamycin is the antibiotic of choice for treating Cutibacterium acnes (C. acnes) which was presented in various dosage forms such as topical solution, gel, and foam. In this current study, clindamycin was formulated as the peel-off mask gel, intending to form film, providing the drug release profile in a sustained manner leading to better antimicrobial efficacy and enabling peel-off after use. The peel-off mask gel was designed into many formulations with varying concentrations of film-forming agents. The major film-forming agent was 10 %w/w PVA that was combined with the additional film-forming agents PVP-K30 or SA. Clindamycin 1 %w/w was loaded in the selected formulations F18 (10 %w/w PVA and 20 %w/w PVP-K30) and F21 (10 %w/w PVA and 1.5 %w/w SA). The physical property evaluation of the peel-off mask gel showed that the types and concentrations of film-forming agents influenced the viscosity, spreadability, and film-forming ability. The film’s tensile strength indicating the preference of use was also determined. There was no interaction between the ingredients in the formulation evaluated by FT-IR, XRD, and DSC. The F18 and F21 formulations showed a good to very good spreadability and film-forming ability, a suitable tensile strength with a drying time of approximately 8 min, and pH 4-5. Both formulations provided the highest antimicrobial susceptibility for C. acnes compared to the blank film and the clindamycin solution indicating the potential to be developed as the new market formulation for acne treatment.

An Integrated Authentication Analysis of Citrus aurantium L. Essential Oil Based on FTIR Spectroscopy and Chemometrics with Tuning Parameters 基于FTIR光谱和可调参数化学计量学的柑橘精油综合鉴定分析

Citrus aurantium L. essential oil or orange oil (OO) became more popular in recent years due to its benefit for human health. An “economically motivated adulteration” can be potentially occurred to achieve more profit in the market. On the other hand, a cheaper oil such as coconut oil (CO) was commonly used as adulterant. The objective of this study was to perform authentication analysis of OO by FTIR spectroscopy and chemometrics. Principal component analysis was applied in the exploratory data analysis at the initial stage of authentication analysis. Multivariate calibration of principal component regression (PCR) and partial least squares regression (PLSR) were constructed using five types of pre-processed FTIR spectral data. The PCR model using Standard Normal Variate (SNV) spectra was selected as the best prediction model for OO (Rcal2 = 0.999; RMSEC = 0.193; RCV2 = 0.998; RMSECV = 0.456; Rval2 = 0.992; RMSEP = 0.989), whereas the PLSR model using SNV spectra was selected as the best prediction model for CO (Rcal2 = 0.999; RMSEC = 0.174; RCV2 = 0.999; RMSECV = 0.476; Rval2 = 0.992; RMSEP = 0.991). SNV spectra of OO, CO, and binary mixture of OO+CO were used to generate sparse partial least squares-discriminant analysis (sPLS-DA) model. Tuning parameters of component numbers, the number of variables “keepX”, and the distance of prediction was executed. The component number of three with “keepX” for component 1, 2, and 3 were 1, 5, and 1, respectively, were selected along with the maximum distance approach to construct the discriminant model. The final sPLS-DA model explained the total variances of 94% with satisfaction separatibility of 100%, 97.8%, and 100% for OO, CO, and OO+CO, respectively.

EDUCATIONAL CURRICULUM TO IMPROVING CLINICAL OUTCOME ON DIABETES MELLITUS PATIENTS: A SYSTEMATIC REVIEW 教育课程对改善糖尿病患者临床预后的系统评价

The success of controlling the current condition of Diabetes Mellitus (DM) is not only determined by the help of medicine. DM therapy management is strongly supported by the success of providing education to DM patients, so that patients comply with treatment, adopt a healthy lifestyle, and can prevent complications that can end in death. This systematic review aims to find out what curriculum or content of educational materials needs to be given to DM patients to improve the achievement of clinical outcomes. Sage, Scient direct, and Pubmed were searched for relevant articles published between 2012 and 2022 using multiple search terms. STROBE tools were used to conduct quality assessments. Following that, a narrative synthesis of the findings was performed yield 1619 citations. After de-duplicating and screening, 18 studies were eligible for inclusion giving results. Educational materials related to diet/nutrition (n=17), physical activity (n=15), DM treatment (n=14), complications and risk factors (n= 13), goal setting or Healthy Living with DM (n=13), monitoring blood glucose levels parameters (n=11), description of DM (n=7), mental health (n=5), and foot care DM (n= 4). The review was hampered by the poor quality of the included studies. However, the evidence presented shows that educational materials provided to DM patients can improve clinical outcomes. Overall, the evidence base for educating DM patients in the community is still limited, and further research is needed.

Ebola virus (EBOV) is a type of RNA virus from the family of Filoviridae. The 2014-2016 Ebola outbreak in African countries Guinea, Liberia and Sierra Leone has a total of 28,616 cases and 11,310 deaths. People suffering from Ebola disease reported to have symptoms such as fever, body aches, diarrhea and internal and external bleeding. Death from Ebola is mostly caused by multi-organ failure due to internal bleeding and fluid loss. Another Ebola outbreak has spiked this February 2021, suggesting low effectivity of the previous vaccines used. Zaire Ebola virus (EBOV) is known to be the species highly involved on the recent outbreak with a high mortality rate. This study is carried out to design B-cell epitope Ebola vaccine based on the conserved region of Zaire EBOV glycoprotein. Reverse vaccinology and immunoinformatic approaches are used in this study. Samples of Zaire EBOV glycoprotein sequences were retrieved from GenBank, NCBI. The 3D modelling was done by using SWISS-MODEL web server and PyMol software. Phylogenetic tree analysis was also done by using MEGA X. B-cell epitope prediction was done by BepiPred 2.0 and Emini Surface Accessibility using IEDB web server. Epitopes were selected based on its conservancy by comparing the sequences with the MEGA X alignment result. Antigenicity, allergenicity and toxicity properties of the peptides were predicted using VaxiJen 2.0, AllerTOP 2.0 and ToxinPred web servers. In silico cloning was done as the final step using pET-24a(+) expression vector. This study revealed that peptide “LEIKKPD”, “TGFGTNETEYLF”, “PYFGPAA”, “PYFGPA”, and “KLSSTNQL” are the best candidate of B-cell epitope vaccine. Phylogenetic tree and 3D modelling successfully showed the genetic and structural differences of Zaire EBOV GP originating from various countries. In silico cloning was also done using pET28a(+) expression vector to design clone vector map to be used for the next phase of vaccine development.

Assessing Quality of Self Medication Services in Pharmacies in Indonesia: A Pilot Study using Mystery Customer in Bandung, West Java, Indonesia. 评估印度尼西亚药房自我用药服务的质量:在印度尼西亚西爪哇万隆使用神秘顾客的试点研究。

Self-medication practice is currently increasing in global population. Apart from the benefits, it is undeniable that self-medication can also have several negative impacts. The outcome of self-medication is affected by pharmacist’s clinical ability to provide self-medication services to the patient. This study was aimed to assess the quality of self-medication services in Indonesia using mystery customer approach. A pilot project had been conducted in Bandung, West Java, Indonesia. Mystery customers act as patients who come to the pharmacy seeking self-medication medications for cough/common cold, diarrhea/constipation, pain/fever, and gastritis cases. A total 232 pharmacies were involved in this study and selected through stratified random sampling. The quality of self-medication services was measured by four subdomains: patient assessment, the accuracy of drug selection, the drug information provided to mystery customer, and lifestyle modification counselling. Result showed that patient assessment regarding signs and symptoms only performed by 42,82% pharmacies in Bandung, 14,44% assessed pharmacological and nonpharmacological therapy prior to visit, and only 10,56% assessed patients’ specific medical condition. Although the percentage of patient assessment performed by pharmacies in Bandung were low, interestingly the accuracy of drug selection is relatively high (85,34%). The most prevalent information provided to patient was instruction to consume the medication (59,09%), meanwhile the least delivered information was about side effect (5,17%). Information about lifestyle modification only delivered by 10,34% pharmacies. The quality of self-medication services in Bandung varies depending on the type of pharmacy and service provider. State-owned pharmacy has a higher overall score of self-medication service than private retail pharmacy and private standalone pharmacy (p<0,05). Based on the service provider, pharmacist have a higher score of self-medication service compared to pharmacy technicians and other personnel in pharmacies (p<0,05). This study emphasizes the importance of the pharmacist’s role in self-medication services to ensure rational and safe treatment for patients.

Efficacy of Different Solvents in the Extraction of Bioactive Compounds and Anticancer Activities of Thyme (Thymus vulgaris L.) Leaves and Twigs 不同溶剂对百里香活性物质提取及抗癌活性的影响树叶和细枝

Thyme contains phytochemicals that exhibit cytotoxic and cytogenic activities. Differ solvents have different abilities in dissolving plant bioactive compounds. The objective of this study is to determine the anticancer properties of thyme leaves and twigs including profile of bioactive compounds, cytotoxicity, ability of apoptosis induction and caspase-3 activation of different solvent extracts on T47D breast cancer cell line. An experimental study was conducted in eight groups. T47D breast cancer cell line was treated with thyme extracts as treatment group, doxorubicin as positive control, while untreated T47D cells were used as negative control. Thyme was extracted by maceration using 6 different solvents: methanol, ethanol, chloroform, ethyl acetate, dichloromethane, and n-hexane. Bioactive compounds of all extracts were analyzed by GC-MS. Cytotoxicity was investigated through MTT assay on T47D breast cancer cell line. The percentage of apoptosis and caspase-3 were detected by flow cytometry using Annexin V-PI and BD Cytofix/Cytoperm, respectively. Thymol as the main bioactive compound in thyme was found in all extracts. MTT results revealed that six solvents extracts had moderate to weak cytotoxicity with dichloromethane had the lowest IC50 values at 120.23 µg/ml. All thyme extracts promoted apoptosis in T47D cells with apoptosis percentage of methanol, chloroform, ethyl acetate, ethanol, dichlorometane, n-Hexane and doxorubicin were 82.67; 80.98; 73.13; 72.28; 60.62; 27.74 and 98.43%, respectively. Among all extracts, methanol extract showed the highest apoptosis and caspase-3 activation percentage. We conclude that all thyme extracts had anticancer properties in T47D breast cancer cell line although efficacy of each solvent extract was differed.

The microbial-based skin health care products that balance one's skin microbiome mimicking healthy skin are rapidly growing. Postbiotics gain more advantages in safety, shelf life, active peptides, and other active compounds such as GABA, which benefit human skin. We have isolated skin commensal bacteria, i.e., Staphylococcus hominis MBF12-19J, Staphylococcus warneri MBF02-19J, Micrococcus luteus MBF05-19J and Bacillus subtilis MBF10-19J in our previous study. This study aimed to make a formula of bacterial cocktail (BC) of those strains, elevate the BC formula into a ferment lysate (FL), and characterize the activities and test for skin sensitivity. The antibacterial activity was done by conducting a competition test using Propionibacterium acnes as an indicator bacterium, whereas the antioxidant activity assay was carried out using the DPPH method. The skin sensitivity was tested using the patch test method. The BC was lyzed by optimizing the enzymatic and ultrasonication methods, whereas sucrose was added as lyoprotectant to obtain a stable powder FL. Potential activity to inhibit P. acnes growth was achieved by a formula of FL consisting of M. luteus MBF05-19J: B. subtilis MBF10-19J: S. warneri MBF02-19J: S. hominis MBF12-19J in a ratio 1.5:1.5:0.5:0.5 or equivalent to DNA copy number/mL 1.01E+16: 1.14E+24: 1.96E+22: 1.50E+18. Skin sensitivity test results showed no sensitivity reaction, guaranteeing that CFS and FL are safe to use for the skin. Sucrose-FL at 12% sucrose formula showed higher physical stability than those without. However, the result of potential antioxidants showed mild and very mild activities compared to standard ascorbic acid.

Co-crystallization of sofosbuvir with sugars for enhanced dissolution rate 索非布韦与糖共结晶,提高溶出率

Sofosbuvir is one of the direct acting antiviral agents which is approved in the treatment of chronic HCV in combination with other agents. The low aqueous solubility of sofosbuvir resulted in slow dissolution which is supposed to be responsible for its low and variable bioavailability after oral administration. Accordingly, the objective of this work was to investigate the effect of co-crystallization of sofosbuvir with hydrophilic sugars on its crystalline structure and dissolution rate. Mixtures of sofosbuvir with hydrophilic sugars at various molar ratios were prepared by ethanol assisted kneading followed by drying. The dry products were then characterized by attenuated total reflectance fourier transform infrared spectroscopy (ATR FTIR), differential scanning calorimetry (DSC), powder X-ray diffraction (XRD) and in vitro dissolution studies. Combined instrumental analysis reflected development of new crystalline species of co-crystal type. This was evidenced by the existence of hydrogen bonding as shown from FTIR spectra, change in the thermal behaviour and appearance of new diffraction peaks in the diffractograms recorded by XRD. The co-crystallization was associated by weakening of intermolecular bonds which resulted in significant increase in the dissolution rate of sofosbuvir. The study introduced hydrophilic sugars as co-crystal co-formers for enhanced dissolution of sofosbuvir.

Optimization of Gelling Agent and Sunscreen Activity Test of 4-OH-chalcone Gel by In Vitro Method 4-羟基查尔酮凝胶的凝胶剂优化及体外防晒活性测定

ABSTRACT The 4-OH-chalcone was suspected to absorb and reduce ultraviolet radiation intensity that exposed to the skin. The 4-OH-chalcone was formulated as gel dosage form using combination of carbopol in content of 0.50-1.00% w/w; propylene glycol in content of 19-20% w/w; and CMC-Na in content of 0.50-1.00% w/w. This study’s purpose was to optimize the three gel bases, evaluate physical properties, physical stability, and activity of 4-OH-chalcone gel as sunscreen by in vitro method. Simplex Lattice Design (SLD) method was used to optimize gel base with Design Expert software version 10.0. Activity of 4-OH-chalcone gel as sunscreen was determined spectrophotometrically by determining Sun Protecting Factor (SPF) value, percent transmission of erythema (% TE), and percent transmission of pigmentation (% TP). Data were analyzed statistically with SPSS Statistic software. Optimum formula of 4-OH-chalcone gel with 0.50% w/w carbopol content, 0.50% w/w carboxymethylcellulose sodium content, and 20,0% w/w propylene glycol content has pH of 5.59, viscosity of 37.4 dPa.s, spreadability of 28.8 cm2, adhesive time of 1.28 seconds, SPF value of 28.65, %, TE value of 16.44%, %TP value of 0.24%, and relatively stable physical properties.
