Practical Effects of Oral Twymeeg for Type 2 Diabetes (T2d) Patient with Special Habit Taking Much Carbs 2型糖尿病患者多碳水化合物的特殊习惯口服Twymeeg的疗效观察

Background: Oral semaglutide (imeglimin) has been prescribed for patients with type 2 diabetes (T2D) as Twymeeg. Case presentation: The patient is 77-year-old male with hypertension, previous cerebral vascular accident (CVA). He continued unbalanced meal with preference of much carbohydrate. In Sept 2021, he became diabetic with HbA1c 8.8%. Results: Imeglimin 2000mg/day was started improving to 6.7% for 2 months. Discussion: Complete blood count (CBC) showed decreased Hb and increased MCV, suggesting macrocytic anemia for lack of vegetables. Much carbohydrate intake may bring T2D. Remarkable diabetic improvement may be from dual mechanisms for increasing insulin secretion and decreasing insulin resistance.

Impact of Spare Time on Improving Anemia Data and the Maintenance of Physical Function in Elderly Caregivers with Chronic Fatigue 闲暇时间对改善慢性疲劳老年照护者贫血数据及维持身体功能的影响

Background: Caregivers reportedly have lower hemoglobin levels compared to non-caregivers. Anemia is associated with decreased physical capacity and increased mortality. Caregivers feel burdened, and experience burnout and fatigue from caring for elderly people on a daily basis. Improving the general endurance of caregivers may lead to improvements in their psychiatric symptoms and quality of life. This study aimed to examine whether securing spare time among caregivers improves anemia symptoms. Methods: The study targeted caregivers aged ≥65 years who cared for a family member at home. Participants were randomly assigned to either an intervention group or a control group. The intervention group was asked to create and secure spare time in their caregiving routine. The control group was asked to continue with their normal caregiving routine. Blood data, including red blood cell count (RBC), hemoglobin (Hb), hematocrit (HT), MCV, MCH, and MCHC, were collected at baseline and six months later and used to assess anemia. Variables for anemia in both groups were compared using t-tests. Results were considered significant when P<0.05. This study was approved by the

Cardiovascular disease risk increases throughout the increased range of blood pressure, including ranges previously considered normal. For example, the person with systolic blood pressure (SBP) of 135 mm/hg has double risk to encounter the coronary heart diseases and stroke than those associated with SBP of 115 mm Hg. Guidelines describe that BP in range of 120-130/80 mmHg is not normal particularly if associated with risk factors and it is termed as pre- hypertension, necessitating screening /identification along with approaches of lifestyle modification.

Introduction: Bacterial vaginosis, or vaginal dysbiosis, is one of the most common vaginal conditions, characterized by a pathological change in the vaginal microbiota. Objective: To establish the effect of the nifuratel - nystatin combination to treat bacterial vaginosis in postmenopausal women. Materials and methods: Cohort study in 57 sexually active postmenopausal women, who consulted for vaginal discharge syndrome, in which the diagnosis of bacterial vaginosis was made; between 2013 and 2016. In a private clinic in Armenia (Colombia). The effect was evaluated by observing the disappearance of symptoms in the first follow-up (seven days after initiating the treatment) and with the Amsel criteria and the Nugent score one month after the end of therapy. The dose used was nifuratel 500 mg - nystatin 200,000 IU intravaginally daily for six days. Convenience sampling was conducted. Descriptive statistics were used. Results: The mean age of the women was 57.36 ± 4.28 years. The most frequent symptom was white-grayish vaginal discharge in 91.22% of the participants, followed by bad odor in 85.96%. In the seven-day follow-up, the symptoms disappeared in 94.73% of the patients. In the month's follow-up, 89.47% of the women reported absence of symptoms; which was confirmed with the Amsel criteria and a median Nugent score of 3. Therapeutic failure, at one month, was 10.52% (n = 6/57). In none of the patients there were adverse reactions. Conclusions: The nifuratel - nystatin combination reports a notable effect to treat bacterial vaginosis in postmenopausal women; being well tolerated and without adverse reactions. Randomized controlled clinical trials of its efficacy and safety in larger populations are required in order to establish recurrence rates.
