: Background: Diabetic retinopathy DR is a complication of DM that affects the blood vessels of the retina and leads to blindness. To control or prevent vision loss, appropriate health education is necessary to encourage those at risk to seek timely and appropriate care. Purpose: To evaluate the effectiveness of an educational program in improving the knowledge of patients having diabetic retinopathy. Design: Quasi-experimental research design was utilized in this study. Sampling; A purposive sample of 80 critically ill patients divided equally into study and control groups. Setting: Ophthalmology Damietta Hospital emergency. Instruments: Knowledge of patients about diabetic retinopathy structured interview questionnaire was used80% of patients in the study group had good knowledge in comparison with 38% of patients in the control group. 76% of study group patients had good knowledge compared to only 38% of patients in the control group Conclusion: Applying the educational program was effective in improving patients' knowledge. Recommendation: Health care providers involving ophthalmic nurses should receive health education about diabetic retinopathy.

: Video games are electronic games that involves human interaction with user interface to generate visual feedback on video device such as I pad, television and computer monitor. Video games are active distraction technique through which the child engage more attentional resources than passive tasks, which involve active problem solving, may be more likely to interfere with pain exacerbating maladaptive thought processes. Implementation of video games play intervention was also one of the resident innovation projects It can carried out with the purpose of reducing pain, fear and discomfort faced by children during hospitalization. Video games are multisensory toys involving audiovisual, kinesthetic, and tactile senses, requiring a child's active cognitive, motor, and visual skills. Video games are improved children’s experience in hospital, it can represent an escape from reality and make the hospital a more pleasant and controllable place for children. They can be created help in a number of ways, distract child from a pain and provide a window for social interactions, most appropriate way to develop empathy and as a channel to understand the world from the viewpoint of children. Conclusion: this study concluded that Implementation of video games had significant effect in alleviation of post-operative pain level among studied preschool children on the posttest compared to the pretest. Recommendation: this study recommended that ongoing health education programs based on application of video games distraction technique for parents should be implemented to manage post-operative pain.

: Ulcerative colitis is a relapsing and remitting inflammatory bowel disease that causes a significant patient's morbidity through its effect on overall quality of life. Purpose: To evaluate the effect of health maintenance program related to ulcerative colitis on selected patient’s outcomes during remission phase. Setting: Medical outpatient clinics at the National Liver Institute, Menoufia Governorate, Egypt. Sampling: A purposive sample of 150 adult patients had ulcerative colitis were selected and assigned alternatively into two equal groups (study and control): 75 patients for each group. Instruments: Four instruments were used; Structured interview questionnaire, Simple clinical colitis activity index, Fatigue severity scale and The RAND 36-item health survey. Results: There was a statistically significant reduction in simple clinical colitis activity index for study group from (2.27±0.24 to 2.02±0.07) post program compared to control group. The mean score of fatigue severity decreased post program from (43.57±10.29 to 24.24±7.49 in study group compared to control group from (42.78±7.36 to 43.02±7.61). Additionally, total mean score of quality of life was significantly improved post program in the study group compared to control group. Furthermore, after 3 months of implementing program there was a significant improvement in weight, BMI and total calories among the study group compared to control group. Conclusion: Health maintenance program had a significant positive effect on reducing mean score of ulcerative colitis symptoms and fatigue, additionally improving quality of life of patients with ulcerative colitis. Recommendations: A supervised health education and maintenance program that includes medication adherence, nutritional guidelines with recommended diet, and stress reduction techniques should be offered in the outpatient clinics to cope with ulcerative colitis

: The patient with hip replacement becomes a chronic patient who require a successful recovery post surgery. Successful recovery requires not only proper surgical technique but also effective pre and postoperative nursing care to improve their activity of daily livings and enhance their quality of life. Purpose: To determine the effect of pre and postoperative nursing intervention on activities of daily livings and quality of life among patients undergoing hip replacement. Setting: The study was conducted at orthopedic departments at Menoufia University and Shebin El-Kom teaching hospitals. Sampling: A consecutive sample of 80 adult patients who were planned for hip replacement surgery was assigned randomly and alternatively into two equal groups, 40 patients for each group. Instruments: Two instruments were used for data collection: Structured interview questionnaire and hip disability and osteoarthritis outcome score. Results: Pre intervention about half of both study and control groups had sever hip disability related function and activity of daily living scores (55.0 % and 57.5% % respectively) that were highly significantly improved among study group to none complains at follow up period (60.0% of study group compared to 5.0% of control group for function and activity of daily living score respectively). Also pre intervention about half of both study and control groups had sever hip disability related quality of life score (47.5 % and 52.5% respectively) that were highly significantly improved among study group to none complains at follow up period (72.5% compared to 7.5% for quality of life score respectively) . Conclusions: Pre and postoperative nursing intervention has a positive impact on improving activities of daily living and quality of life among patients undergoing hip replacement. Recommendations: Supervised health teaching regarding pre and postoperative care should be given for hip replacement patients to improve their activities of daily livings and quality of life.

The successful recovery post hip replacement surgery requires not only proper surgical technique but also effective pre and postoperative nursing care which may be of great value in increasing level of post-operative comfort and reducing or preventing the occurrence of postoperative complications. Purpose: To determine the effect of pre and postoperative nursing intervention on pain and postoperative complications among patients undergoing hip replacement. Setting: The study was conducted at orthopedic departments at Menoufia University and Shebin El-Kom teaching hospitals. Sampling: A consecutive sample of 80 adult patients who were planned for hip replacement surgery was assigned randomly and alternatively into two equal groups, 40 patients for each group. Instruments: Three instruments were used for data collection: Structured interview questionnaire, visual analogue pain scale and observational checklist for performance and complications. Results: The majority of the study and control group (92.5%, 90% respectively) had worst pain level at pre intervention, while 65% of study group compared to 20% of control group had no pain at follow up period. There was statistically significant reductions of postoperative complications occurrence among study group compared to control group at postoperative period. Conclusion: pre and post postoperative nursing care had a positive impact on reducing patient’s pain intensity and complications after hip replacement among study group than control group. Recommendation: Supervised health teaching regarding pre and postoperative care should be given among hip replacement patients to reduce pain and postoperative complications.

: Clinical decision making is a fundamental aspect of nurses` clinical practice and has a direct impact on the health and well-being of each patient. It is a process that nurses undertake on a daily basis when making judgments about the care they provide to patient. Purpose: The present study aimed at clarifying and defining the concept of clinical decision making, its attributes, antecedents, and implication in the nursing practice. Method: In this study, the steps of concept analysis were as follows: Select a concept, determine its aim, various definitions, determine the concept attributes and identify the antecedents and implications. Data Collection: For purposes of this concept analysis, PubMed, Google search engines, Ovid, CINAL and ProQuest, were scanned and searched using the keywords. Conclusion: A nurse‟s clinical decision making is the nurse‟s intuitive ability to facilitate a healthy life for the patient, and it is to actively break through the crisis situation that appears to the patient and make a desirable choice for a challenging alternative. In addition, the nurse professionally assesses and solves problems for the safety of the patient. Recommendations: Encourage periodic staff meeting between nurses and their supervisors to allow them express their feelings, seeking opinions, exchange their experiences during different situations and getting feedback and support. Hospitals need to represent nurses in hospital committees, sharing and participating in decision making about patients` problems and hospital policies. Nursing administrators and leaders should devise a program for staff nurses about decision making skills.

: Trust in the healthcare system, is an important determinant of healthcare utilization, unlicensed personnel and registered nurses have provided hospital-based nursing care for decades. Although ineffective health care providers' role has been associated with poor patient outcomes, little is known of the perspectives of nursing assistive personnel (NAP) role. Purpose: The purpose of this study was to gain insights into the perceptions of NAP role and its relation to patients' health care trust. Method: Comparative, correlational design was conducted at inpatient units where the unlicensed assistive personnel (UAP) work at Menoufia University Hospital and National Liver Institute. Purposeful equal samples of (130) of unlicensed assistive personnel and (130) patients were recruited from previous mentioned hospitals. Data was collected by two instruments, unlicensed assistive personnel role observation checklist and health care trust questionnaire. Results: The highest percentage (90.77%) of assistive personnel in Menoufia university hospital and National liver institute (83.08 %) performed their roles improperly. Whereas, the majority (87.69%, 84.62%) of UAPs not performed their roles properly either under supervision or without supervision in both Menoufia university hospital and National liver institute (78.46%, 76.92%). Also, more than half (53.8) of patients in Menoufia hospital and (52.3%) in National liver institute reported that they were not sure that the unlicensed assistant personnel were technically competent. Moreover, (63.1%% and 53.84%) of patients in the two hospitals had a low trust level regarding the health care. Conclusion: It was concluded that there was a very highly statistically significance negative associations between improper role of unlicensed assistive personnel and health care trust. Recommendations: written job descriptions, and clear definition of roles of unlicensed assistive nursing personnel should be available; specific training program should be developed. Further research should be conducted to develop regulations for educational preparation, utilization of these providers besides the factors that contribute to patients feeling trust in the health care setting

Although ineffective health care providers' role has been associated with poor patient outcomes, little is known of the perspectives of nursing assistive personnel (NAP) role. Purpose: The purpose of this study was to gain insights into the perceptions of NAP role and its relation to patients' health care trust. Method: Comparative, correlational design was conducted at inpatient units where the unlicensed assistive personnel (UAP) work at Menoufia University Hospital and National Liver Institute. Purposeful equal samples of (130) of unlicensed assistive personnel and (130) patients were recruited from previous mentioned hospitals. Data was collected by two instruments, unlicensed assistive personnel role observation checklist and health care trust questionnaire. Results: The highest percentage (90.77%) of assistive personnel in Menoufia university hospital and National liver institute (83.08 %) performed their roles improperly. Whereas, the majority (87.69%, 84.62%) of UAPs not performed their roles properly either under supervision or without supervision in both Menoufia university hospital and National liver institute (78.46%, 76.92%). Also, more than half (53.8) of patients in Menoufia hospital and (52.3%) in National liver institute reported that they were not sure that the unlicensed assistant personnel were technically competent. Moreover, (63.1%% and 53.84%) of patients in the two hospitals had a low trust level regarding the health care. Conclusion: It was concluded that there was a very highly statistically significance negative associations between improper role of unlicensed assistive personnel and health care trust. Recommendations: written job descriptions, and clear definition of roles of unlicensed assistive nursing personnel should be available; specific training program should be developed. : Background: Placental insufficiency is one of the highly prevalent clinical concerns in obstetrics. It is becoming increasingly obvious that its presence significantly affects fetus and placenta, with ramifications on the metabolic, cardiovascular, and neurological development until adulthood. Purpose: To assess the effect of umbilical cord milking (UCM) on selected maternal and premature neonatal outcomes among women with placental insufficiency. Design: Quasi experimental design. Setting: Obstetrics and Neonatal Intensive Care Units (NICU) in Menoufia University Hospital at Shebin El-Koom. Sampling: A purposive sample of 80 pregnant women diagnosed with placental insufficiency and their premature neonates.a simple random sample was done to assign women into study(G1=40) and control G2=40) groups. Instruments: For data collection three instruments were used. Characteristics of pregnant women and premature neonates’ assessment sheet, maternal health outcomes assessment sheet, and preterm neonates’ health outcomes assessment sheet. Results: Premature infants in group I had elevated CD34 (1.13 ± .65 vs .40 ± .20), hematocrit percentage (48.25 ± 5.71vs 40.47 ± 3.37), hemoglobin (Hb) (17.37 ±.95 vs 14.18 ± .78), and peak TSB within 24 h of life than premature neonates in group II. No statistical significant differences were found between studied women in relation to duration of third stage of labor or occurance of postpartum hemorrhage (PPH). Conclusion: Performing UCM for prematures elevated CD34 percentage and Hb levels (at birth and at 2 months) reduced their need for PRBCs transfusion and reduced the duration of oxygen management. However, it did not influence the length of third stage of labor or maternal PPH. Recommendations: Further studies related to UCM should be developed to a larger number for pregnant women and neonate in other units assure the generalizability of the research results.

: Background: Improving women's knowledge, beliefs, and practices based on wound care guidelines should be recognized as being essential to reduce post-cesarean wound infection. The purpose of the study was to assess the effect of wound care guidelines on women's knowledge, beliefs, and practices regarding cesarean section wound healing. Methods: A quasi-experimental design was utilized. Sample: A purposive sample consisted of 200 pregnant women undergoing cesarean section. Setting: The study was carried out at the Obstetrics and Gynecological Department& outpatient clinic at Beni-suef University Hospital in Beni-suef Governorate, Egypt. Instruments of this study were a structured interviewing questionnaire to assess pregnant beliefs in women knowledge and beliefs regarding wound care guidelines and self-care practice tool. Results: There was a statistically significant reduction beliefs in postoperative wound infection in the study group compared to the control group. Conclusion: Women had higher knowledge, beliefs, and practice score about wound care guidelines regarding caesarean section wound healing. Recommendations: There is more knowledge, beliefs and practices related to cesarean wound healing that need to be corrected.
