One-year clinical outcomes following non-surgical treatment of peri-implant mucositis with adjunctive diode laser application. 辅助二极管激光非手术治疗种植体周围粘膜炎后一年的临床结果。

Background: Limited information is available on the application of diode laser in the treatment of peri-implant diseases. The aim of this study was to investigate the clinical efficacy of the adjunctive application of diode laser in the non-surgical treatment of peri-implant mucositis during a 12-month follow-up period.

Methods: The sample was composed of 73 systemically healthy patients with one implant diagnosed with peri-implant mucositis (bleeding on probing [BoP] with no loss of supporting bone). Implants were randomly assigned to mechanical debridement with hand and powered instruments and 980-nm diode laser application (test group, N.=38) or mechanical debridement alone (control group, N.=35). At the completion of active treatment patients were included in a periodontal maintenance program. Recalls were provided every three months in both treatment groups for reinforcement in oral hygiene instructions and professional implant cleaning with rubber cups. Baseline parameters were repeated at 3 and 12 months postoperatively.

Results: Intragroup analysis showed that plaque index, BoP and probing depth presented statistically significant improvements when compared with baseline values (all P<0.001). No statistically significant difference in clinical outcomes was observed between treatment groups at each time point. At 12 months no significant difference in the percentage of sites showing BoP resolution was observed between test (60.9%) and control treatment (52.6%), as well.

Conclusions: Based on the present results, the adjunct use of diode laser showed little but not statistically significant additional benefits in the treatment of peri-implant mucositis after an observation period of one year.

COVID-19: how to decrease the risk of infection in dental practice? COVID-19:如何降低牙科诊所感染的风险?

A new pandemic classified by the World Health Organization and called COVID-19, is causing widespread respiratory infections and deaths in several countries on the five continents. Although it does not have a high lethality rate, this new virus can present rates of complications and hospitalizations in the intensive care unit in up to 20% of patients, especially the elderly and those with compromised health, which can cause a collapse in the health system national public health. Dentistry is one of the most vulnerable professions due to work in the oral cavity, one of the areas with the highest concentration of the virus, therefore having an important role in controlling the disease. The objective of this work is through a review of the current literature, to establish conducts that can reduce the contamination by COVID-19 between the population and the dental team during the service in the public and private health system.

Socioeconomic behaviors on dental professions: a Google Trends investigation in Italy. 牙医职业的社会经济行为:谷歌趋势在意大利的调查。

Introduction: Dental profession has expanded, and the knowledge that oral health affects systemic health is of greater concern to patients. The purpose of this study is to understand the demand for dental treatments in Italy, knowing if there is any correlation between socioeconomic condition and oral rehabilitation typology. Moreover, focusing on the requested rehabilitations type and on finding alternatives, unconventional solutions.

Evidence acquisition: The research was conducted using an important research database defined Google Trends, with search terms such as "dolore denti" (toothache). A statistical analysis has been conducted in this study evaluation Pearson Correlation Coefficient.

Evidence synthesis: The results divided region by region provide data that reflect the socio-economic conditions of the population concerned. Other results have been obtained from government source, to learn about the economic situations of the individual regions, for this reason the GDP was examined. Furthermore, there are important results regarding the parasanitary professions to which patients, for economic reasons, tend to refer.

Conclusions: This study will surely be an excellent starting point to investigate professional abuse and above all to assess the needs of the different regions.

Maxillary dento-skeletal outcomes after orthopedic forward (class III) and backward (class II) traction in growing subjects. 成长期受试者矫形前牵引(III级)和后牵引(II级)后的上颌齿骨结果。

BACKGROUNDThis retrospective study investigated the treatment effects produced by two orthopedic appliances, an extraoral orthopedic traction on Stephenson plate (SP) in skeletal Class II malocclusion and Delaire Facial Mask (FM) in skeletal Class III malocclusion.METHODSFM Group comprised 25 patients (13 males; 12 females, mean age 9.3 years, CS 1-2) undergoing Delaire Facial Mask therapy (mean treatment duration 1.5 years ± 1.2, 700-800 gr per side). SP Group comprised 24 patients (9 males; 15 females, mean age 10.5 years ± 1.9, CS 1-3) treated with extraoral orthopedic traction on Stephenson plate (mean treatment duration 1.6 years ± 0.8, 700-900 gr per side). For all subjects lateral cephalograms before (T0) and after (T1) therapy were obtained and traced. Descriptive statistics, Shapiro-Wilks and T test were calculated for each variable.RESULTSA mesial movement of the maxillary bone of 2.4 mm in the FM group and a distal movement in the PS group (0.4 mm) was found. SNA° was reduced 1.4° after SP and increased 0.7° after FM. The overjet decreased by 4 mm after SP, while it grew after FM by 2.4 mm, as well as molars relationships (4.5 vs 0.8 mm).CONCLUSIONSFacemask induced greater skeletal movements than extraoral traction on Stephenson plate. PS produced a limited downward movement of the whole maxilla together with notable incisors and molars back corrections. Based on our results, it could be speculated that orthopedic mesial traction of maxillary bone may result easier than the distal movement.

Bisphosphonate-associated osteonecrosis of the jaw: systematic review. 双膦酸盐相关性颌骨坏死:系统综述。

OBJECTIVEThe purpose of this systematic review was to determine the possible risk factors related to pathophysiology of Bisphosphonate-Related Osteonecrosis of the Jaw (BRONJ) and identify adequate treatment strategies available and success rates.EVIDENCE ACQUISITIONWe performed a search for publications about the treatment of BRONJ, published between 2003 and 2018 in the PubMed/Medline data base using the key words: 'Bisphosphonate-Associated Osteonecrosis of the Jaw' OR 'Bisphosphonate Osteonecrosis' OR 'BRONJ', based on the list of MeSH and DeCS.EVIDENCE SYNTHESISAccording to preestablished criteria for data collection concerning the treatment of BRONJ, we found 19 articles covering a total of 400 patients. Treatments that showed good outcomes were Ozone, PRF, PRP/Debridement/Laser bio-stimulation, Laser surgery and Laser surgery/Laser bio-stimulation. HBO did not achieve good results and was used in only 10 patients. BRONJ is a rare condition in patients with osteoporosis/other pathologies using BP orally. These patients had long exposure time and cumulative doses of BPs until onset of the lesion. The oncological patients were exposed to more potent intravenously administered BPs such as pamidronate and zoledronate. These patients had a shorter exposure time until onset of the lesion.CONCLUSIONSThe treatment of BRONJ is still under debate and there are promising treatments that need randomized trials with larger numbers of patients to confirm their results. Patients receiving BPs or those who will start treatment should be encouraged to perform preventive dental treatment and maintain good oral hygiene.

Psychological reactions to COVID-19 and epidemiological aspects of dental practitioners during lockdown in Italy. 意大利封锁期间牙科医生对新冠肺炎的心理反应和流行病学方面。

BACKGROUNDDue to droplet production and exposure to saliva and blood, dental practitioners are at high risk of COVID-19 contagion during their routine procedures. The aim of this study is to investigate the behavior of Italian dentists and to analyze their reactions in relation to Sars-CoV-2 pandemic professional restrictive measures.METHODSAn online structured survey composed of 40 questions has been sent to dental practitioners all over Italy to investigate their behavior and to analyze their reactions in relation to Sars-CoV-2 pandemic restrictive measures introduced by the Italian national administrative order of 10 March 2020 (DM-10M20).RESULTS1109 dentists replied. To assess concerns and psychological responses the sample was divided into two groups based on the number of cases registered in their work area. In the first group were included all the responders working in the Italian regions that had more than 15,000 confirmed cases of COVID-19 as of April 29, 2020. The second group included responders working in the Italian regions that had less than 15,000 confirmed cases. The 45.2% of the respondents showed minimal anxiety, 34.5% showed mild anxiety, 13.9% showed moderate anxiety, while 6.4% showed a score indicative of a severe level of anxiety.CONCLUSIONSThe COVID-19-related emergency condition had a highly negative impact on dental practices in Italy. Those who completed the survey reported practice closure or reduction during the lockdown, and a high level of concern about the professional future for all dental practitioners. Concerns related to professional activity were accompanied by severe anxiety levels.

BACKGROUNDPatients with severe dentofacial deformities are more susceptible to psychological distress since they are more likely to be emotionally unstable, less sociable and have higher levels of anxiety and neuroticism. The aim of this study is to assess the relationship between dentofacial deformity, negative social comparisons, anxiety and discomfort in social situations due to face appearance in two independent samples.METHODSThe sample consisted of 136 patients (Group A- 90 college students; Group B- 46 patients with dentofacial dysmorphosis that require orthognathic surgery). The impact of dentofacial deformity was evaluated through the following questionnaires: scale of social comparison through the appearance of the face and the scale of anxiety and discomfort in social situations due to the appearance of the face.RESULTSGroup B showed higher levels of anxiety and discomfort than the college student's sample (p = .004). Individuals with dentofacial dysmorphosis presented a greater degree of anxiety and discomfort than individuals without dentofacial dysmorphosis (p = .002). Finally, skeletal pattern and social comparison predicted anxiety and discomfort in the group A [F (1.88) = 7.270; p<.05], but only social comparison emerged as a significant predictor of anxiety and discomfort in the group B [F (2, 42) = 4.463; p<.05].CONCLUSIONSPatients with dentofacial deformity have higher levels of anxiety and discomfort. This deformity can be reduced with orthodontic-surgical treatment, which can promote improvements in social and interpersonal well-being.

Standardization of a cone beam computed tomography machine in evaluating bone density: a novel approach. 评估骨密度的锥形束计算机断层扫描机的标准化:一种新方法。

BACKGROUNDAdvent of Cone Beam Computed Tomography (CBCT) in dentistry has brought us to a new era of precise imaging. Radiographic evaluation of a CBCT image is more informational when compared to CT. The density measurements in CBCT images are based on greyscale values, which are more accurate in CT and these values are inconsistent across various CBCT machines. Hence, we aim at standardizing a single CBCT scanner to evaluate or determine tissue density from the greyscale values.METHODSA total of 8 halves of undamaged, dry goat mandibles are included in the study. Scans of the bone are obtained using the KODAK CBCT unit and the PHILLIPS CT machine respectively. Densities are evaluated at 96 points on both the CT scans and the CBCT scans, respectively, using the Radiant Dicom viewer. The obtained data is entered into the excel spreadsheet and subjected to statistical analysis.RESULTSThe greyscale values are obtained from each of the CBCT scans. Hounsfield units are calculated from CT images coinciding with the same points on CBCT scans. The collected data is subjected to linear regression analysis and an equation is derived to determine Hounsfield units (calculated HU units) from greyscale values of CBCT images. We found no significant difference between the mean original HU units and the mean calculated HU units, thus making the equation reliable for calculating HU units from CBCT greyscale values.CONCLUSIONSOur results conclude that the technique was effective in calculating the Original density of tissues using grey standards of CBCT scans.

Background: Telemedicine can provide valuable strategies to deliver high-quality training to young doctors and students, as well as to discuss complex clinical cases and follow patients. However, in many cases, similar benefits are limited by the costs due to the expensive hardware resources and optimal infrastructure required. This study presents a novel protocol to improve the accessibility of telemedicine services into a modern hospital department. The protocol is referred to as "SEF" (Smart videosurgery, Easy teleteaching, Fast teleassistance) and was developed in an Oral and Maxillofacial Surgery department. The aims of this study were: 1) to provide a solution to the high initial costs of the configuration of a telemedicine network; 2) to support the simultaneous transmission of multiple video streams; 3) to adopt a modular architecture that amplifies the distance of transmission on the basis of the network infrastructure (department, academic hospital, city, region), thus permitting distance delivery of visual content without any quality loss.

Methods: A local Wi-Fi network was created to connect a wireless presentation device with smartphones, laptops, tablets, and a visualization platform represented by an interactive whiteboard (IWB), allowing to display multiple video streams at the same time. The SEF Project was applied during nine operations and a sample of 90 students (10 students for each procedure) was chosen to evaluate the impact of distance telementoring on their learning performance, in contrast with traditional surgical education.

Results: The interface was successfully tested in all surgeries without any quality loss for their whole duration. High resolution and fluidity were confirmed in the recorded clips as well. The impact of telementoring system was assessed by groups of 10 students for each procedure who were administered a questionnaire, and a total satisfaction index of 9/10 was achieved.

Conclusions: SEF protocol appeared to be a stable and highly customizable interface, and has the potential to empower many centers with accessible telehealth systems.

Following the publication of the Italian Ministerial recommendations relating to dentistry in Phase 2 of the COVID-19 pandemic (focusing on operational protocols for all dental staff), we believe that the patient/dentist relationship should increasingly take into account the heightened fears and anxious thoughts of patients. This particularly regards patients who are about to undergo dental work. Moreover, dentists should also pay close attention to recent events, which have determined the new recommendations regarding SARS-COV-2 biocontainment. Furthermore, the authors of this paper consider it appropriate to make suggestions and develop interventionist techniques regarding the interface with the patient starting from the initial consultation. The latter is invariably determinant in establishing clear communication of the Ministerial recommendations in encouraging a relaxed atmosphere with the patient. This interface is also a decisive factor in promoting patient empowerment, including specifying the time period envisaged for treatment in the new COVID-19 era in as calm a manner as possible. Such an approach will have a positive impact on the dentistry team.

This particularly regards patients who are about to undergo dental work. Moreover, dentists should also pay close attention to recent events, which have determined the new recommendations regarding SARS-COV-2 biocontainment. Furthermore, the authors of this paper consider it appropriate to make suggestions and develop interventionist techniques regarding the interface with the patient starting from the initial consultation. The latter is invariably determinant in establishing clear communication of the Ministerial recommendations in encouraging a relaxed atmosphere with the patient. This interface is also a decisive factor in promoting patient empowerment, including specifying the time period envisaged for treatment in the new COVID-19 era in as calm a manner as possible.
