近年の加速する高齢化社会において,椎間板変性の発症機序を解明·整理することはますます重要となり,臨床および基礎分野においてさまざまな研究が行われている.そこで,今回われわれは最近注目されている老化関連物質である終末糖化産物(AGEs)と椎間板変性の関係について検討し,髄核におけるaggrecan分泌がAGEsの濃度かつ作用時間依存性に減少することを明らかにした.さらに,炎症性サイトカイン(IL-1β)が椎間板細胞のRAGE発現を上昇させることを確認しており,炎症による椎間板変性の悪性サイクルの可能性を示唆した.現在のところ,この機序がAGE化タンパクによる直接的な作用なのかRAGEを介する作用なのかは明確にされていないが,いずれにせよ,AGEsの蓄積が生体内のさまざまな場所で,加齢性疾患の原因となっている可能性は大きい.

Evaluation of treatment of epidural block for lumbar disc herniation 硬膜外阻滞治疗腰椎间盘突出症的疗效评价

腰椎椎間板ヘルニア(LDH)に対する硬膜外ブロック(EDB)の治療評価を行った.対象は46例,平均年齢は46歳で治療の内訳はEDB単独で治療終了したもの33例をEDB有効群とし,EDB後根ブロック,手術を行った13例を非有効群として,年齢,罹病期間,EDB回数,EDB終了時VAS,初回・終了時SLR,FFDを比較した.評価法はJOA:自覚+ADL(23点),VAS,RDQ偏差得点,SF-36を使用した.EDB有効群と非有効群の比較でEDB終了時VAS,SLRのみ有意差を認めた.JOA,RDQ偏差得点はブロック前有効群と非有効群で有意差を認めた.EDB有効例の治療前後におけるSF36下肢尺度は全体的健康感を除いては有意の改善を示した.

本研究の目的は,端坐位後屈時における腰痛の有無と骨盤の動きを比較し,関連性を明らかにすることである.対象は,端坐位での後屈時に腰痛がある患者10名(p+群)と,疼痛のない患者10名(p-群)とした.はじめに,端坐位での骨盤前傾角度を左右とも水平角度計で測定した.次に端坐位のまま後屈するように指示を与え,疼痛の有無を確認すると同時に骨盤前傾角度を測定した.端坐位での骨盤前傾角度は,有意差がなかった.後屈時の骨盤前傾角度は,p+群はp-群に比べ有意に後傾していた.また,p-群はすべて両側5°以上だったのに対し,p+群は10例中7例が両側または片側が5°未満であった.結果より,端坐位での痛みのない後屈動作を行うには,少なくとも5°以上の骨盤前傾角度が必要であり,後屈時に腰痛が生じる患者に対し,端坐位での後屈時痛の有無と骨盤前傾角度を評価することは有用であることが示唆された.

Intramuscular oxygenation of exercising trunk muscle in elderly persons 老年人躯干肌运动的肌内氧合

We evaluated the relative changes in tissue oxygenation during and after exercise in human lumbar muscle using near-infrared spectroscopy. Oxygenation changes in the trunk muscle were measured non-invasively using near-infrared spectroscopy (NIRS). The subjects analyzed were 111 volunteers over 60 year-old, comprised of two groups; the low back pain (LBP) group who complained of low back pain lasting more than 3 months and the non-LBP group who had no complaints of back pain. A near-infrared spectrophotometer was applied on the back, while maximally extending and bending the lumbar spine for 15 seconds, and isometric exercise in a standing position for 10 seconds. On lumbar extension, the amount of relative change in oxygenated hemoglobin (Oxy-Hb) and the tissue oxygen saturation index (SdO2) were significantly less in the LBP group than in the non-LBP group. No significant differences in deoxygenated hemoglobin (Deoxy-Hb) were found between the two groups on both extension and flexion. Relative changes of Deoxy-Hb and SdO2 were significantly higher in the LBP group than in the non-LBP group. The increase of Oxy-Hb during lumbar extension is conceived as the most available parameter in NIRS measurements, taking into account the performance of exercise even in patients with severe low back pain.

Relation between focus of low back pain and pain evoking motion 腰痛病灶与致痛运动的关系

Comparison of radiological findings and neurological findings in severe lumbar spinal canal stenosis 严重腰椎管狭窄的影像学表现与神经学表现的比较

腰部脊柱管狭窄症では,硬膜管の圧排が強い症例ほど神経学的症状が重症化する傾向にあり,神経学的所見と画像上の狭窄部位の高位が一致しない症例が存在する.腰部硬膜管内は馬尾であり,馬尾は神経根の集まりであるという解剖学的構造に一因が考えられる.脊髄造影にてtotal blockまたはsubtotal blockを呈した高度狭窄例に限定し,腰部脊柱管高度狭窄例において,神経学的所見と画像所見の高位の相違をretrospectiveに比較検討した.手術施行した腰部脊柱管狭窄症例中,脊髄造影像でtotal blockまたはsubtotal blockを呈した高度狭窄例38例を対象とした.神経学的所見は,疼痛·感覚障害,筋力低下について,画像所見は脊髄造影像における最狭窄部位を画像障害高位と評価し,神経学的障害高位と画像障害高位の相違を比較検討した.神経学的所見と脊髄造影所見の高位不一致例は13例34.2%,その詳細はL3/4狭窄によるL5障害が最多であり,臨床上重要と考えられた.
在腰部椎管狭窄症中,硬膜管压排越强的病例,其神经学症状就越严重,有时会出现神经学的观察结果与影像上狭窄部位的高位不一致的病例。腰部硬膜管内是马尾,马尾是神经根的集合,这是解剖学构造的原因之一。脊髓造影限于呈total block或subtotal block的高度狭窄例,腰部脊柱管高度狭窄例,retrospective比较讨论了神经学的观察和图像的观察的高位的差异。在实施手术的腰部椎管狭窄病例中,以38例脊髓造影显示为total block或subtotal block的高度狭窄为对象。神经学意见针对疼痛、感觉障碍、肌力低下,影像学意见将脊髓造影的最狭窄部位评价为影像学障碍高位,比较讨论了神经学障碍高位和影像学障碍高位的差异。神经学和脊髓造影的高位失配病例为13例34.2%,其中以l3 /4狭窄引起的L5障碍最多,在临床上很重要。
