Pre-exposure prophylaxis (PrEP) refers to the use of antiretroviral drugs to prevent one from acquiring human immunodeficiency virus (HIV) infection when engaging in high-risk sexual behaviour. Adequate PrEP initiation is key in the clinical outcome and survival of not only vulnerable and high-risk populations, but also to ensure that a society which is free of HIV, is built.

Insomnia is an important public health burden and is the most ubiquitous sleep disorder in the general population, requiring accurate diagnosis and effective treatment. Sleep hygiene and cognitive behavioural therapy form the foundation of management. In addition, various pharmacological entities are at hand to augment insomnia disorder. Acute insomnia requires short-term management with appropriately indicated hypnotic agents, while chronic sleeping difficulties benefit more from antidepressants. This article informs the reader about the currently available sleeping agents in South Africa, and may not include more effective or potent agents used in other parts of the world that are not yet accessible for local prescription.

Serotonin is said to determine our emotions, moods and affect. The physiology of serotonin implies it in the regulation of biological functions in line with our mood, like accelerated heart rate and sweating triggered by feeling flustered. Serotonin has multiple pharmacological actions in the body and alterations in serotonergic function may account for behavioural disturbances commonly observed in the elderly. The clinical effectiveness of selective 5-HT reuptake inhibitors (SSRIs) is believed to be mediated through enhancement of extracellular 5 HT. Various studies have shown that the SSRIs are better tolerated in older people compared with the tricyclic antidepressant or serotonin-norepinephrine reuptake inhibitors (SNRIs). It is obvious that serotonin plays a significant role in the emotional wellbeing of people – whether they are young or old and there are various ways of increasing serotonin in the body in order to create a feeling of wellbeing and content.
