A CASE OF NEGLECTED SNAKEBITE COMPLICATED BY NECROTIZING FASCIITIS AND LOWER LIMB AMPUTATION 被忽视的蛇咬伤并发坏死性筋膜炎及下肢截肢1例

Background: Snakebite is considered an underreported health issue in the rural areas of the 3 rd world countries that was recently recognized by the world health organization as a neglected tropical disease. Case presentation: In the following case study, we are discussing a case of a female patient who was bitten by an unidentified snake where she sought non-specialized medical advice who prescribed non-specific management. Moreover, a traditional local snakebite therapist performed multiple local incisions. The condition was complicated by the development of necrotizing fasciitis in her right foot that ascended to the level of the thigh. Surgical consultation was done where the patient received Rocephin 2g24H and Tavanic 500mg/24h in addition to surgical debridement of the necrotic tissue. However, below-hip limb amputation was performed as a salvage procedure for the patient life. Conclusion: the early administration of antivenom could prevent the development of complications by neutralizing the effect of the venom. This could be achieved through providing training to primary health care physicians working in rural and remote areas on the proper management of snakebites. In addition, it is essential to ensure a sufficient supply and proper storage of the antivenom in those areas.

In Egypt, Forensic medicine have taught over decades, and the curricula have changed several times. However, teaching and learning methods mainly taught by traditional methods . This review outline the importance of reforming teaching& learning methods in undergraduate curriculum in Egypt to prepare medical graduate students for better understanding their medico-legal duties as doctors in promoting the wellbeing of their patients

Outcome of Chest Trauma Among Covid-19 Positive Cases Compared to non COVID Patients In Sohag Governorate 苏哈格省COVID -19阳性病例与非COVID -19患者胸部创伤结局的比较

Background : COVID-19 is a potentially fatal and highly contagious disease, both factors making it a significant public health problem. trauma is a leading cause of utilization of healthcare, disability, and mortality. The presence of COVID-19 infection may worsen the outcome of chest trauma Objective : This study aimed to describe the complications and outcome of chest trauma in patients with confirmed COVID-19 pneumonia and compare these findings with that of chest trauma in negative COVID-19 controls in Sohag. Patients and methods: This prospective observational study was conducted in the Department of Thoracic Surgery, Sohag university hospital. The study included 60 patients of middle age, of both genders, admitted with chest trauma. They were divided into 2 groups: Group I: 30 patients infected with COVID-19 had chest trauma and Group II: 30 patients with negative COVID-19 had chest trauma. Management of cases and data collection were done. Results: COVID-19 infection increase complications and mortality in chest trauma. Pneumonia and ARDS were common complications associated with higher mortality. Conclusion; COVID-19 infection increase complications and mortality in chest trauma.

Background: Nandrolone-decanoate (ND); is an anabolic steroid. Its toxic effects on the heart as arrhythmias, acute myocardial infarction, and sudden death have been recorded. Moringa oleifera extract (MOE); is a herbal extract used to treat various diseases due to its antioxidant effect. Aim; This study was designed to evaluate the efficacy of MOE as a protective agent against cardiotoxicity induced by ND. Material and methods ; Thirty-two adult male albino rats were allocated into four groups. Control group; MOE treated group; rats were given MOE by oral gavage (400 mg/kg/day) for four weeks. ND group; rats were injected ND S.C; at a dose of 20 mg/kg/week for four weeks. ND+MOE-treated group; rats simultaneously administrated ND; (20mg/kg/week S.C.) with MOE (400 mg/kg/day by oral gavage) for four weeks. Serum levels of cardiac enzymes were estimated. Also; MDA, GSH, SOD, IL-6, TNF- α, and IL-10 levels in cardiac tissue were estimated. Cardiac samples were processed to light, an immune-histochemical study using anti-P53 and anti-CX43 antibodies as well as electron microscopic examination. Morphometric; and statistical studies were done . Results ; In the ND group, serum levels of cardiac biomarkers and cardiac tissue levels of MDA, TNF-α, and IL-6 were significantly increased with significant diminution in the activity of catalase, SOD, diminished GSH, and IL-10 in the cardiac tissues. Light; microscopic examination showed wide endomysium, inflammatory cell infiltration, and lacerated and wavy cardiac muscle fibres were observed. There was a marked deposition of collagen fibres at areas of interrupted muscle fibres. Positive; P53 immuno-expression in the nuclei and decreased expression of CX43 were observed. Ultra-structurally; interrupted myofibrils, dilated sarcoplasmic reticulum, and a disorganized intercalated disc was observed. In (ND+MOE) group; showed marked amelioration in the cardiac architecture and ultrastructure, down-regulation of P53, and CX43 immuno-expressions, and a significant reduction in serum level of cardiac biomarkers, and cardiac tissue levels of MDA, TNF-α and IL-6 with a significant increase in activity of catalase, SOD, diminished GSH and IL-10 in the cardiac tissues. Conclusion ; MOE could alleviate the cardio-toxic effects induced by ND in rats through anti-inflammatory, anti-oxidant; and anti-apoptotic effects.

DETERMINATION OF STATURE FROM TYPE OF FINGERPRINTS AMONG EGYPTIAN 根据埃及人指纹类型测定身高

Introduction: Stature determination is an important parameter in person identification. Fingerprints form a valuable physical evidence in crime investigation. The use of fingerprints in identification currently focuses on the patterns of fingerprints. The current study aimed to assess the ability of stature identification from the type of fingerprints among Egyptian. Material and Methods: The study was conducted on 150 adult Egyptians in the age group above 18 year. Stature was measured by standard methods using a portable body meter measuring device. The fingerprint was taken by rolling the thumb of the right hand on paper from outward to inward. The relation between fingerprint patterns of right hand thumb of individuals with their heights was evaluated and analyzed. Results : Among the studied cases, loop was the most common fingerprint main type representing followed by arch then whorl. While ulnar loop was the commonest fingerprint sub type. Whorl showed the higher mean stature (172.32 ± 8.25) followed by arch then loop. As regard the sub types, plain whorl showed the higher mean stature (176.0 ± 7.62) followed by double loop whorl. Whorl showed the highest mean stature among males while loop showed the highest mean stature females and there was a significant difference (p<0.001). Conclusion: fingerprint pattern can provide useful information for stature determination among male and female adult Egyptians.

Fingerprints form a valuable physical evidence in crime investigation. The use of fingerprints in identification currently focuses on the patterns of fingerprints. The current study aimed to assess the ability of stature identification from the type of fingerprints among Egyptian. Material and Methods: The study was conducted on 150 adult Egyptians in the age group above 18 year. Stature was measured by standard methods using a portable body meter measuring device. The fingerprint was taken by rolling the thumb of the right hand on paper from outward to inward. The relation between fingerprint patterns of right hand thumb of individuals with their heights was evaluated and analyzed. Results : Among the studied cases, loop was the most common fingerprint main type representing followed by arch then whorl. While ulnar loop was the commonest fingerprint sub type. Whorl showed the higher mean stature (172.32 ± 8.25) followed by arch then loop. As regard the sub types, plain whorl showed the higher mean stature (176.0 ± 7.62) followed by double loop whorl. Whorl showed the highest mean stature among males while loop showed the highest mean stature females and there was a significant difference (p<0.001). Variation of Y-chromosomal STRs in Yezidi and Chaldean populations in Iraqi Kurdistan 伊拉克库尔德斯坦地区雅兹迪族和迦勒底族人群y染色体STRs变异

Background: Many ethnic groups live in the northern part of Iraq which represents the Iraqi part of Kurdistan. Short tandem repeats are widely used in population genetics and forensic science. Objective: This research aims to analyze the Y-chromosomal STR markers of two ethnic groups living in Iraqi Kurdistan, Yezidi, and Chaldean groups. Methodology: Samples of peripheral blood from a total of 44 unrelated males (22 for each ethnic group) were taken. DNA was extracted by using a DNA Extraction kit and analyzed for eight Y-chromosomal short tandem repeats (Y-GATA-H4, Y-GATA-C4, DYS458, DYS456, DYS448, DYS437, DYS392, and DYS19). Then, the PCR products were run on 10% polyacrylamide gel and stained by silver nitrate. The results were analyzed by Power marker V3.25 and the dendrogram was created by Mega X software. Results: The highest diversity was observed at Y-GATA-C4 (GD: 0.81) while the lowest diversity was observed at DYS456 (GD: 0.64) in the Yezidi group. In the Chaldean group, DYS458 (GD: 0.88) was the most diverse, while the least diverse marker was Y-GATA-H4 (GD:0.66). The marker Y-GATA-C4 was found to be the most informative marker in both groups with a PIC value of 0.8605. Conclusions: The study confirmed the high discrimination ability of the Y-chromosomal STRs analysis and provided a dataset on these two ethnic groups of Iraqi Kurdistan. The dendrogram of Yezidi and Chaldean datasets reveals that the Yezidi individuals are more closely related to each other as compared to the Chaldean group because intermarriage among Yezidi people is more than that among the Chaldean individuals .
