Imaging in dentistry has been limited generally to traditional applications of radiography and photography. Recent technical advances stimulated by parallel development of computers include digital subtraction radiology and magnetic resonance imaging. All of these modalities share certain fundamental constraints which limit effectiveness in biomedical applications. In sequence of encounter these are (1) ambiguous signal definition, (2) inappropriate data sampling strategy, (3) inadequate signal strength relative to intrinsic sources of noise, and (4) misinterpretation of sampled data. Controlled tests suggest that the first two of these factors largely control performance of common dental diagnostic tasks.

The Personal Health Data Recording System (PHDRS) i s a system that an individual person controls and maintains a portable recording mediun which contains all the health data and information of himself digi - tized and recorded in course of time. As a recording medium we plan to use an erasable optical magnetic disk of 5.25" in diameter. With the introduction of this system a comparison of the past examination data becomes easy to detect an irregularity for possibly obtaining a diagnosis in an early stage, avoiding unnecessarily repeated examinations in a short time interval. Besides, this system offers information relating to the treatment in other medical institutions which helps a doctor in charge for a correct treatment. Furthermore by applying this system a doctor can make his own image file for research and education as well as to make a reading device for mass screening.

First the use of patient data in a hospital is considered in general. The different handling of alpha-numeric data on one hand and images on the other hand is discussed. This leads to a comparison of the concepts integrated HIS and PACS. The present situation as to HIS realization is described and some lessons to be learned for PACS development are identified. The need for a relation between HIS and PACS is emphasized and some remarks are made on the realization of a connection.

Implementation of a digital imaging network in routine clinical use is a difficult task. Not only the high technical requirements, but especially the complexity of the organization of the diagnostic information flow in a hospital makes commitment essential in PACS implementation. The application of project management with a dedicated project team is a good approach to establish this complex endeavor. The different phases of a project for implementation are initiative, definition, design, preparation, realization, and evaluation and are exemplified by what we learned from the Dutch PACS project.
