The National Library of Medicine has taken an active role in the use of electronic media for the production of image libraries. Problems have been encountered in the adequacy of currently available technology to render an image of acceptable quality. Interim solutions are being used. The question is whether or not NLM should take a proactive role in the production and dissemination of image libraries through unstable, rapidly evolving, largely untested technology thereby adding some stability and direction to that technology's development.

A broadband switched public network is needed to allow image management to realize its full potential. The worldwide telephone network-actually a network of networks - provides any-to-any communication with high quality, availability and reliability and exemplifies all the characteristics needed, except for full digital connectivity and wide bandwidth. This paper describes how the public network is closing this gap by evolving toward Broadband ISDN.

Current technology and perspective of future technology related to IMAC are discussed in terms of hardware as well as software. Items covered are: computers, communication, file media, data acquisition, display, image data compression, AI, opto- electronic devices, software, standardization, and future IMAC. Important specific items such as calculation speed, communication speed, archiving capability, and bit cost are quantitatively depicted and predicted. Three axes, communication speed axis, data volume axis, and time axis are taken as x, y, and z axis respectively and key technologies are expressed by vectors in this xyz space. Performance of present IMAC and prediction of it are illustrated as the sum of these vectors. Deficiency in technology, standing problems to be solved and obstacles to be broken through are pointed out and the methods in what way they could be done with are proposed. Finally, integrated IMAC systems in future inside and outside of hospitals are proposed being based upon conclete development prediction such as functional assignment of LAN for each work and ISDN application.

Technical review on advances in PACS is made briefly. New' technologies such as magneto-optic disk, digital signal processor, neuro computer, fuzzy inference and optical computer are forecast in relation to PACS. Focussing on these, future perspective on digital imaging, diagnostic image workstation, memory and image database, and communication and network are investigated. The author forecast what the technical situation in PACS might happen in the year of A.D.2000.

Knowledge of how radiology relates to general medicine is vital to understanding both the problems that Image Management and Communication Systems (IMAcs) may solve and those that IMACS may create. For such understanding, one must also know something of the pressures for change that bear universally upon the current form of health care delivery. To simplify problem study, this paper will first examine the present; second, the coming period of change; and finally, the future. Current problems will be illustrated and examined in terms of processes tending to undermine stability. The issue of problems created by solutions and solution processes will be discussed. Finally, the prospect that IMACS may contribute to a shift of paradigm for medical practice will be raised.
