Aims: 'Poly C' stretch extending from 303 to 315 nucleotide positions known as (D310) by GeneBank within the non-coding region HVR-II of mitochondrial DNA (mtDNA) has been identified as a mutational hotspot of primary cancer. We aimed to find this mutation in Bangladeshi breast cancer patients and control samples for screening the changes in poly C stretches. Materials and Methods: We have analyzed a total of 98 breast cancer blood samples and 98 healthy control blood samples and extracted DNA from blood samples for direct sequencing using ABI®3130 Genetic Analyzer. Results: We observed 70.41% C insertion at 310 regions (P<0.001, OR = 16.30 and 95% CI=7.83-33.93) in breast cancer patient whereas it is present only in 12.24% healthy individuals. No alterations were observed in 29.59% breast cancer samples. We also check the mutation pattern at D315 regions, but no significant observation was found (P=1.00). Conclusions: This is the first study from Bangladeshi breast cancer (BC) patients indicating a relatively high frequency of D310 mutations, which suggests that mtDNA instability at D310 may be a common characteristic of BC, study also supports the hypothesis that mtDNA D310 screening may represent additional blood based biomarker for breast cancer prognosis.

Adenoid cystic carcinoma is a rare form of adenocarcinoma, which is a type of cancer. It can exist in many different body sites and most often occurs in the areas of the head and neck in particular the salivary glands. Adenoid cystic carcinoma has also been reported in the breast, lacrimal gland of the eye, lung, brain, bartholin gland, and trachea. Here symptoms, diagnosis and treatment of Adenoid Cystic Carcinoma (AdCC) are discussed.

Background: Triage in an emergency department (ED) plays a pivotal role as the volume of ED visitors is unpredictable. All ED patients are triaged to make sure that patients with urgent or life-threatening conditions are seen immediately while others with more stable conditions are safe to wait. Purpose: To examine the Canadian Triage and Acuity Scale (CTAS) guidelines to determine if the urgency of oncological emergencies can be prioritized appropriately using the CTAS guidelines. Methods: We used the Complaint Oriented Triage (COT 2012), which is an interactive computerized CTAS tool, to triage select oncological emergencies; superior vena cava syndrome, cardiac tamponade, tumor lysis syndrome, and febrile neutropenia. Results: Patients with cancer have a higher acuity compared to many other ED patients. However, most of the oncological emergencies can be subtle and nonspecific. The CTAS guidelines need to be strengthened to better represent the urgency of these life-threatening conditions. Conclusion: Although revisions have been implemented and the reliability of the CTAS tool has improved, the guidelines are designed to be generic and cannot address every health situation. Febrile neutropenia is an excellent example of the additional supports needed at triage to accurately determine the patient’s health status. Knowledge of the signs and symptoms of these emergencies will enable triage nurses to accurately differentiate the urgency of the different presenting complaints. Formalized education that prepares triage nurses to better understand the complexity of the symptom presentation and the needed care for patients with different oncological emergencies is essential.
