Analysis of the Effects of the Implementation of Transfusion-Related Education for Nurses at Medical Institutions 医疗机构护士输血相关教育实施效果分析

Background: Nurses play a central role in the process of blood transfusion because they have the closest interac-tions with patients. Providing nurses with the appropriate knowledge and competency through education can help ensure transfusion safety. This study investigated the effectiveness of the first nationwide transfusion-related educa- tion for nurses by analyzing questionnaires for a self-assessment of competency on transfusion-related knowledge before and after the education and general evaluation for the educational program. Methods: The education program was composed of four lectures and was conducted in seven regions in South Korea. One hundred and ninety-two nurses participated, and the questionnaires for 170 nurses were analyzed. Results: The participants consisted of 90 nurses (53.0%) from tertiary hospitals, 23 (13.5%) from general hospitals, and 57 (33.5%) from other hospitals. The majority of the participants (103/170, 60.6%) were from hospitals with ≥ 500 beds, and 69.4% had a work period of ≥ 10 years. The scores for pre-/post-education self-assessment of competency were as follows: blood components, 3.03/3.73; pretransfusion testing, 2.86/3.64; management of transfusion, 3.18/3.84; and transfusion reactions, 3.11/3.78. In all categories, there was a significant increase in the score after the education program. The majority of participants (99.4%) provided a positive response regarding the necessity of a transfusion-related education program. Conclusion: South Korea’s first attempt at transfusion-related education for nurses showed a strong positive effect by improving participants’ transfusion-related competency. Considering the important role of nurses in blood transfusion, the educational program should continue and be expanded in the future. (Korean J 2020;31:222-229)

Identification of Rh Alloimmunization after Rh Antigen Incompatible Red Blood Cell Transfusion Rh抗原不相容红细胞输血后Rh异体免疫的鉴定

The rhesus (Rh) blood group system is one of the most important systems in transfusion medicine. Rh alloantibodies can cause hemolytic transfusion reactions and difficulties in finding compatible blood. On the other hand, the phenotypic test and pre-transfusion compatibilization for RhCE antigens have not been routinely performed in most clinical laboratories. This study examined the occurrence of Rh alloimmunization, including C, c, E, and e antigens, after Rh incompatible red cell transfusion during a specific period. From November 2016 to December 2017, clinical and laboratory information of transfused patients was collected; this included the transfusion history and Rh phenotypes of the patients and transfused red blood cell (RBC) products. Of 3,623 patients who received red cell transfusion, 99 (2.8%) revealed unexpected antibodies. Among them, 48 patients (48.5%) had alloantibodies to the Rh-system antigen, including the anti-D antibody. Based on the transfusion history and Rh phenotypes of patients and transfused packed RBCs, 11 patients (11.1%) were assumed to develop Rh alloantibodies after discrepant Rh phenotype red cell transfusion. A considerable proportion of unexpected antibodies was caused by the exposure of Rh mismatched red cells. Therefore, extended Rh antigen matching, including the C, c, E, and e, is needed to prevent alloimmunization. (Korean J Blood Transfus 2020;31:247-253)

Cloning of ABO PCR Products Including Exon 2 to Exon 7 Using Seamless Cloning Technique 利用无缝克隆技术克隆包括外显子2至外显子7的ABO PCR产物

Background: Sequencing and allele separation from exon 2 to exon 7 in the ABO gene can be required to deter-mine the genotype. This study used the seamless cloning technique to clone approximately 7.3 kb containing a portion of exon 2 to exon 7 of the ABO gene. Methods: The previously confirmed DNA sample of the ABO*B.01/O.01 genotype was used for seamless cloning using the Overlap Cloner DNA Cloning Kit (ElpisBio). Cloning was performed three times using the same cloning protocol. The plasmids obtained by cloning were also confirmed by sequencing. Results: Ninety-two colonies (average of 30 colonies per culture plate) were obtained. Of the fifty colonies, 48 colonies were properly amplified. Plasmid sequencing confirmed that 31 heterozygous alleles were well separated in accordance with predictions through the BGMUT database. Conclusion: The data suggest that the seamless cloning technique is a reliable technique that can be used to separate long-range alleles, such as the ABO gene. (Korean 2020;31:239-246)

Whole Blood Processing Using the Buffy-Coat Method 使用黄皮法处理全血

Blood components can be prepared from whole blood either using the platelet-rich plasma (PRP) method or buffy-coat (BC) method. The use of these two whole blood processing methods varies greatly between countries. The PRP method is used in Korea, while the BC method is used in Europe. On the other hand, the BC method with the automated device is being used more widely in many countries. Only a few countries still use the PRP method for whole blood processing, with Korea remaining among them. The choice of the use of a specific whole blood processing method should consider all aspects of safety, quality, and efficiency as well as process costs. This review summarizes the existing information on PRP and BC methods for whole blood processing, particularly comparing both methods. (Korean J Blood Transfus 2020;31:201-210)

Good Manufacturing Practices (GMP) for Blood and Blood Components, What Do You Know and How to Prepare? 血液和血液成分良好生产规范(GMP),你知道什么和如何准备?

Until recently, there was no formal regulation to cover the preparation of blood and blood components for transfusion in Korea; it was perceived as part of medical practice rather than the preparation of a biological therapeutic product. On the other hand, in accordance with the revision of the Korean Pharmaceutical Affairs Act, blood and blood components for transfusion have been under formal regulations enforced by the Ministry of Food and Drug Safety (MFDS) since January 2019. The regulation requires that blood establishments be licensed and subjected to regular inspection for good manufacturing practices (GMP) compliance. The preparation of blood and blood components is different from plasma-derived medicinal products (PDMPs) manufacturing and pharmaceutical medicines in general. Therefore, to accelerate the implementation of a regulatory system and reach an acceptable compliance status, blood establishments should be engaged to address the specific attributes of blood and blood components for transfusion. Current Status of Our Journal, The Korean Journal of Blood Transfusion 韩国输血杂志的现状

The Korean Journal of Blood Transfusion (KJBT) is a representative journal in the field of transfusion medicine and has contributed to the development of the national blood business and clinical research. The KJBT was selected as a candidate journal registered in the Korea Citation Index (KCI) in 2009 and was selected as a KCI-registered journal in 2016. The KJBT has published 796 articles, including 601 original articles and 144 case reports from the first issue to volume 31, number 2. The KJBT has continuously introduced new tools for improving journals but was assessed as having a high self-citation rate because of the characteristics of the journal. Nevertheless, it is necessary to increase the number of papers to improve the value of the KJBT. To accomplish this, continuous efforts of the editorial committee will be needed to provide better services to authors and readers and to promote journals. (Korean J Blood Transfus 2020;31:193-200)

Rare ABO Allele Encoding Glycosyltransferase with Dual Specificity Found in a Cambodian Individual with the A2B Phenotype 罕见的ABO等位基因编码糖基转移酶与双特异性发现柬埔寨个体与A2B表型

Cis-AB and B(A) alleles encode an ABO enzyme with dual A and B glycosyltransferase activity. Although globally rare, the cis -AB phenotype is found relatively often in Korean, Japanese, and Chinese populations. Cases of the B(A) allele have been reported mostly in the Chinese population. Forward typing performed in a Cambodian woman with an ABO discrepancy demonstrated a strong reaction with anti-A and anti-B reagents, while there was no reaction with lectin anti-A 1 . The anti-A 1 antibody was detected in reverse typing. Through ABO gene sequence analyses of exons 6 and 7, one of the alleles was identified as ABO*B.01 . In contrast, the other allele harboring a c.803G > C substitution was either ABO*cisAB.05 or ABO*BA.06 allele. The ABO*cisAB.05 and ABO*BA.06 alleles remain indistinguishable despite routine serological testing and ABO genotyping. To the best of the author’s knowledge, this is the first case report of these variants discovered in a Cambodian individual residing in Korea. (Korean J Blood Transfus 2020;31:254-259)
