Smoking after a heart attack - a case report 心脏病发作后吸烟——一个病例报告

A Middle aged man suffered acute myocardial infarction while at work and angiography confirmed triple vessel disease requiring stenting followed by lifelong medication. Important to note that upon follow up his lifestyle had not significantly changed, and he continued to smoke cigarettes. Soon after he developed severe low back pain which made him bed bound for a few weeks and further medications were added however all this could not deter him from smoking. This paper aims to discuss why some patients continue to smoke after such major life events. Keywords: smoking, heart attack

among Diabetic Patients in Najran Armed Forces Hospital 在Najran武装部队医院的糖尿病患者中

Aim of Study: To investigate what factors affect patients’ compliance with an insulin regimen as part of self-management in Najran Armed Forces Hospital. Methodology: This study followed an exploratory descriptive cross-sectional research design. The study population consisted of diabetic patients receiving insulin. A simple random sampling technique was followed to select diabetic patients among those registered at the Chronic Disease Clinics at Najran Armed Forces Hospital. A Self-administered questionnaire was constructed and used to collect data over a two-week period. Results: The study sample included 90 patients, of whom 53.3% were males. Many participants expressed poor attitudes and lacked self-management skills and the necessary health education information. There were statistically significant associations regarding the relationship between poor attitude and poor self-management. Participants’ knowledge regarding how to take insulin injections; fear of insulin injection; having a monitoring device; health conditions; diabetes type residence location education; and seeing diabetes educators were significant factors related to patients’ attitudes. Conclusion: Diabetic patients’ compliance regarding self-management advice is variable. It is of utmost importance for diabetic patients to be educated and supported in all aspects of self-management to avoid diabetes complications. Concerns of parents of students with autism spectrum disorders (ASD) towards the safety of COVID-19 Vaccination in Saudi Arabia (SA) 沙特阿拉伯自闭症谱系障碍(ASD)学生家长对COVID-19疫苗接种安全性的担忧

Background: Although previous scientific studies have confirmed that vaccinations are safe and do not result in harm to students with and without disabilities, some parents of students with autism spectrum disorders (ASD) had concerns that vaccines might cause potential medical issues for their children. These concerns may have an impact on these parents’ decision to vaccinate their students against COVID-19. Therefore, this study aims to explore and understand the concerns of parents of students with ASD about the safety of the COVID-19 vaccination and to what extent these concerns can diminish the willingness of parents of students with ASD to vaccinate their children against COVID-19 in Saudi Arabia (SA). Methods: a qualitative design was adapted through a developed semi-structured interview. Participants included eight parents of students aged 12 years and over with ASD, who were enrolled in two male state-funded institutions that provide educational services to those with developmental disabilities, including ASD in Riyadh, SA. The study was conducted between 17th of September and the 15th of November 2022. Results: The results indicated that parents had several concerns regarding the safety of the COVID-19 vaccines. However, their concerns have not diminished their willingness to vaccinate their children. To increase parents’ knowledge and relieve their concerns, several recommendations should be applied such as strict penalties being imposed on parents who refuse to have their children vaccinated, because they put their children in a risky position and the authorities in this matter and using the media to convince the community of the importance of the vaccine and the necessity of addressing it. Conclusion: The current study emphasized the need to provide more care and support for parents with children with disabilities. An urgent intervention should be provided for children with ASD because they are more likely to be at a higher risk of COVID-19 illness due to the increased prevalence of underlying health conditions, suboptimal vaccination rates, and systemic inequities. Case History: A Case of primary Amelanotic Malignant Melanoma 病例史:原发性无色素恶性黑色素瘤1例

This case study discusses the atypical presentation of amelanotic melanoma in a 37-year-old Caucasian female patient and the challenges it poses for diagnosis and the consequent delay in treatment. 5-year survival rate is 93% for all melanomas but amelanotic melanoma survival is 88% and this is predominantly due to delayed diagnosis(1). Better patient and doctor education and readily available histopathological diagnosis confirmation can lead to earlier diagnosis and better outcome. An exaggerated capillary endothelial edema may be the cause of sudden deaths in sickle cell diseases 过度的毛细血管内皮水肿可能是镰状细胞病猝死的原因

Background: Sickle cell diseases (SCDs) are inborn and severe inflammatory processes on vascular endothelium, particularly at the capillaries which are the actual distributors of the sickled or just hardened red blood cells (RBCs) into the tissues. Methods: All patients of the SCDs were included. Results: We studied 222 males and 212 females with similar ages (30.8 vs 30.3 years, p>0.05, respectively). Disseminated teeth losses (5.4% vs 1.4%, p<0.001), ileus (7.2% vs 1.4%, p<0.001), cirrhosis (8.1% vs 1.8%, p<0.001), leg ulcers (19.8% vs 7.0%, p<0.001), digital clubbing (14.8% vs 6.6%, p<0.001), coronary heart disease (18.0% vs 13.2%, p<0.05), chronic renal disease (9.9% vs 6.1%, p<0.05), chronic obstructive pulmonary disease (25.2% vs 7.0%, p<0.001), and stroke (12.1% vs 7.5%, p<0.05) were higher in males but not acute chest syndrome (2.7% vs 3.7%), pulmonary hypertension (12.6% vs 11.7), deep venous thrombosis and/or varices and/or telangiectasias (9.0% vs 6.6%), or mean age of mortality (30.2 vs 33.3 years) (p>0.05 for all). Conclusion: The sickled or just hardened RBCs-induced capillary endothelial damage, inflammation, edema, and fibrosis are initiated at birth, and terminate with disseminated tissue hypoxia, multiorgan failures, and sudden deaths even at childhood. Although RBCs suspensions and corticosteroids in acute and aspirin plus hydroxyurea in acute and chronic phases decrease severity of the destructive process, survivals are still shortened in both genders, dramatically. Infections, medical or surgical emergencies, or emotional stresses-induced increased basal metabolic rate aggravates the sickling and capillary endothelial edema, and may terminate with multiorgan failures-induced sudden deaths in the SCDs. Helicobacter Pylori infection; the challenging task of improving eradication rates in light of rising antibiotic resistance, a literature review 幽门螺杆菌感染;鉴于抗生素耐药性的上升,提高根除率的挑战性任务,一篇文献综述

Helicobacter pylori (H. pylori) infection continues to be a major public health issue worldwide. A global systematic review shows that in 2015, approximately 4.4 billion individuals worldwide were estimated to be positive for H. pylori. Overwhelming evidence has demonstrated that H. pylori infection is associated with significant medical conditions including peptic ulcer disease, stomach cancer and MALToma. The consensus report from the Kyoto H. pylori conference in 2015 recommended that all H. pylori infections should be eradicated whenever they are found unless there are compelling reasons not to do so. However, increasing H. pylori antibiotic resistance has been reported globally over the past two decades coinciding with a continuous decrease in eradication success rates. To tackle this global problem, we need new antimicrobial drugs and treatment strategies but also a better understanding of the emergence and spread of resistant bacteria as well as improved diagnostic tools to guide clinicians in further optimizing currently available regimens. Knowledge about Hypertension and its Associated Risk Factors among Saudi University Students in Riyadh City 利雅得市沙特大学生对高血压及其相关危险因素的了解

Objectives: To determine the level of knowledge of hypertension and its associated risk factors among Saudi undergraduate students in the universities of Riyadh City. Methods: A cross-sectional study was conducted among the students from 11 universities in Riyadh City. Convenience sampling was used, and the survey was not validated. The survey was distributed manually but collected electronically. It consisted of four sections: socio-demographics, risk factors, complications, and general statements. Results: We analyzed 605 participants’ data; the highest participation percentage was from King Saud University (14.1% of the total); 42.1% of the participants were from the health track, and 69.8% of the participants who had heard about hypertension described the source as their families. The highest level of knowledge was on the general statement section where 45% had good knowledge, and the lowest was on the complications section where 30% had good knowledge. The total level of good knowledge was 31.1%. Conclusion: This study identifies some gaps in the knowledge of hypertension among undergraduate university students. Further research is advised to reach conclusive and more accurate data. Key words: knowledge hypertension, risk factors, undergraduate students, Riyadh, Saudi Arabia

Symmetrical Posterior Lower Limb Bruising in A Qatar 2022 FIFA World Cup Fan – A Case Report 2022年卡塔尔世界杯一名球迷下肢对称后挫伤一例报道

Suspected spontaneous ecchymoses may indicate rare but serious underlying bleeding disorders. By undertaking a thorough clinical assessment and appropriately safety-netting patients, clinicians can reduce the need for unnecessary investigations and follow-up. Objective: To report a case of symmetrical posterior lower limb bruising in a Qatari male football fan twelve-hours following his attendance at a Qatar 2022 FIFA World Cup match in Doha, Qatar. Background: A 36-year-old previously fit and well Qatari male presented routinely to a primary healthcare centre in Doha, Qatar, after waking with painless bruising to the posterior aspects of both thighs and calves. He attended a Qatar 2022 World Cup football match as a fan the evening before, and reported being completely well prior to this. He scored 0 on both the Bleeding Assessment Score and Wells’ Criteria for DVT. Following a thorough history, examination and laboratory studies to exclude possible bleeding disorders, a diagnosis of simple purpura was made. The patient was reviewed a week later, and confirmed complete resolution. Conclusions: Fit and well football fans presenting with bruising to the dorsum of the legs in a similar pattern can, following a thorough assessment, be diagnosed with simple purpura.
