Gaps in health and social care services to older adults living with HIV: a qualitative study on the perspectives of older adults and service providers 向感染艾滋病毒的老年人提供保健和社会护理服务方面的差距:关于老年人和服务提供者观点的定性研究

The aging of people living with HIV (PLHIV), along with the rise in new HIV infections in people aged 50 and over, has attracted an increasing amount of interest among researchers and practitioners. Within both research and practice, PLHIV have been identified as an « aging » cohort beginning at the age of 50, due to the reality of premature aging and comorbidity associated with living with HIV.1 Medical research on the accelerated aging of PLHIV and the multiple comorbidities associated with HIV and aging has shed light on the significant health needs of this population.2‒4 Social science research has also revealed the many psychosocial difficulties experienced by older adults living with HIV (OAHIV).5‒9 Older adults living with HIV (OAHIV) are particularly vulnerable to mental health problems,4,10,11 isolation,5,12 and HIV and age-related stigma1.8,12,13 In addition, OAHIV often find themselves confronted by a lack of emotional and instrumental support from the community due to distance from family and the absence of intimate partners.15,16 This lack of informal support results in a situation whereby OAHIV may be required to seek support from formal resources.2,4,16,17

Health care sectors have an extremely competitive environment and patient satisfaction is one of the important factors which determine the success of health care facility.1 The real challenge is not getting ready with mere requirements, but also in delivering good quality services.2. Patient’s satisfaction with nursing care is considered the major predictive factor of overall satisfaction and hence it can be further utilized as an important tool to improve the quality of services. Improved satisfaction in emergency department is likely to have a significant impact on the public view of hospital. Satisfied patients may be more compliant with their medical regimens which further promote health and well-being parts.3

Currently there are specializations recognized by the Federal Nursing Council (FNC), which also requires, through FNC resolution number 543/2017, “the presence of nurses in these units” as well as a Resolution “for the duties of nurses in radiology” according to FNC resolution number 211/1998.1

The use of physical restraints has been a controversial, yet frequently used as nursing intervention for confused and hospitalized frail elderly and patients with severe mental disorders over the past two decades.1 The use of physical restraints is a very common practice in various clinical settings i.e. ICU, medical-surgical, psychiatric and pediatric units. Their use in hospital is often considered to be an accepted and perhaps unquestioned practice related to patient safety.2

Hygiene is the practice of keeping oneself and one’s surroundings clean, especially in order to prevent illness or the spread of disease. Hand washing for hand hygiene is the act of cleaning the hands with or without the use of water or another liquid, or with the use of soap, for the purpose of removing soil, dirt, and/or microorganisms. The Centers for Disease Control and Prevention (CDC) has stated, “It is well-documented that one of the most important measures for preventing the spread of pathogens is effective hand washing.” The main medical purpose of washing hands is to cleanse the hands of pathogens and chemicals which can cause personal harm Or disease.1 Most healthcare-associated infections (HAI) are thought to be transmitted by the hands of Healthcare Providers (HCPs) through direct contact, mainly when the hands of HCPs transfer microorganisms between individuals or between individuals and the environmental reservoir.2

Nursing handover is the transfer of responsibility and accountability for patients care between nurses that conducted three times a day in most nursing wards globally.1 It has been described as having different functions such as transfer of information as an overt functions and socialization, cohesiveness and release of frustrations and emotions as covert functions.2,3 Each ward vary in their method of sharing information during handover, range from face-to-face verbal reports in the station or at the patient’s bedside to written and taped reports.4,5

The vital role of Health Promotion is a research based communication process involving planning, design, and implementation of strategic intervention. It provides relevant information and adequate motivation to impact attitudes and behaviors in individuals or groups.1 Health education has always been a vital component of nursing.2 Nurses have a long history as patient educators. Patient teaching is a tenet of all nursing education programs and an important nursing role.3,4 Most nurses recognize the importance of patient and caregiver education but are frustrated by the lack of time and resources to support it. They know that the limited time they have with patients can restore health to some degree, but the patient and family are ultimately responsible for continuing the gains and maintaining health through self-care.5 Nurses often respond to the pressure by saying they have “no time to teach” and then ask, “How can I get more time to teach?” In these tight economic times, the response is a frustrating “no money, no staff, no time.” As our world changes, the methods available to teach patients and caregivers expand. Using video as a method of teaching has been found to be an effective and economic in times of nursing shortage.6 The use of educational video offers many favorable characteristics in the variety field of medical and nursing area. Video teaching can provide a consistent from of teaching and can communicate certain concepts in a visual and realistic manner.7 Furthermore, video education has been shown to be superior to traditional method and improve knowledge score.8,9 The purpose of this study was to evaluate how the implementation of a VATM on home care of schizophrenic patient would influence the caregiver understanding of the nature of illness, attitude towards the patient, treatment and social interaction, caregiver’s burden and skill in handling schizophrenic patient at home. Patient teaching is a tenet of all nursing education programs and an important nursing role.3,4 Most nurses recognize the importance of patient and caregiver education but are frustrated by the lack of time and resources to support it. They know that the limited time they have with patients can restore health to some degree, but the patient and family are ultimately responsible for continuing the gains and maintaining health through self-care.5 Nurses often respond to the pressure by saying they have “no time to teach” and then ask, “How can I get more time to teach?” In these tight economic times, the response is a frustrating “no money, no staff, no time.” As our world changes, the methods available to teach patients and caregivers expand. Using video as a method of teaching has been found to be an effective and economic in times of nursing shortage.6 The use of educational video offers many favorable characteristics in the variety field of medical and nursing area.
