Background: Peritonsillar abscess (PTA) is the most common deep infection of neck space that occurs in adults and is potentially life threatening if not treated appropriately. The surgical treatment of peritonsillar abscess that is not complicated with upper airway obstruction still remains controversial. Objectives: This review attempts to explore the controverseries between different types of management and compare the efficacy of needle aspiration, tonsillectomy and/or incision and drainage. Methodology: This study was done in King Abdulaziz University. All researches, between 1988 to 2015 were recruited and reviewed in this study. Results and conclusion: Studying the three accepted methods of draining: incision and drainage, abscess tonsillectomy, or needle aspiration, Incision and drainage is an efficient and safe procedure to treat peritonsillar abscess. As well as it is superior to Needle aspiration in terms of the post-procedure pain score. It can be performed as the first-line treatment of peritonsillar abscess. Quinsy tonsillectomy can offer several advantages, by allowing full evacuation of the abscess cavity, effectively relieving symptoms, but holds risk in general anesthesia intubation. and also higher post-operative complication like Peritonsillar abscess make it not a favorable step.

Hypernasality which is rare symptom commonly occur as a consequence of velopharyngeal insufficiency (VPI). VPI usually manifested as nasal air emission and hypernasal resonance during speech. The cause can be divided into congenital, neuromuscular disorder and surgical complication. Congenital cause of VPI includes cleft palate, nasal septum malformation such as vomer agenesis, submucous cleft palate and velar dysplasia, while neuromuscular VPI can be due to cerebral palsy or cerebrovascular accident. Surgical cause of VPI could be due to adenoidectomy and scarring of the velum post palatoplasty in cleft palate repair. We present a 17-year-old man who was diagnosed of congenital left nasolacrimal duct obstruction referred to us for left endoscopic dacrocystorhinostomyin which during nasoendoscopic examination revealed absence of vomer.

In Cameroon, where the prevalence of infectious complications remains high, several factors that may or may not lead to these infectious complications are still to be known in our region. The aim of this work was to contribute to improved care delivery. This was a prospective study coupled with an exploratory study from May 10 th to August 31 st , 2016. All patients who had a gynecological and obstetric surgery at the HRN and without any infection before surgery were included in this study, and were followed each for 30 days. In total, we had 65 surgical procedures in gynecology and obstetrics. The average age of the patients was 29.23 years. Clean surgeries accounted for 98.5% of interventions, with contaminated clean surgeries accounting for 1.5% of interventions. A total of 6 cases of infections of the surgical site was observed (9.2%). Significant variables associated with postoperative infections were the use of antibiotic, the date of first dressing, the duration of surgery. The infection rate of the surgical site which is 9.2% is clearly greater to the data of the literature, despite the antibiotics in prophylactic in clean surgeries. However, better pre and post-operative care as well as strict adherence to the principles of perioperative asepsis should be the rule in our context.

Introduction : Vocal cord paralysis often causes mortality by upper airway obstruction in some neurodegenerative diseases such as Parkinson’s disease and multiple system atrophy. Vocal cord paralysis is uncommon in Parkinson’s disease (PD) in contrary it is more common in multiple system atrophy (MSA). The pathogenesis of vocal cord paralysis in Parkinson’s disease is not well understood but may involve degeneration of the nucleus ambiguous. In terms of managing patient with bilateral vocal cord paralysis in PD, it can be either performing tracheostomy to relive the upper airway obstruction or by optimizing the medical treatment. There are very few available reported cases whereby patient are treated with medical treatment alone. Case Report : We report a case of 65 years old lady who presented with stridor resulting from bilateral vocal cord paralysis and she has been diagnosed to have Parkinson’s disease for more than 10 years. She had her antiparkinson medication optimized and requiring no surgical intervention to relieve the upper airway obstruction. Conclusion : In conclusion we would like to emphasize that it is important to recognize bilateral vocal cord paralysis in Parkinson’s disease and early optimization of medical treatment could avoid a need of tracheostomy.

Diabetes is one of the most common chronic diseases, usually appears during childhood or adolescence; however, it can also develop in adults. The objective of this study was to evaluate the knowledge and awareness about diabetes among teaching stuff in Taif province of Saudi Arabia. A cross sectional study, using a structured pre-tested questionnaire, was conducted in Taif. Our study involved 50.5% males and 49.5% were females. The age of participants ranged from less than 30 years old to more than 50 years old. Our results showed that he majority of teaching staff in our sample study are aware of diabetes and how to deal in case of low blood glucose level. However, the knowledge of causes, symptoms, risk factors, complications of diabetes and signs of low blood glucose level was not as high as expected. Therefore, training is required for the teaching stuff as more than half of our sample study had taught a diabetic child.

Introduction: The Cervicofacial rotation advancement flap CRAF is a random-pattern flap that provides an excellent match for cheek reconstruction. In this report, the authors review their experience with this flap and present their advantages in order to expand the awareness and promote the application. Methods: A retrospective analysis of reconstruction of cheek defects using CRAF was performed in 12 patients. Data on patient demographics; diagnosis, defect type, and outcome were collected. Results: These patients included 07 male and 05 female; they ranged in age from 56 to 88 years, with a mean age of 67 years.05 patients used tobacco and one patient had a history of radiation. On histopathological examinations of the specimen, 03patients had squamous cell carcinoma, and 09 patients had basal cell carcinoma. The mean defect size was 40cm 2 [range 10 to 67cm 2 ]. The mean follow-up was 06 months [0 to 24 months]. One patient was lost to follow-up after suture removal. As complication 01 patient had flap necrosis No facial nerve injury was observed. All patients were pleased with the Cosmetic results outcome. Conclusion : Cervicofacial flap provides a technically simple, reliable, safe, efficient and cosmetic means to reconstruct defects of the cheek.

Introduction: Chronic kidney disease (CKD) prevalence in Saudi Arabia has been rising over the past few years. The risk of developing left ventricular dysfunction is high in patients with CKD on hemodialysis. Our aim is to study the frequency of emergency room visits and the length of ER stay in patients LVD on hemodialysis. Methods: All patients who were on hemodialysis between the period of January 2011 and November 2016 were included in our study. Patients’ demographic, medical and laboratory data were extracted for all patients. Patients were classified into three groups according to their ejection fraction (EF<40%, EF= 40-49% and EF≥50%). Descriptive statistics were done for all variables. Logistic regression was used to assess the outcome while adjusting for confounder. Results: Analysis included 333 patients. Two-hundred and fifty seven patients had an EF ³50% and 36 patients with EF 40-49% and 40 patients with EF <40 %. Age was significantly higher in patients with EF<50% compared to patients with EF ³50% (P=0.002). Comorbidities were more prevalnt in patients with EF<40% and EF 40-49%. Number of ER visits and length of stay were significantly different between the three groups (P=0.005, P=0.023) ICU admissions shows a statistically significant diffrence between the three groups (P=0.013). Conclusion: Patients with low EF on hemodialysis have a higher rate of ER visits and length of stay in ER when compared to patients with EF≥50.

Following the controversial use of the mitochondrial replacement technique in IVF procedures to prevent the transmission of potentially incurable and fatal inherited mitochondrial disorders, various ethical and safety concerns and regulatory issues were raised in the medical community. The diseases in question affect mitochondria, and are only inherited from the mother [9], patients with more than 60% mutated mitochondrial DNA (mtDNA) may develop severe systemic diseases, such as cancer, diabetes, blindness, deafness, liver failure, infertility, neuropathy, myopathy, cardiomyopathy, sensory deficit and many others [1, 2, 3]. The incidence of clinically present mtDNA disorders is at least 1 in 10 000 individuals, but the frequency of the pathogenic mtDNA mutation is approximately 1 in 200 of the general population [4] . Currently there is no effective therapy or genetic screens for these maternally inherited diseases, and at present, the mitochondrial replacement technique (MRT) holds promises for human reproductive health and the production of healthy babies. This article will discuss some objections that oppose the procedure in principle and present a perspective surrounding the legalization of these techniques in Morocco.

The incidence of clinically present mtDNA disorders is at least 1 in 10 000 individuals, but the frequency of the pathogenic mtDNA mutation is approximately 1 in 200 of the general population [4] . Currently there is no effective therapy or genetic screens for these maternally inherited diseases, and at present, the mitochondrial replacement technique (MRT) holds promises for human reproductive health and the production of healthy babies. Kikuchi-Fujimoto disease (KFD) is a very rare, self-limited and benign inflammatory disorder. It was first reported in Japan in 1972. Diagnosis of Kikuchi disease is confirmed by histopathological investigation of lymph node (LN) biopsy under ultrasound guidance. Computed tomography (CT) and magnetic resonance imaging (MRI) help in diagnosis of Kikuchi cervical lymphadenopathy but don’t provide a definitive diagnosis. We describe a case of KFD affecting the cervical lymph nodes of a 24-year old Saudi female.

Granulomatous mastitis is indeed a rare affliction, but it can constitute a veritable diagnosis and treatment predicament for several reasons. The etiology of this disease remains shrouded in mystery and the treatment depends on the gravity and extension of the lesions, ranging from corticosteroids to surgical excision with an important recurrence rate. We present the case of 40-year-old woman with granulomatous mastitis for which clinical and radiological remission was obtained via medical treatment.
