新冠病毒(SARS CoV-2) Omicron變異株於兒童族群感染具有高病毒量及高傳染性,且臨床症狀與其他秋冬常見之呼吸道病毒感染類似,加上兒童非藥物介入措施(non-pharmacological interventions, NPI)順從性及疫苗接種率低,若無適當之感管措施,容易於兒童病房中造成群聚感染。兒科病房COVID-19群突發事件常發生於社區流行高峰時期,可能造成住院之高風險兒童出現重症,亦可能引起醫護人員感染,並導致醫療照護人力不足。預防兒童病房COVID-19群聚感染,除包括全院規範之感染措施外,另須特別加強住院時病人全面篩檢、醫療人員個人防護裝備選擇(尤其是眼睛黏膜保護)、減少不必要的氣溶膠產生之醫療處置及加強環境清消。而一旦於病房發現確診個案,亦需在第一時間進行隔離措施、流行病學調查及健康管理。具風險因子之兒童若有必要應盡速給予抗病毒藥物治療,並注意重症之危險徵兆。 Compared with prior variants, the SARS-CoV-2 Omicron variant causes high viral loads and high infectivity in the pediatric population. However, the clinical manifestations of COVID-19 in children are similar to those caused by other respiratory viruses. Besides, poor non-pharmacological interventions (NPI) compliance and low vaccination coverage in the pediatric population also contributed to the magnitude of the COVID-19 outbreak in Taiwan. Outbreaks of COVID-19 in pediatric wards might spread to the rest of the community, contribute to severe symptoms in children with underlying comorbidities, and infect healthcare workers leading to inadequate healthcare staffing. In addition to universal infection control measures, the infection control bundle for COVID-19 in pediatric wards should include measures such as universal screening for SARS CoV-2 before admission, provision of proper personal protection equipment (especially eye protection) of healthcare workers, minimization of the unnecessary aerosol generation procedures, and enforcement of environmental sanitization. If a new COVID-19 case is detected in the ward, it should be epidemiologically investigated, and the patient should be immediately quarantined and monitored. Children with comorbidities should be prescribed antiviral treatment and receive close monitoring for severe manifestations of COVID-19.

Stakeholder interviews were conducted to identify potential problems in the video accreditation process, such as problems with software, hardware, funding, human resources, and resource allocation. The development team implemented four solutions: (1) using 5G, 4G, and Wi-Fi 6 together, (2) designing innovative video carts, (3) developing a group video function, and (4) conducting educational training and continuous simulation exercises. From December 28, 2020, to January 1, 2021, we conducted JCI accreditation through video surveys and promoted effective communication between medical staff and the accreditation committees. This project can provide a reference for the development of accreditation policies and lead to the transformation of Taiwan’s healthcare accreditation system so that it will be better equipped to handle future pandemics.

多元性別群體擁有獨特的生理與心理健康議題以及其特殊健康需求。臺灣為亞洲地區同志議題開放國家,更在2019年5月通過同性婚姻法案,然而在多元性別群體健康促進、健康需求與文化照護探討與實務上較為侷限。雖然臺灣醫護人員或相關醫護學生對於多元性別群體持正向態度並具有高度意願提供群體照護,醫護人員或學生對於多元性別文化照護知識與技能較為缺乏,建議醫護教育課程與繼續教育課程納入多元性別文化照護知識與技巧訓練,藉以提升多元性別文化照護品質並降低多元性別群體健康差距。 Sexual and gender minorities have been shown to have specific physical and mental health issues and needs. Same-sex marriage was legalized in Taiwan in May 2019, and sexual and gender minorities are becoming increasingly visible in Taiwanese society. Taiwanese healthcare providers and students hold positive attitudes toward sexual and gender minorities and have a high willingness to provide culturally competent care for sexual and gender minority populations. However, these providers and students often lack the knowledge and skills necessary to provide such care. Offering relevant coursework for healthcare students and continuing education courses for healthcare providers could improve the quality of culturally competent care and reduce health disparities among sexual and gender minorities in Taiwan.

The Joint Commission of Taiwan (JCT) also launched the “Disease Specific Care - Chronic Obstructive Pulmonary Disease (DSC-COPD)” certification program in 2017 and certified many medical institutions in an effort to improve the quality of COPD care. In this paper, we summarize the series of plans created by the TSPCCM, JCT, and COPD-certified medical institutions that can be used to develop a combined care and treatment model for COPD.

目的:降低牙科尖銳物扎傷,以提升安全的工作環境。方法:經專案小組重新修訂污染器械處理標準作業流程、舉辦在職教育、拍攝影片「照顧病患,更要保護 自己」宣導、調整洗牙器械擺放位置、制定標語口號「給我3分鐘,給你下一台」。結果:由改善前佔全院16.7%,改善後降低至0%,整體降低69.4%。結論:經實施對策後,能有效降低牙科尖銳物扎傷,將原有高風險環境再造改良後降低同仁傷害,日後仍需持續營造安全的工作環境。 Objective: To promote a safe workplace in hospital dentistry department by minimizing injuries caused by sharp objects. Methods: A task force revised the standard operating procedures for the handling of contaminated equipment and instruments, conducted on-the-job training, produced a promotional film titled “Caring for Patients, but Protecting Yourself,” rearranged the placement of dental cleaning instruments and tools in the clinic, and created the slogan “Give me three minutes, then the bench is yours.” Results: The sharp object injury incidence rate in the dentistry department dropped from 16.7% to 0%, resulting in a decrease of 69.4% in the overall hospital incidence rate. Conclusions: Following the interventions of this study, the number of sharp object injuries in the dentistry department was reduced, and the previously high-risk work environment was improved, which minimized staff exposure to hazards. Nevertheless, continuous efforts are necessary to foster a safe workplace.
