本文旨在介紹台灣性別醫學發展。性別醫學在國際上是由醫學界人發起,台灣卻是由一群社會科學、公衛與護理學界的性別研究者開始發起,之後從中央公部門推動醫學研究納入性別分析與醫學教育融入性別觀點。經歷二十多年的努力,國防醫學院及三軍總醫院一群醫生於2022年9月27日成立「臺灣性別醫學學會」(TGMS),其宗旨為促進臺灣性別醫學之發展。TGMS以舉辦國內外相關學術演講,以及運用季刊會訊,發表性別醫學文章等方式逐漸建立台灣性別醫學新知。已辦理三次全國活動,討論性別醫學在國際與臺灣的發展、性別醫學倫理、性別醫學研究指引、醫學教育、高血壓、紅斑性狼瘡、腦中風、老年醫學等等議題,啟發國內醫學領域對於性別醫學的瞭解及興趣。2023臺灣性別醫學學會年會特別邀請日本性差醫學學會(性差医学・医療学会) 秋下雅弘教授(Dr. Masahiro Akishita),以及美國史丹佛大學( Stanford University )Pro. Internationally, gender medicine originated from the medical community, whereas in Taiwan, it was initiated by gender researchers from the social sciences, public health, and nursing fields. Subsequently, the central government promoted the integration of gender analysis into medical research and the incorporation of gender perspectives into medical education. On September 27, 2022, after more than two decades of effort, a group of doctors from the National Defense Medical Center and the Tri-Service General Hospital established the Taiwanese Association for Gender Medicine. The goal of this group was to advance gender medicine in Taiwan. The Taiwanese Association for Gender Medicine gradually established a body of knowledge in gender medicine in Taiwan by hosting academic lectures both domestically and internationally and publishing gender medicine articles in its quarterly newsletter. The association has organized three national events discussing various topics, including the development of gender medicine internationally and domestically, ethics in gender medicine, guidelines for gender medicine research, medical education, hypertension, lupus, stroke, and geriatric medicine. These events have sparked understanding and interest in gender medicine within the domestic medical field. In its 2023 annual meeting, the association invited distinguished speakers, including Dr. Masahiro Akishita from the Japan Society for Gender-Specific Medicine and Professor Londa Schiebinger from Stanford University. Evidently, the association is of interest to the medical community. To date, the association has published five issues of its newsletter, featuring 15 articles on gender medicine. The association aims to foster progress and innovation in gender medicine in Taiwan, ultimately enhancing overall medical quality and patient welfare.

結核病由結核分枝桿菌引起,透過空氣傳播,全球仍是重大公共衛生挑戰。根據WHO數據,2022年全球結核病例約有1,060萬,130萬人死亡。台灣2023年報告6630確診個案及457死亡案例。潛伏結核感染(latent tuberculosis infection, LTBI)指結核菌在體內休眠,不致病也不具傳染性,但可能轉為活動性結核病。診斷依賴皮膚測試或血液檢測,治療目的是防止未來發展為活動性病例。針對高風險族群進行LTBI治療是全球消除結核病目標的重要策略之一,如接觸者、HIV感染者、以及具發病高風險之潛在疾病者等。治療方式包括多種藥物處方,並需監測副作用。台灣政策已導入相關策略,以期達成2035年消除結核的目標。 Tuberculosis (TB) is a disease caused by infection with Mycobacterium tuberculosis. Transmitted through the air, TB remains one of the great challenges in global public health and infectious disease control. Latent TB infection (LTBI) refers to a state where TB bacteria survive in the body but remain dormant and inactive, not causing clinical symptoms such as cough, fever, weight loss, or night sweats; thus, LTBI is not contagious. However, untreated LTBI can later develop into active tuberculosis. The bacteria will begin to proliferate and cause disease-related symptoms. TB-related abnormalities may be detected through imaging checks such as X-rays even without obvious clinical symptoms. The World Health Organization estimates that about one-quarter of the global population has LTBI. Although most individuals with LTBI do not develop active TB in their lifetime, those with weakened immune systems, such as patients with HIV, diabetes, or on immunosuppressive drugs, and older adults with age-related immune decline, face a significantly increased risk of LTBI progressing to active TB. This article introduces the latest global and Taiwanese strategies and advancements in diagnosing and treating LTBI, aiming to enhance health-care workers’ understanding of LTBI treatment, thereby contributing to Taiwan’s goal of eliminating TB by 2035.

鑒於風濕免疫中心照護的病人多為重大傷病及難治疾病,且易合併共病,如早發性心血管疾病、間質性肺病變及骨質疏鬆等,進而減低平均餘命及降低生活品質,因此亟需優質的跨領域團隊合作及持續的照護品質提升,方能達成全人照護目標。故風濕免疫中心參加第24屆醫策會國家醫療品質獎系統類卓越中心競賽,期待藉由外部評核來改善缺失及進行品質優化。 本中心自2016年成立以來,依據優質的醫療照護需求及嚴謹的全面品質要求,建置優質的跨領域醫療照護團隊及建立共同照護及轉介簡易流程,提供「病人至上」之創新與全方位醫療服務,並在選擇標靶藥物治療時,鼓勵病友參與醫療決策,因應2020年新冠疫情爆發,更著手建置醫療照護之雲端資訊系統,以達衛教無時限及醫病零距離。為能提升醫療照護品質,訂定具時效性的關鍵指標及優質照護相關的標準作業流程,並運用商業智慧進行品質監控及藉由PDCA手法進行改善。近五年來成效卓越,包括質量兼具的醫療照護品質、線上衛教高總數及高滿意度、提高風險藥物(標靶藥物)使用的安全性、及明顯提升病友的自我照護品質等。 Patients under the care of the Rheumatology and Immunology Center often experience severe and refractory diseases that are accompanied by comorbidities such as early-onset cardiovascular disease, interstitial lung disease, and osteoporosis, which reduce both life expectancy and quality of life. High-quality interdisciplinary teamwork and continuous improvement in care quality are required to achieve holistic patient care. Thus, the Rheumatology and Immunology Center participated in the 24th Joint Commission of Taiwan National Healthcare Quality Award for the Quality Improvement System category, aiming to use external evaluations to identify deficiencies and optimize quality. Since its establishment in 2016, the Rheumatology and Immunology Center has developed a high-quality interdisciplinary medical care team and established streamlined processes for collaborative care and referrals, meeting the demand for superior medical care and the rigorous requirements of total quality management. With a patient-first approach, the center provides innovative and comprehensive medical services and encourages patient participation in medical decisions, particularly in selecting targeted drug therapies. In response to the COVID-19 outbreak in 2020, the center also implemented a cloud-based medical care information system to achieve continuous health education and seamless doctor–patient communication. To enhance care quality, the center established timely key performance indicators and standard operating procedures related to high-quality care, using business intelligence for quality monitoring and the Plan-Do-Check-Act method for improvement. Over the past 5 years, the center has achieved remarkable results, including high-quality medical care, extensive online health education with high satisfaction rates, improved safety in the use of high-risk (targeted) medications, and significantly enhanced patient self-care quality.

Methods: This was a single-blind experimental study. HCP of cerebral neurological departments were invited and randomly assigned to the intervention group (IG) (n=53) or control group (CG) (n=60) block by their wards, with the IG participating in online EHPC CST and the CG receiving regular in-service training. The outcomes of the confidence in communication and preparation of palliative care were collected before (T0) and immediately after intervention (T1). Hierarchical regression was used for data analysis. Results: This study found no significant differences in confidence in communication (B = -5.456, p = 0.341) and preparation of palliative care (B= -0.612, p = 0.667) between the IG and the CG. Conclusion: Most HCP in this study already have palliative referral perceptions and behaviors, so there is limited room for improvement in interventional measures. However, during the COVID-19 epidemic, researchers should continue to strive to revise and improve the quality of online education courses to promote early hospice palliative care.

To understand the operation of domestic traumatic brain injury (TBI) care teams and the benefits of certification for such teams. Method: We evaluated the motivations for applying for DSC—TBI certification, performance scores, and certification recommendation reports of care teams that participated in the DSC—TBI certification program from 2019 to 2021. Furthermore, we investigated whether the performance and certification benefits differed among teams of different levels with identical certification standards. Result: The results revealed that the teams’ primary motivation for participating in the DSC—TBI certification program was to improve the quality of care they provided. The performance of the medical center in this accreditation is better than hospitals at other levels. Regarding certification benefits, the teams were most satisfied with the operation of the teams after integration. Conclusion: We suggest that other care teams refer to the common problems encountered by the teams included in this study and comprehensively inspect the operation of their teams. The health authorities should provide integrated relevant healthcare incentive subsidies policies.

2023年醫院評鑑暨教學醫院評鑑於臺北區與北區有14家醫院參加醫學中心評鑑,分析統計醫學中心任務指標任務三卓越的醫療品質與病人安全的4個項次9條基準32款評分說明的執行結果,大部分皆能達到評分說明的要求,有少數醫院可能未在任務指標審查資料表說明,或缺少部分內容,但整體都能呈現出卓越的醫療品質與病人安全成果。不過醫院投入醫品病安金額占醫院總收入的佔比差異很大,臺灣病人安全通報系統(TPR)件數與其他全院性通報系統之通報件數,每家醫院呈報的數字差異也很大,可能與醫院規定的病人安全通報類別與流程不同導致,將來也許需要清楚的界定。2023年醫學中心評鑑結果,各醫院評鑑成績持續進步,差異逐漸縮小,14家申請醫學中心評鑑之13家醫院,成績均達醫學中心評鑑標準,顯示參加醫學中心評鑑的醫院,都具有一定的水準,尤其醫療品質與病人安全皆能按照任務三的基準執行,得到良好的表現。建議以後,除落實執行衛福部規定的醫療品質及病人安全工作目標外,病人安全通報(TPR)後,進行根本原因分析(RCA)應確實按照衛福部「病人安全事件根本原因分析作業程序」參考指引執行,找出造成重大病安事件或流程執行失效根本原因,提出具體改善措施,以防止再次發生危害事件的機會。 In 2023, 14 hospitals in Taipei and in the North District of Taiwan applied for Hospital and Teaching Hospital Accreditation. An analysis and compilation of the implementation results of the medical center’s mission indicators for Mission Three, “Excellent Medical Quality and Patient Safety,” revealed 4 categories, 9 standards, and 32 scoring instruction items. The majority of the implementation results met the requirements of the scoring instructions. A few hospitals may not have provided explanations in the mission indicator review information sheet or may have been missing some content, but overall, they demonstrated excellent medical quality and patient safety results. Nevertheless, the proportion of each hospital’s investment in medical quality and patient safety relative to its total revenue varied greatly. The number of reports submitted to the Taiwan patient safety reporting system and other hospital-wide reporting systems also differed greatly among hospitals, possibly due to differences in the patient safety notification categories and procedures prescribed by each institution. These aspects may require clearer definition in the future. The medical center accreditation results demonstrated continued improvement in each hospital’s scores, with the differences gradually narrowing. In total, 13 out of the 14 hospitals that applied for medical center accreditation achieved scores that met the accreditation standards, demonstrating a certain level of excellence, particularly in implementing Mission Three’s standards for medical quality and patient safety and achieving good performance. In the future, hospitals should not only implement the medical quality and patient safety goals stipulated by the Ministry of Health and Welfare but also conduct root cause analyses following Taiwan patient safety reporting system in accordance with the Ministry of Health and Welfare’s ""Root Cause Analyses Operational Procedures for Patient Safety Incidents."" Hospitals should identify the root causes of major health and safety incidents or process execution failures and propose specific improvement measures to prevent the recurrence of hazardous incidents.
