Comparison between two types of grids for placement of mini-screw implant in posterior maxillary segment 上颌后段微型螺钉种植体两种格架放置的比较

Use of mini-screw implants is in vogue. It has brought about a drastic change in day to day practise for orthodontists. Despite being the most popular source of anchorage, it is not easy for newer clinicians to perfectly figure out the site of implant placement and the accuracy by which it should be placed to avoid failure. A customised grid or mesh is the simplest way to gain assistance for placement of miniscrew implants. This article focuses on comparison of two types of grids for placement of implant accurately and to guide the clinician in such a way that minimises all the possible errors.

Management of impinging “Nickel titanium palatal expander” in cleft palate cases by “Soft flow” (temporary mucosal protector) “软流”(临时粘膜保护器)治疗腭裂碰撞型“镍钛腭扩张器”

The temperature-activated Arndt nickel titanium palatal expander has been a good choice for cleft palate applications because it creates transverse maxillary expansion, uprights and rotates the maxillary molars, and allows a smooth transition to fixed retention with no requirement for patient compliance and little manipulation by the clinician. But the clinical problem sometimes encountered is the impinging of “Nickel-Titanium” metal component of the Nickel titanium palatal expander on the soft palate. “Soft Flow” is a Temporary Mucosal Protector introduced recently in the orthodontic market which prevents the impinging of micro- implants or hooks of the molar buccal tubes. The following article highlights the clinical problem encountered after placement of Nickel Titanium Palatal Expander and how “Soft Flow” helps to overcome it.

Extra oral appliances have been used since ages to bring about dental changes, skeletal changes and for anchorage reinforcement. Growth modifications can be done with help of these appliances. This can be done by inhibiting the growth or redirection of the growth. The biomechanics of the extraoral appliances needs to be understood well for using these appliances. The topic of headgear is vast and the following article tries to cover the details regarding the same in brief. The location of centre of resistance for application of force is important and plays a significant role in biomechanics and outcome of treatment results. Various types of headgears and their use depending on different criterias are discussed in detail.

Do medical and paramedical students have a better awareness regarding oral health practices and orthodontic treatment? A KAP study 医科学生和准医科学生对口腔健康和正畸治疗有更好的认识吗?KAP研究

oral health is the most important for the wellbeing of the patient. But it is being neglected in the developing and the underdeveloped countries and malocclusion being the third most commonly occurring dental anomaly. Medical and paramedical students have been found to be the point of contact the community on a large scale. Hence, the present study was carried out to evaluate the awareness regarding oral health and orthodontic treratment in KIMS College. A validated self-administered questionnaire, pertaining to demographic data, awareness of oral health knowledge regarding orthodontics as a specialty were included. A total of 42 questions included in the study. The answers from the students were recorded as Yes or No on a 2-point Likert scale.Significantly higher proportions (70.7%) of the dental students were aware of the oral health practices, Orthodontic treatment and the purpose of orthodontic treatment followed by the, medical students. Female students opined with a greater frequency (70.8%) that irregular teeth have a negative impact on self-esteem and quality of life compared to male students. The Dental students have more knowledge than the health care workers on awareness of oral health practices regarding the use of mouthwashes, flossing technique etc. and also regarding the orthodontic treatment and its importance.

An innovative approach for treatment of anterior open bite in Class II division 1 patient with zygomatic implant and spur assisted appliance - A case report 一种创新的方法治疗II类1级患者的前开咬,颧种植体和骨刺辅助矫治器- 1例报告

Controlling anchorage is one of the foremost necessary aspects of treatment. Moderate anchorage is comparatively simple to manage with the use of some intraoral appliances and biomechanical procedures. On the opposite hand, cases that need the most anchorage need extraoral support to strengthen the anchorage. In some instances, 100 percent anchorage has got to be maintained, such an anchorage may be termed as absolute anchorage. It's tough and not possible to get absolute anchorage by using typical ways like extraoral force application. In this case report, we describe the treatment of Open bite by using the support of Zygomatic plates and Spurs anteriorly placed with the combination of posterior bite blocks.A 20-year-old female reported to our department of orthodontics with the chief complaint of anteriorly placed upper front teeth. There was no relevant medical history; she was having tongue thrusting habit, acute nasolabial angle and incompetent lips. Overjet of 12mm and anterior skeletal open bite from canine to canine of 5mm was present associated with anterior tongue thrusting habit, and constricted upper arch. The molar relation was end-on on the right side and class II on left side. Canine relation is end-on, on both the side. Growth pattern is vertical. Upper arch is well aligned with constriction in premolar region; lower arch has crowding of 6 mm. The treatment plan was selected as alignment of the teeth with fixed mechanotherapy and impaction of the maxillary posteriors using zygomatic anchorage with posterior bite block and spurs soldered to the anterior part of the appliance. The sutures were removed after 7 days, the appliance was into the patient’s mouth and secured with a power chain to the hooks of the appliance and the hook of zygomatic implant, 400gms of force was applied on both the sides.

Comparative evaluation of accuracy, time and patient acceptance between intraoral scanner and conventional alginate impression technique – An invivo study 口腔内扫描与常规海藻酸盐印模技术的准确性、时间和患者接受度的比较评估-一项体内研究

Orthodontists use various dental records such as dental models or casts for diagnosis and treatment planning which includes making of dental impressions and study casts. Conventional alginate impressions are challenging for the patients with extreme gag reflex, irritation & discomfort. Recently 3D technology has led to development of scanning and digital models, but one major concern has been the accuracy. So this study was done to compare the accuracy, time and patient acceptance between intraoral scanner and conventional alginate impression technique. 15 patients were selected. Alginate impressions were made using zhermack neocolloid. The patients were subjected to digital scanning with Medit i500 intraoral scanner. Procedures were timed. After the impressions, each patient was asked to complete survey. Tooth width measurements were made using digital vernier caliper from stone models. In the second method digital images were measured using medit link software. Anterior and overall Bolton ratio was calculated to determine accuracy. Digital impressions are accurate and comparable to conventional impressions as tooth width measurements did not differ significantly. Conventional impression consumed more time. Patients preferred digital impressions.Intraoral scanners are accepted by patients and they have comparable accuracy and time efficient compared to conventional impression.

This article explores the conundrum of biofilm formation in orthodontics, its impact on oral health, and the role of the bonding procedure. Biofilms are resilient structures formed through stages like attachment, adhesion, growth, and maturation, with the extracellular matrix (EPS) playing a vital role. Management of skeletal Class III malocclusion using two-phase therapy in young patient - A case report 两阶段治疗治疗年轻患者骨骼III类错牙合1例

Maxillary and mandibular skeletal discrepancies are one of the commonly prevailing malocclusion in the childhood. Many a time it is ignored by the parents as well as the clinicians. It can present itself with aesthetic, functional and psychological problems of the child. Proper identification, diagnosis and treatment planning at the proper age can lead to correction and improvement in the overall facial balance and psychological esteem of the child. Over the years orthopedic and myofunctional appliances has been used to correct Class II and Class III skeletal malocclusion in a growing child. Face mask therapy with RME has been used as treatment choice to treat a patient with skeletal Class III malocclusion. Orthopedic correction in the early stage of life can eliminate the severity and further complex treatment modalities in the later stages of life. This case report describes the treatment of male child aged 12 years who had skeletal Class III malocclusion with maxillary deficiency. The treatment was done in two phases; orthopedic correction was done initially using RME Facemask followed by fixed orthodontic correction. There was overall improvement in the facial profile and occlusion of the patient post treatment.

Orthosurgical synergism in the management of severe skeletal Class-III malocclusion- A case report 矫形外科协同治疗严重骨骼iii类错牙合1例报告

: What is now known as a Class-III malocclusion was initially recognized by Pierre Fauchard. True Class-III malocclusion is a genetically based condition that manifests in childhood. As the youngster reaches the teenage growth spurt, it becomes more obvious. Typically, we will discover a parent or grandparent who is dealing with the same issue. Class-III malocclusion, however, is not a disease. The abnormality itself is a compensatory mechanism to make up for variation that has happened at some point during the course of the individual's development.The case presented here demonstrates the importance of presurgical decompensation, surgical planning, including cephalometric predictions, and mock surgery in the management of severe skeletal Class-III malocclusion. The treatment included comprehensive orthodontic and surgical workup that included bilateral sagittal split osteotomy for mandibular setback of 3mm and LeFort-I osteotomy for maxillary advancement of 3mm. Class-I jaw relationships with a pleasing profile and correctly aligned arches were attained after 2 years of detailed treatment. This led to an aesthetic improvement and a significant increase in the patient's confidence. Class-III skeletal malocclusions can be corrected surgically or through camouflage. However, an ortho-surgical approach may be necessary if the issue cannot be resolved with orthodontics alone. When done correctly, bi-jaw surgery has repeatedly been shown to be quite successful.

Evaluation of surface-modified orthodontic brackets with silver nanoparticles and its influence on bacterial inhibition – An in vitro study 纳米银表面修饰正畸托槽的评价及其抑菌效果的体外研究

: Compared to other fixed orthodontic appliances, orthodontic brackets have a substantial impact on enamel demineralization. This demineralization is a result of organic acid production by various microorganisms, primarily from Streptococcus mutans and Lactobacillus acidophilus. Preventing white spot lesions and caries during orthodontic treatment presents a considerable challenge for clinicians. Silver exhibits broad-spectrum antimicrobial activity against various microorganisms, including Gram-positive and Gram-negative bacteria, certain viruses, fungi, protozoa, and antibiotic-resistant strains. As such, the purpose of this research is to investigate the antibacterial properties of orthodontic brackets coated with silver nanoparticles against Streptococcus mutans and Lactobacillus acidophilus. The study involved 80 stainless steel orthodontic brackets, which were divided into four groups, each containing 20 brackets. In each group, there were uncoated brackets serving as the control group, and the experimental group consisted of brackets coated with silver dioxide. Coating was done by physical vapor deposition with the help of RF Magnetron sputtering unit. Surface morphology and material composition was analyzed by SEM and EDS unit. Afterward, the brackets underwent microbiological tests to evaluate their antibacterial efficacy against Streptococcus mutans and Lactobacillus acidophilus.The study found that silver nanoparticle-coated stainless-steel brackets exhibited effective antibacterial properties against both Streptococcus mutans and Lactobacillus acidophilus when compared to the uncoated brackets. Silver nano particle coated SS brackets showed increased anti-bacterial effect towards S. mutans than L acidophilus.Using silver nanoparticles to coat orthodontic brackets represents an innovative and effective approach to prevent the formation of white spot lesions in patients receiving fixed orthodontic treatment. The antimicrobial properties of silver nanoparticles can combat bacteria like Streptococcus mutans and Lactobacillus acidophilus, responsible for enamel demineralization and the occurrence of white spot lesions. This advancement in bracket technology has the potential to significantly reduce the risk of enamel damage and improve overall oral health outcomes for orthodontic patients.
