Background: Smoking may cause a low-grade inflammation on vascular endothelium terminating with an accelerated atherosclerosis-induced end-organ insufficiencies. Plasma triglycerides may be positive acute phase reactants (APRs) whereas fasting plasma glucose (FPG) negative APR indicating the inflammatory effects of smoking. Method: Consecutive patients with irritable bowel syndrome (IBS) and age and sex-matched control cases were included. IBS was diagnosed according to Rome II criteria in the absence of red flag symptoms including pain, nocturnal diarrhea, weight loss, fever, or any abnormal finding of the physical examination.

Leptin is predominantly generated in adipose tissue and circulates in serum both as a free and as a protein‐bound entity. Leptin, a 16 kDa nonglycosylated polypeptide product of the obese (ob) gene, is an adipocyte-derived hormone which has long been recognized as a key factor in regulating a wide range of biological responses involving energy homeostasis, neuroendocrine function, angiogenesis, bone formation and reproduction. Leptin is a key afferent signal linking adiposity level and nutritional status to neuroendocrine regulation of energy homeostasis chiefly through decrement in caloric uptake and enhancement in energy expenditure. Serum levels of leptin reflect the amount of energy stored in adipose tissue. Short-term energy imbalance as well as serum levels of several cytokines and hormones influence circulating leptin levels. Leptin acts by binding to specific receptors in the hypothalamus to change the expression of several neuropeptides that regulate neuroendocrine role and energy uptake and expenditure. Leptin plays a significant function in the pathogenesis of obesity and eating disorders and is thought to mediate the neuroendocrine response to food deprivation

Salivary gland neoplasm is rare entity in head and neck region, out of that pleomorphic adenoma is most common benign tumor. Submandibular gland is very unusual site for its occurrence while the parotid gland is the most prominent location. Here, we are presenting a case report of a twentyeight-year-old female patient with a swelling at right submandibular region which was managed by surgical excision.

This is a research-based perspective article into patient non-compliance within the domains of medicine, physical rehabilitation, and exercise/strength and conditioning. The article identifies major pillars behind patient non-compliance: age, education, motivation, and trust as being the most influential variables that affects the amount of compliance a patient has with medical/exercise and therapeutic treatments. The article summarizes research that has identified theses variables and gives pragmatic options to help reduce the occurrence of ongoing patient non-compliance. Communication, patient compliance, motivational interviewing, virtual information gathering and barrier identity are all techniques discussed to overcome patient non-compliance. There are pragmatic tips for medical providers, therapists and trainers to assist in the management of non-compliance in their patients and clients in all aspects of practice in an evidence-based approach.

Diagnostic imaging plays a significant role in both the diagnosis and treatment of complications of pneumonia in children and chest radiography is the imaging modality of choice. Computed tomography (CT) on the other hand, is not currently a first-line imaging tool for children with suspected uncomplicated community-acquired pneumonia and is largely reserved for when complications of pneumonia are suspected or there is difficulty in differentiating pneumonia from other pathology. This review outlines the situations where CT needs to be considered in children with pneumonia, describes the imaging features of the parenchymal and pleural complications of pneumonia, discusses how CT may have a wider role in developing countries where human immunodeficiency virus (HIV) and tuberculosis are prevalent, makes note of the role of CT scanning for identifying missed foreign body aspiration and, lastly, addresses radiation concerns.

We report a case study of an adult man with severe COVID-19 who attended Psychological Intervention after prolonged hospitalization in the Intensive Care Unit, intubation and finally, participation in Rehabilitation Programme of a Greek Teaching Hospital in Athens. To export results, a qualitative data analysis were collected from Focused Interview, Psychological Intervention based on Short/ Systemic Psychotherapy and a quantitative data analysis from the Beck Depression Inventory and further triangulation of qualitative and quantitative data took place. The study focuses on the psychology of COVID-19 survivor patient, in line with complex needs, psychological effects and specific stressors, following a life-threatening disease. In the light of a therapeutic ‘flexibility’ and finally of a combination of multi-factorial nature of the disease that requires integration and simultaneous exploring of not only the physical, but also of the psychological needs of the patient

Plantar fasciitis (PF) is a common musculoskeletal problem that causes so much disability in the foot. Recently, platelet-rich plasma (PRP) injection has been recommended for the treatment of patients with chronic plantar fasciitis (PF), particularly where physiotherapy or other conservative treatments have failed or pain is limiting exercise, in order to reduce heel pain and improve functional restoration. In this case study, a patient who PF for 16 years and had failed all conservative treatments including cortisone injection was treated with platelet-rich plasma injections twice and made significant improvements with 100% reduction in pain intensity measured using numeric pain scale (NPS) from 10 to 0 and improvement in her lower limb functional by 56% from 68 to 24/80, which was assessed using lower extremity functional scale (LEFS) score. There were no safety issues recorded. Conclusion: Injection of autologous PRP for chronic plantar fasciitis was found to be beneficial in terms of pain relief (reduced NPS score) and functional outcome (improvement in LEFS score). However, a large scale and a properly randomised controlled study is needed to prove further efficacy of PRP injection.

Introduction: The effectiveness and safety of romiplostim were evaluated by immune thrombocytopenia (ITP) phase (newly diagnosed/persistent/ chronic) at romiplostim initiation. Methods: This is a post hoc analysis of a prospective, Japanese, multicentre, observational study in adults with ITP who received ≥1 dose of romiplostim. Follow-up data were collected for ≤2 years. Outcomes included overall platelet response (≥1 platelet count ≥50 × 109/L at 2–24 weeks after romiplostim initiation) or durable platelet response (≥75% of measurements ≥50 × 109/L at 14–24 weeks) and adverse drug reactions (ADRs), evaluated by ITP phase.

Remote Patients’ Monitoring System is an essential feature of remote healthcare efficiency and durability. It assures adequate clinical supervision and care to admitted patients in a timely and effortlessly manner. The objective of this case study is to show the impact of a health informatics solution application through the implementation of a remote physiological monitoring system in a cardiac care center. It also discusses the following questions: how health informatics was employed in this case study? how did this case study help healthcare providers to a better performance? how can we decide if we need to implement such a system in other organizations? In addition, an overview of the Philips, IntelliVue Patients’ Monitoring System (PMS) will be demonstrated and justified. The problem statement will be introduced, then, the organizational contextual settings, summary of key facts, problem implications, and the actual solution tailored and achieved by KACC management is verified in an advance detail.
