Here in this abstract we retrospectively review preliminary findings on 374 samples of circulating DNAextracted from 173 patients treated by multitargeted epigenetic therapy (MTET), which is a combination of natural histone deacetylase inhibitors and DNMT methyl transferase inhibitors. This therapy could dynamically interrupt the expression of altered genes, in a variety of solid tumor types, both in invitro and invivo models. We hypothesize that serial monitoring of the circulating DNA provides a feasible option for therapeutic decisions making based on presence of the driver genes in these cases. We also were able to track the antineoplastic response in these group of patients by monitoring their tumor circulating DNA mutated allele fractions and propose a direct correlation with interim epigenetic therapy effectiveness.

Metaplastic breast carcinoma accounts for less than 1% of all breast malignancies. This subtype is the most aggressive triple negative breast cancer and remains therapeutically challenging to treat, highly unresponsive to chemotherapy. Here we present a case of a 54 year old female presenting as a non-tender, left breast mass. Core needle biopsy reports atypical cells present, suggestive of malignancy: considerations are Malignant Phyllodes tumor and Invasive Ductal Carcinoma. Subsequently she underwent a modified radical mastectomy (MRM). Microscopic examination revealed spindle cell neoplasm without epithelial component. On immunohistochemically examination, the tumor cells were positive for Cytokeratin and Smooth Muscle Actin and negative for S-100, ER (Estrogen receptors), PR (progesterone receptors) and Her2/Neu.

Acute Cholecystitis is a common condition that may present with unusual complications like gallbladder perforation. Such an uncommon complication usually presents a challenge to surgeons to diagnose and provide the necessary management on as early as possible. The mortality rates are directly related to early surgical intervention. Therefore, it is crucial to keep in mind such a complication and know when to further investigate such patients. In this report we will describe a case of acute perforated cholecystitis which was almost missed due to the unspecific clinical presentation.

Background: Ultrafiltration, which is currently considered as a standard method to remove the excess water administered during cardiopulmonary bypass (CPB), aims to minimize the adverse effects of hemodilution, such as tissue edema and blood transfusion. Three ultrafiltration techniques can be used before, during and after CPB procedure, including conventional ultrafiltration (CUF), zero-balance ultrafiltration (Z-BUF), and modified ultrafiltration (MUF) at the end of CPB. The aim of study: The present research attempts to revise the efficiency of Z-BUF ultrafiltration method, laboratory results, and clinical impacts. Material and methods: 94 adults (54 men and 40 women) with acquired heart diseases undergoing a single cardiac surgical procedure in condition of cardiopulmonary bypass (clamp aortic 89, 9± 38, 8 min. and pump 135,14 ± 45,17 min.) were divided into 3 groups. 1st group (no ultrafiltration) - 34 patients with classic cardiac surgery, 2nd with the use of zero-balance ultrafiltration (ZBUF) group - 37 patients with classic cardiac surgery, and 3rd group ZBUF with mini invasive cardiac surgery - 23 patients.
