From the inside: Dancing between burnout and engagement in a prison. 从内部看:在监狱里的倦怠和投入之间跳舞。

Objectives: Professionals who work in prisons present high levels of chronic stress that affect their daily performance and health. The incorporation of training workshops on emotional regulation and stress coping strategies from a psycho-body approach could have a positive impact on their well-being. This article presents an exploratory study that analyzes chronic stress, emotional repertoire and body awareness, of a sample of 15 professionals from a prison, together with the effect produced by an intervention through training workshops with Dance Movement Therapy.

Material and method: A mixed methodology with a pretest-posttest design was used. The variables of burnout, engagement, emotional repertoire and body awareness were measured. With the information obtained from a participatory focus group, a six-hour body awareness program was designed and implemented, during which participants completed reflective diaries for subsequent analysis.

Results: High levels of stress and low body awareness were found. Qualitative analysis generated evidence for improvements in body awareness and the incorporation of coping strategies based on listening to body sensations and emotional regulation. Pre-post test analysis for a subsample (n = 4) of the total showed improvements in different subscales, not statistically significant, but with medium effect sizes.

Discussion: Despite the limitations of the study, the results are promising. The participants value the workshop very positively and consider having incorporated tools to cope with work stress.

38 The prevalence of mental disorders in prisons both in Spain and abroad has clearly increased in recent years1-6. The causes leading to this increase in the number of inmates are many and varied: from the psychiatrisation of day to day problems (as is the case in the community), to the high levels of substance abuse prior to entering prison and during the prison sentence, the effects of prison life and the existence of a mental illness presented prior to the legal proceeding that was diagnosed (or not) during same. There are several studies that indicate that the general public commits proportionally more crimes7-9, but it is no less true that in situations where alcohol or drugs are consumed and during periods of delirium tremens or the start of a paranoid psychosis, the resulting episodes of violence may help to explain why a part of the prison population in ordinary prisons is labelled as such10. However, the aim of this editorial is not to offer a detailed analysis of the causes of such a high prevalence of mental disorder, but rather on how to deal with this important problem as effectively and ethically as possible. Prison healthcare in Spain is the responsibility of the Ministry of Home Affairs, which means that the prison primary healthcare teams that answer to the ministry are responsible for caring for inmates. When diseases requiring specialised care are identified, the teams carry out the traditional inter-consultation process with the reference hospital in the province where the prison is located. This entails a degree of coordination with specialists and transfer of the inmate by the police. The most common medical problems that require specialist care are usually linked to dental, mental health and physical trauma issues11. The most commonly observed mental pathologies are schizophreniform disorders, affective disorders and inmates with type B traits B (impulsiveness, low tolerance to frustration, emotional instability), drug consumption, and in many cases there is a dual pathology5,6,12. In response to the growing frequency of these disorders, the Ministry of Home Affairs designed a comprehensive care programme for the mentally ill (PAIEM in Spanish), which contains a protocol that establishes the bases for correct identification of a case, treatment and monitoring through an internal social-healthcare process13. The psychiatric consultation system continues to be the predominant approach used in Spain, with the exception of Catalonia and the Basque Country, who have transferred competences in healthcare matters and whose organisation is very different. The comorbidity of diabetes-depression and its association with disability amongst elderly prison inmates. 老年监狱囚犯中糖尿病-抑郁共病及其与残疾的关系

Objectives: The objective of the study was to verify the strength of association of depression as a comorbidity of diabetes with the presence of six types of disability in the elderly in prison.

Material and method: Cross-sectional association study based on a secondary analysis of the 2016 Peru National Penitentiary Census. The population were older adults, who were held in prisons in Peru. Inmates 60 years of age or older, and both sexes were included. The response variables were the six types of permanent disability. The exposure variables were the self-report of diabetes, depression and the comorbidity of depression with diabetes diagnosed by a health professional. The sample was constituted by 2. 658 older adults.

Results: There was an increase in the probability of presenting the six disabilities analyzed due to the presence of comorbid diabetes with depression (p <0.05). The measure of greatest strength of association is for the disability to relate OR(a) 10.23, followed by the disability to move OR(a) 6.12 and the lowest strength of association found was for the hearing impairment OR(a) 2.80.

Discussion: A significant increase was found in the probability of presenting the six disabilities analyzed due to the presence of comorbid diabetes with depression compared to suffering from only one of these conditions or not suffering from them.

Mental and behavioral disorders in the prison context. 监狱环境中的精神和行为障碍。

Introduction: During the execution of sentences in prison units, there are deficiencies within the adaptation mechanisms of persons deprived of liberty, this varies depending on the place where they serve the sanction and the length of stay in it.

Objectives: To collect information on the prevalence of mental and behavioural disorders in the prison setting, as well as the importance of early psychiatric intervention.

Material and method: Theoretical review. Primary sources of information: scientific articles from indexed journals, in specialized medical information search engines. Original publications on mental and behavioural disorders in the prison and prison context were included, from 2016 to April 2021, with an analytical, observational, prospective, retrospective, cross-sectional and randomized design, systematic reviews and meta-analyzes, complete articles, carried out in any country, with subjects over 15 years of age, of any sex or gender, in English and Spanish.

Results: 16 articles were included in this theoretical review. The prevalence of mental disorders within the prison population is high; depression, anxiety, substance use and psychotic disorders predominate. The importance of having mental health programmes in prisons, with initial diagnosis and personalized interventions, was observed. The authors recommend psychopharmacological interventions and cognitive behavioural management.

Discussion: The need to restructure the mental health approach in prisons was evidenced, and early diagnosis and personalized follow-up should be guaranteed. Pharmacological intervention and, to a greater degree, cognitive behavioural therapy seem to be effective for these types of patients.

Abstract Objectives To assess the perceived health and quality of life, perceived social support, prison life, and cannabis use among a sample of inmates in Spanish prisons. Material and methods We hosted a discussion with inmates in order to learn their reasons for using cannabis (phase 1). Then, a questionnaire was prepared specifically for this study (phase 2), containing indicators from the Catalan Health Survey and other items regarding patterns of cannabis use and life in prison. The questionnaire was administered by the authors to a sample of Catalan inmates. Results and discussion A sample of 63 inmates completed the questionnaire. Cannabis was used mainly for therapeutic purposes, which can be interpreted as a coping strategy related to the stress associated with prison life. Nearly half of the subjects used cannabis as a substitute for prescription medications and illicit drugs. It was found that the only variable that predicted self-perceived health status was the number of visits inmates received. This suggests that social support is a key factor for improving the health and well-being of inmates. Cannabis use in prison seems to be related to coping with a challenging context. The benefits and risks of this practice need to be further researched.

Material and methods We hosted a discussion with inmates in order to learn their reasons for using cannabis (phase 1). Then, a questionnaire was prepared specifically for this study (phase 2), containing indicators from the Catalan Health Survey and other items regarding patterns of cannabis use and life in prison. The questionnaire was administered by the authors to a sample of Catalan inmates. Results and discussion A sample of 63 inmates completed the questionnaire. Cannabis was used mainly for therapeutic purposes, which can be interpreted as a coping strategy related to the stress associated with prison life. Nearly half of the subjects used cannabis as a substitute for prescription medications and illicit drugs. It was found that the only variable that predicted self-perceived health status was the number of visits inmates received. This suggests that social support is a key factor for improving the health and well-being of inmates. Cannabis use in prison seems to be related to coping with a challenging context. Substantial concordance between transient elastography and APRI and FIB-4 combination amongst hepatitis C inmates with non advanced liver fibrosis 短暂弹性成像与APRI和FIB-4联合在丙型肝炎非晚期肝纤维化患者中的一致性

Abstract Objectives To establish concordance between transient elastography (TE) and non invasive markers (NIM) APRI and FIB-4 combination in cronic hepatitis C (HC) patients with non-advanced liver fibrosis (NALF). Material and method Multi-centre retrospective study carried out at two different Barcelona Prisons HC inmates who had the TE done at 2019. We compared the ET vs. NIM results. The NALF consideration was ≤2 (≤12.5 Kilopascal (kPa) in TE). In the NALF cases was calculated de NIM APRI and FIB-4 and the kappa index agreement was established between TE and NIM. Results 107 cases were included, but only 82 were assessable. The average age was 42 (DS: ±3.2) years. The 96.5% were men, 51.2% spanish, 70.7% drug users and 39% HIV co infected. The 45.1% of those HC infected had genotipe 1. The 90.2% of the evaluated patients by TE the ALD was not detected. The kappa index was 0.78. 65 (79.3%) studied inmates got HC treatment. The 20.7% could not be treated because the evaluation was not completed. Conclusion Most of the HC infected inmates have no ALD, and in such cases concordance between NIM/TE is substantial. The NIM can be used to shorten the evaluation time and prescribe the treatment faster, especially if the length of stay in prison is short and risk of transmission is high.

Healthy prisons: analysis of health promotion in Catalonian prisons 健康监狱:加泰罗尼亚监狱健康促进分析

Abstract Introduction There is currently sufficient evidence that health can be created through certain strategies combined and maintained long enough to produce measurable results: this is what is called health promotion. The World Health Organization has promoted programs based on healthy environments; among them we highlight healthy prisons. Living conditions in a penitentiary are extraordinarily specific, and involve limitations that can have significant health consequences. However, prisons can be environments for health, since there is the effective possibility of promoting it. Objectives To analyze the implementation of health promotion in prisons in Catalonia. Material and method Retrospective and observational cross-sectional multi-centre study, carried out on a sample of 29 nurses who worked in the closed-regime prisons in Catalonia between 2019 and 2020. The information collection instrument is made up of an ad-hoc questionnaire (Annex 1). A descriptive analysis of the socio-demographic variables and the variables of the study was carried out. A comparative analysis is performed using correlation and linear regression models. Results 82.8% of those surveyed state that a health promotion project is being carried out in the prison where they work, with mental health being the most carried out programs, followed by tobacco and drug cessation. Discussion The results show that health promotion policies are currently being developed in the Catalan prison environment.

Abstract Objectives To describe the clinical and epidemiological characteristics of inmates who were infected and died from COVID-19 in Peruvian prisons from April to October 2020. Material and method Descriptive, cross-sectional, retrospective study with a secondary data source. All the inmates who were infected and died from COVID-19 in Peruvian prisons during the study period were considered. The information was collected through the validated data collection sheet and was analyzed with descriptive statistics applying the SPSS v26 software. Results 37,103 (42.3%) inmates were analyzed out of a prison population of 87,754. All of them reactive to the rapid test, with a mean age of 39.9 ± 12.6 years; 95.5% were male. 60.1% were IgG reactive; 36.1% were IgM/IgG reactive and 3.8% were IgM reactive. 20.1% of reactive inmates had COVID-19 symptoms at the time of testing. The most frequent symptoms were headache (55.6%), general malaise (49.7%), fever (49.0%) and cough (48.0%). Among the risk factors for COVID-19 were: age over 60 years (8.4%), high blood pressure (2.8%) and diabetes mellitus (2.4%). 445 deaths were registered. Total fatality reached 1.2% of the number of infected. The highest number of deaths was recorded in April and May (89 and 162, respectively). Discussion The study findings imply different approaches to managing epidemics in the prison context compared to the general population. The short and long term scenarios are uncertain, but the need to reorganize the prison health system, and to prioritise and modernise it are evident. The long-delayed reform of the prison system and the measures that set out to contain the spread of the COVID-19 disease in prisons are related processes in this regard.
