Effectiveness of nursing interventions on level of anxiety, stress, and depression of elderly patients undergoing upper gastrointestinal endoscopy 护理干预对老年上消化道内镜患者焦虑、压力和抑郁水平的影响

Background: Gastrointestinal Endoscopy is a widely used procedure in medicine today. It is the diagnosis and treatment of choice among elderly patients with upper gastrointestinal disorders. Thus anxiety, stress, and depression of adverse out-come or side effect and discomfort regarding endoscopy reduce the patient’s acceptance ability. Effective procedure ex-planation and safety measures have positively impact on decrease level of anxiety, stress, and depression and improve elderly patient’s general health condition Aim: Evaluate effectiveness of nursing interventions on level of anxiety, stress and depression of elderly patients undergoing upper gastrointestinal endoscopy. Design: A quasi experimental study design with pre and post procedure was used. Setting: The study was conducted at Gastrointestinal Surgery Center (GISC) (Endoscopy Unit) Mansoura University Subjects: A convenient sample of 76 elderly patients undergoing GIT endoscopy. Data collected over a period of 9 months from first of May 2019 at the end of January 2020. Tools: Three tools were used: Socio Demographic Characteristics and Clinical Data Structured Interview Schedule, Depression, Anxiety and Stress scales (DASS-21), and Endoscopic Nursing interventions. Results: The study results revealed that statistically significant improvement in all aspects of elderly patient's anxiety score at pre and post nursing intervention (p<0.001). There were significant improvements of level of stress from pre and post implementation of nursing intervention to endoscopy (p<0.05). Also, there was a statistically significant difference in the pre, and posttest mean score of depression (p<0.05). Statistically significant differences regarding all sociodemographic characteristics within the studied subjects and their total mean score of anxiety, stress, and depression. Conclusion A clear and noticeable improvement in the mean score of anxiety, stress and depression of all studied subject after the implementation of the endoscopic nursing interventions than before. Recommendation: Replication of this study is highly recommended on a large representative probability sample to achieve generalizable results.
背景:胃肠内窥镜检查是当今医学中广泛使用的一种检查方法。是老年上消化道疾病患者诊断和治疗的首选方法。因此,对内镜检查不良结果或副作用的焦虑、压力、抑郁和不适降低了患者的接受能力。有效的程序解释和安全措施对降低老年患者的焦虑、压力和抑郁水平,改善老年患者的总体健康状况有积极的影响。目的:评价护理干预对老年上消化道内镜患者焦虑、压力和抑郁水平的影响。设计:采用前后程序的准实验研究设计。背景:本研究在曼苏拉大学胃肠外科中心(GISC)(内镜组)进行。研究对象:76例接受胃肠内镜检查的老年患者。从2019年5月1日至2020年1月底的9个月内收集的数据。工具:使用了三种工具:社会人口学特征和临床数据结构化访谈表,抑郁、焦虑和压力量表(DASS-21),以及内镜护理干预。结果:研究结果显示,护理干预前后老年患者各方面焦虑评分均有统计学意义的改善(p<0.001)。护理干预实施前后患者的应激水平有显著提高(p<0.05)。两组抑郁前、后测平均分比较,差异均有统计学意义(p<0.05)。在研究对象的所有社会人口学特征及其焦虑、压力和抑郁的总平均分方面存在统计学上的显著差异。结论实施内镜护理干预后,所有研究对象的焦虑、压力和抑郁平均得分均较术前有明显改善。建议:强烈建议在一个大的代表性概率样本上复制本研究,以获得可推广的结果。

Lowering blood sugar levels with kersen leaf stew (muntingia calabura l) and black onion (alium sativum) extract in mice (mus musculus) 克森叶炖菜和黑洋葱提取物降低小鼠(小家鼠)血糖水平

Diabetes mellitus (DM) is a metabolic disease characterized by an increase in blood sugar levels. Blood sugar levels in patients with DM are mostly challenging to control, and this will threaten the occurrence of complications. Alternative therapies used are active flavonoids in reducing blood glucose. The purpose of this study was to analyze the decrease in blood sugar levels by administering cherry leaves stew and black onions to mice (mus musculus). The design of this study was the Quasy Experimental design (pretest-posttest with control group design). The sample in this study was 32 mice, 16 in the intervention group, and 16 comparison groups. There are independent variables (Kersen Leaf Stew and Black Onion Extract) and the dependent variable (Blood Glucose Level). The data were collected using an observation sheet, then analyzed using the independent sample T-test and Kolmogorov Smirnov test. The results of this study showed that the pre and post administration of cherry leaf decoction was 36.5 mg/dl, and the difference between pre and post administration of black onion extract was 24.3 mg/dl. While the analysis showed p = value 0.98, it showed no difference in the effectiveness of the two extracts in reducing blood sugar levels in mice. Obtained comparative test results from the two extracts were equally effective in lowering blood sugar in mice. Thus further research needs to be done with more samples.

Perception of women towards contraceptive methods: using health belief model 妇女对避孕方法的看法:使用健康信念模型

Introduction: The Health Belief Model (HBM) is one of the major conceptual frameworks guiding current research as well as practice in the health sciences. It takes into account the multiple factors present in a person’s decisions to live a healthy life, seek help when needed, and maintain periodic check-ups and screening. Aim of the study is to identify perception of women towards contraceptive methods using the health belief model. Research design: A descriptive research design was utilized in this study. Setting: This study was conducted at four family planning clinics affiliated to four maternity hospitals namely: ELShatby Maternity University hospital affiliated to Alexandria University, Dar-Ismail Maternity hospital affiliated to Ministry of Health, Faisal hospital affiliated to Health Insurance and Dar ALWelada hospital affiliated to Medical Health Association. Subjects: A convenience sample of 320 women who were seeking family planning services. Tools; Tool I: Contraceptive users' basic data structured interview schedule; Tool II: Contraceptive users' Health Belief scale. Results: Approximately an equal percent (56.56% & 43.44%) of the contraceptive users perceived themselves either highly susceptible or moderately susceptible for threat of getting pregnant, respectively. More than three-fourths (76.25%) of them had moderate perceived severity to the problems associated with contraceptive use compared to only 23.75% of them who had high level of perception. As much as 84.37% of them highly perceived the benefits of contraception uptake. Meanwhile, almost all (93.44%) of them moderately perceived barriers associated with contraception utilization. Conclusion: Contraceptive users had high perception related to susceptibility of the threat of pregnancy and benefits of contraceptive methods utilization. Moreover, the vast majority of them had moderate perception to severity and barriers associated with contraception uptake. Last but not least, there was a statistically significant correlation between women's health beliefs and their utilization of contraceptive method.

Assessment of nurses’ knowledge and practice regarding the use of transillumination device for establishing venous access 评估护士使用透照装置建立静脉通路的知识和实践

Background: Peripheral venous access is frequently performed in various health care settings. However, babies’ veins are smaller and poorly supported by surrounding soft tissue casing further difficulty of insertion compared to adult patients’vein. Transillumination device is recommended for difficult insertion of peripheral veins. Nursing practice should accommodate with evolving health care advancement, and therefore the nurses’ are required to develop their knowledge and attitude towards new skill and practice.Objectives: To assess the level of nurses’ knowledge and practice regarding the use of transillumination device for establishing venous access.Methods: A descriptive study design that was conducted among a sample of 20 Nurses working at Hail General Hospital, KSA. The study tool included a questionnaire sheet of 2 parts as demographic variables and then knowledge and practice regarding use of transillumination device for establishing venous access. Selected nurses were asked to fill up the questionnaire sheet.Results: The majority 50% of the nurses were under the age group of 31yrs and above. Most of the 95% nurses were bachelor’s degree and only 5% was master’s degree. The year of experience were majority of nurses 35% had 7-9 yrs. Designation of nurses indicates, almost 50% of the nurses were working as a Charge Nurse, among 45% as staff Nurse and only 5% as Head Nurse.Obviously, 100% of the nurses perform cannulation in first attempt. Before, 65% of the nurses had no awareness of using veinlite. 100% of the nurses used 26G size cannula.Furtherly, 85% assures that they could incorporate transillumination device into practice. The opinion of 65% nurses feel, this device was easy to use.Conclusion: The study says that, most of the 65% NICU nurses has only average and 35% has good knowledge and practice regarding use of transillumination device for establishing venous access. The nurses have interest in practicing transillumination device and seems to be time saving and effective. Suitable interventional packages need to be given periodically for the effectiveness of qualitative nursing service.

AbstractIntroduction: Preoperative fasting is vital in preventing pulmonary aspiration, regurgitation and vomiting during the induction of anesthesia. Every patient scheduled for surgery including pregnant mothers need to be on fasting for certain period of time. Women undergoing cesarean section need full energy for themselves and the fetus to overcome the stress of birth. However, fasting time in Eritrea is generally longer than is recommended by American Society of Anesthesiologists. Prolonged fasting time has undesirable effects including, nausea, vomiting, dizziness, thirst, hunger etc. Objectives: This study compared the associated discomforts between traditional versus instructing standard guidelines of fasting times among mothers undergoing elective cesarean section. Methods: A non-randomized clinical trials among 106 mothers was conducted. Two groups for comparisons in two different hospitals were selected. One group received instructions of preoperative fasting according to the American Society of Anesthesiologists Task Force guidelines and the other group followed through the traditional approaches, i.e. “nothing by mouth after mid-night”. Results: Mild to moderate degrees of discomforts was seen in the study participants. Feelings of thirst and hunger were the serious discomforts experienced. The associated discomforts were not determined to be significantly different (p=0.256) among the two groups of comparison. Conclusion: The study concluded that the associated discomforts were not statistically different between the traditional and standard groups. There was mild to moderate degrees of discomforts in both groups of study. Though the finding shows similar magnitude of discomforts between the two comparison groups, postoperative discomforts appear to worsen as the timing of fasting increases.

Registered nurses’ attitude and willingness to precept nursing students in Jamaica 牙买加注册护士指导护生的态度和意愿

Background: Registered nurses’ willingness to precept nursing students is an important factor in ensuring a positive clinical learning environment. However, in resource poor settings with high patient to nurse ratio and other challenges in the delivery of nursing care preceptorship may represent an additional challenge.Aim: To determine the willingness of registered nurses to precept student nurses in Jamaica; their attitude towards the paired preceptorship model and factors which may influence the precepting process.Design: This descriptive cross-sectional study utilized a 32-item self-administered questionnaire to collect data from 76 full-time registered nurses assigned to medical/surgical wards at a major teaching hospital.Results: Nurses had a positive attitude towards precepting (97%) and believed their actions and attitudes positively influenced the learning outcomes of students. Two-thirds of registered nurses (63%) were willing to precept students and 87% preferred the paired preceptorship model. Self-efficacy (p<0.05) best explained their willingness to participate in both paired preceptorship and clinical teaching associate models while, normative beliefs regarding clinical learning environment was significant predictor of overall willingness.Conclusion: Registered nurses studied displayed positive attitude towards preceptorship and were willing to precept students. Policy makers should note that the paired preceptorship model was favoured by the group and that personal satisfaction and professional development were motivating factors for nurses’ willingness to precept students. Improving falls in nursing homes: a post-fall huddle quality improvement project 改善养老院的跌倒情况:跌倒后抱团质量改善项目

Falls are one of the most common preventable health problems in adults 65 years and older (AHRQ, 2013). A fall in this population can have a devastating effect often leading to a significant change in morbidity or death. Adults in assisting living, nursing homes, and skilled facilities (SNF) have an increased risk of falling and having a subsequent fall due to an acute illness, weakness, or confusion. This makes individualizing a plan of care to prevent a secondary fall and identifying the root cause of falls within a facility imperative.In our agency, the fall rate is nearly triple that of the national benchmark. To address this problem, a Post-Fall Huddle project was implemented. The literature recommends and supports the practice of a post-fall assessment program in fall reduction to identify intrinsic and extrinsic fall risk etiologies. There was found to be a reduction in the absolute values of recurrent patient falls per quarterly reporting after the implementation of the post-fall huddle. The results also provided pertinent data that can be used for recommendations in future fall prevention for the SNF

Effect of Exercise Program on Fatigue and Depression among Geriatric Patients Undergoing Hemodialysis 运动方案对老年血液透析患者疲劳和抑郁的影响

Background: Fatigue is one of the foremost common complications faced the hemodialysis elderly patients. Fatigue not only impact the daily lives and activities of patients but it conjointly influences their quality of life, which can afterwards result in magnified depression, tiny social interactions and increase dependency on others.Aim: Determine the effect of exercise program on fatigue and depression among geriatric patients undergoing hemodialysis.Design: Randomized controlled trial (pretest post-test) design was used.Setting: The study was carried out in the dialysis units of Mansoura University Hospital and New Mansoura General Hospital.Subjects: A purposive sample of 62 geriatric patients undergoing hemodialysis was selected and randomly allocated into two equal groups, study group (n = 31) and control group (n =31).Tools: Three tools were used; Structured interview questionnaire sheet, Multidimensional Fatigue Inventory Scale and Geriatric Depression Scale Short Form.Results: The mean age of the study and control groups was 63.45±4.49 and 64.7±5.25 respectively. A high statistically significant change in fatigue and depression score (pre and post 2 months) between the study and the control groups was observed (P=0.000). Moreover, a statistically significant relation was observed between fatigue and age of the hemodialysis geriatric patients after implementation of the exercise program (P= 0.002).Conclusion: Implementation of the exercise program proved to be effective in improving the level of fatigue and depression among geriatric patients undergoing hemodialysis in the study group.Recommendation: Encourage geriatric patients undergoing hemodialysis to perform regularly range of motion and relaxation exercises in order to alleviate fatigue and overcome depression.

Background: Contact lens (CL) are widely used in Saudi Arabia, and interest in the lens among the population is increasing rapidly especially among the younger population. CL has become more important as an optical correction method. Many people compromise their visual health due to bad habits for wearing a contact lens.Aim: The present study aimed to assess the pattern of CL uses among university female students.Methods: Descriptive cross-sectional study design was used. 225 students were agreed to participate in the study from four colleges. A structured questionnaire was developed to collect necessary information regarding; socio-demographic characteristics, history of using CL and eye complains.Results: The findings revealed that the majority 74.7% of university students using lens mostly for cosmetic purpose and 45.2% wear CL from 5-12/day.Conclusions: Based on the results of the present study, health education programs incorrect and careful practice regarding CL wear should be directed to the university students to prevent complications resulting from the wearer's inappropriate behavior.

Quality of cesarean section nursing care and its reflection up-on women's satisfaction with labor experience 剖宫产护理质量及其对产妇分娩体验满意度的影响

 Women satisfaction is a crucial predictor for maintaining and monitoring the quality of health care and can inform service development and delivery.Aim: To evaluate the quality of cesarean nursing care at hospital of Mansura University, the outcomes of it preserve ad-vanceandsupportqualityof care and womensatisfaction. By using a descriptive design the study conducted at obstetrics and gynecology department in Mansoura university hospital. On a total of 200women had undergoingelective cesarean section operation. Data collection by, structured interviewed questionnaire, observationchecklist and satisfactionassessment scale. Results; Findings indicated that most of studied women had received a complete care during intraoperative & immediate post-operative 92.5% & 95.0% respectively. Meanwhile, 80.0% & 62.5% of them didn't received health education and emotional support also , the higher percentage of studied women were satisfied with general environment, cleanliness, communication and physical care 75.0%, 75.0%, 65.0% and 58.0% respectively. While, 60.0% of them were dissatisfied with continuity of care. Regarding involving in decision making, all of studied sample100% were dissatisfied. Finally there was a positive association of quality of CS nursing care with the level of women's satisfaction with (p=<0.05) .Conclusion: The current study indicated that, there are several factors that affected on women's satisfaction. higher percentages of them were satisfied with general environment, cleanliness, communication and physical care and dissatisfied with psychological care, continuity of care and involvement in decision making. There was a highly statistically associationof women’s satisfaction with CS nursing care offered. Recommendation: Woman satisfaction is an imperative health care outcome its assessment is recommended tobe a part of hospital quality ofcare monitoring and improvement programs.

By using a descriptive design the study conducted at obstetrics and gynecology department in Mansoura university hospital. On a total of 200women had undergoingelective cesarean section operation. Data collection by, structured interviewed questionnaire, observationchecklist and satisfactionassessment scale. Results; Findings indicated that most of studied women had received a complete care during intraoperative & immediate post-operative 92.5% & 95.0% respectively. Meanwhile, 80.0% & 62.5% of them didn't received health education and emotional support also , the higher percentage of studied women were satisfied with general environment, cleanliness, communication and physical care 75.0%, 75.0%, 65.0% and 58.0% respectively. While, 60.0% of them were dissatisfied with continuity of care. Regarding involving in decision making, all of studied sample100% were dissatisfied. Finally there was a positive association of quality of CS nursing care with the level of women's satisfaction with (p=<0.05) .Conclusion: The current study indicated that, there are several factors that affected on women's satisfaction. higher percentages of them were satisfied with general environment, cleanliness, communication and physical care and dissatisfied with psychological care, continuity of care and involvement in decision making. There was a highly statistically associationof women’s satisfaction with CS nursing care offered.
