Effect of educational program on improving knowledge and practice for adolescences with type 1 diabetes 教育项目对提高青少年1型糖尿病患者知识和实践的影响

Background: Diabetes mellitus (DM) is one of the most common endocrine and metabolic conditions in childhood. Objectives: to determine the effect of educational program on the adolescences children knowledge and reported practices with type 1 diabetes. Method: Design pretest -posttest design was utilized to conduct this study. Setting: study was conducted at outpatient clinic for diabetes at Minia University Hospital for Obstetrics and Pediatrics. Sample: 50 adolescences children with type 1 diabetes. Tools: Pre-designed questionnaire sheet and Educational and training program. Results: this study showed significant improvement in adolescences knowledge and reported practices in post/test I and II. Conclusion: after the implementation of the program, there was remarkable improvement of adolescences children knowledge and reported practices. Recommendations: a developed program should be applied and repeat again every 6 months in the same study setting and adopted in other similar settings with required modifications, provision of continuing education programs.

A low-fidelity game-based approach to teaching basic health assessment: It’s not just a game! 基于低保真游戏的基本健康评估教学方法:这不仅仅是一款游戏!

Game-based teaching-learning activities positively influenced student learning in academic programs, particularly in nursing education where learning nursing concepts, disease processes, assessment skills and management may be overwhelming in the traditional classroom setting. The purpose of this paper was to describe a teaching—learning approach, and to discuss lessons learned to use games to teach basic health assessment in first-level BSN program. The approach and organization constructs were interventional, descriptive and non-experimental. In conclusion, lessons learned yield that teaching health assessment content as a game may promote student and faculty involvement, excitement, and motivation. A game checklist was developed based on feedback from students and faculty. The game led to the development of a guide for game-based approaches to teaching pre-clinical undergraduate nursing students health assessment content.

The development of a guide for nurse leaders in higher education 为高等教育中的护士领导制定指南

Aim: Little is known regarding mentoring and supportive resources to assist, retain, and support nurse administrators at Historically Black Colleges and Universities (HBCUs). The purpose of this paper is to discuss and describe the analysis of nurse leaders expressed journey to leadership, and a provide a guide for nursing leadership development. Methods: Thematic analysis of nurse leaders expressed challenges and terms and concepts of nursing leadership that prepared them for their role as a successful nurse leader. Results: Themes were derived from the analysis of past distinguished nurse leaders and provided a foundation for the development of mentoring guide for African American nurse leaders. Conclusions: Mentoring and supportive resources are essential in retaining nurse leaders in higher education. A guide on how to retain and support nurse administrators at HBCUs. This guide serves as a foundation for colleges and universities in their recruitment, orientation, and retention of nurse administrators in higher education.

Implementation of a functional supportive programme for survivors of cerebro-vascular accidents and their caregivers at home 为脑血管事故幸存者及其家中照顾者实施功能性支持规划

A cerebro-vascular accident is a devastating experience and those who survive the initial incident often suffer from a variety of physical disabilities. These disabilities frequently result in an adult becoming incapable of, or being limited in, the provision of ongoing effective self-care. In anticipation of improving such situations the researcher developed a supportive-educative programme to be conducted in a home-based setting. This programme formed part of the main study and was implemented over four days. The researcher considered these educative sessions to be an ideal approach for implementing the programme, because adjusting, stabilising and coping can only be facilitated and put into practice through interactive participation. Eight survivors of cerebro-vascular accidents and their caregivers, all of whom had participated in the main study, were selected for the training sessions. During these sessions, the purpose, objectives and content of the programme were presented and discussed. Following the sessions, the participants were encouraged to apply what they had learnt from the training. After two months the researcher and the participants came together to conduct an evaluation of the programme.

The risk factors and outcome of early identified acute kidney injury among critically ill obstetric patients 危重产科患者早期发现急性肾损伤的危险因素和结局

Background: Acute kidney injury (AKI) is one of the most challenging and serious complications of pregnancy and postpartum period that facing critical care nurses in Intensive Care Unit (ICU). Having a uniform standard for identifying and classifying AKI would enhance critical care nurses’ ability to recognize these patients and leading to better outcomes. Objective : This work aimed to explore the risk factors and outcome of early identified acute kidney injury of critically obstetric patients in Obstetric ICU. Design. A descriptive cross sectional research design was used in this study. Participants: A total sample of 338 women admitted to Obstetric ICU at Woman Health Hospital, Assiut City, Egypt. Method: Three tools were used.Tool I was developed by the researcher and included demographic and obstetric history, lab parameters, complications and outcomes arising from AKI. The Sequential Organ Failure Assessment (SOFA) score as tool II to determine the extent of a patient's organ function or rate of failure. Measurement of serum creatinine and urine output were used to early identify AKI stages according to Acute Kidney Injury Network (AKIN) Criteria (tool III). Results: The prevalence of AKI among obstetric patients admitted to obstetric ICU was 10.1%; of them 52.9% needed renal replacement therapy and the mortality rate was 29.4%. Postpartum hemorrhage was the most common cause of AKI and its prevalence was 41.2%. It was also found that 74.5% of AKI patients developed complications. Conclusion: AKI complicated 10.1% of total admitted women to the OICU in the studied period. Postpartum hemorrhage represents the most prevalent risk factors with a highly significant SOFA score compared to other risk factors as sever preeclampsia, eclampsia, HEELP & APH with acute fatty liver.

Perceived stress among nurses of psychiatric ward patients with and without suicidal behavior 精神科病房护士对有或无自杀行为病人的压力感知

Background and Objectives : The present research was conducted to obtain a perceived stress pattern among nurses attending the psychiatric patients with suicidal and non-suicidal behavior in the different hospitals and clinics of the twin- cities (Islamabad and Rawalpindi) of Pakistan. The main objective of the study was to have an idea about the environment of nursing in psychiatric wards in Pakistani culture. Participants and Method : The sample (N= 36) comprised of the nurses of psychiatric ward attending the one patient for 12 hours (n=16) with and (n=20) without suicidal behavior working into night and day shifts respectively. All the participants gave responses on the perceived stress scale. Participants were approached during their duty hours and were given perceived stress scale 10- items along with demographic information sheet and the consent form. Between group-design was used in the present study. Results and Conclusion : The results indicated that verification of both hypotheses, i.e. the nurses of patients with suicidal behavior had more perceived stress (M= 42 ±3.5) than the nurses of patients without suicidal behavior (M= 19.11 ± 5.7) and also the old nurses had a greater level of perceived stress than the younger nurses according to the same statistics. The t- test, t (28.5) = 14.9 p< 0.05 was significant on 95% confidence interval with variance not equal. Limitations and suggestions for future were also discussed. Both the hypotheses were supported by the analysis- results and by the previous literature.

The present study aimed to assess quality of life of elderly asthmatics and to determine risk factors that may influence it. The study was carried out in the chest out-patient clinics and chest departments at Assiut University Hospital. The sample type of this study total coverage during one year consists of one hundred and five elderly people aged 60 years and above suffering from bronchial asthma disease. Two tools were used for data collection, tool one divided into three parts, socio-demographic data risk factors assessment, and history of bronchial asthma. Tool two to assessment health related quality of life of asthmatic patients was using St. George's Respiratory Questionnaire (SGRQ). The main results yielded by the study proved that, there were highly significant differences between risk factors of bronchial asthma disease and health-related quality of life. The study recommended that further research be implemented to improve the effect of health related quality of life for asthmatic patients.

Emotional, behavioral and social difficulties among children with type 1 diabetes mellitus 1型糖尿病儿童的情绪、行为和社会困难

Background: Children and adolescents with diabetes are at a greater risk for emotional and behavioral problems . Aim of the study was to assess behavioral, emotional and social difficulties of diabetic children and examine the association of these difficulties with demographic and disease related variables. Subjects and method: A cross sectional study of 423 diabetic children attended Sidy Galal health insurance outpatient clinic in Assuit city – Egypt was conducted. A similar number of healthy children matched for age, sex and socioeconomic status were included in the study as a control group used for comparison. Strength and Difficulty Questionnaire (SDQ) was used as a screening tool. The SDQ is a screening instrument consisting of five separate 5-item subscales for emotional symptoms, conduct problems, hyperactivity-inattention, peer problems, and prosocial behavior. Results: Most of diabetic children (92.9%) were in the abnormal group of the total score compared to 20.6% of the controls. In all SDQ subscales except prosocial, diabetic children had significantly higher percentages of abnormal indices than in controls (p value <0.001) . Emotional, hyperactivity and conduct problems were significantly associated with age and sex of the child. Prolonged duration of illness was significantly associated with peer relationship problems . Glycemic control was not a significant risk factor for any problem . Conclusion: Many of diabetic children had behavioral and psychosocial problems. Age and sex of the child and duration of the disease were important determinants for these problems. Recommendation: Diabetic children need to be screened by psychologist and social worker to identify and manage any behavioral or emotional problems.

Plasma levels of irisin in children with idiopathic premature adrenarche 特发性肾上腺早衰患儿血浆中鸢尾素水平的变化

Background: Premature adrenarche (PA) is characterized by presence of isolated pubic and/or axillary hair, acne and body odor before age 8(9) in girls (boys). These individuals are at increased risk of developing insulin resistance, metabolic syndrome and polycistic ovarian syndrome. Irisin seems to have an active role in the metabolism of carbohydrates and lipids, however little is known about this hormone in PA. Objective: To analyze irisin levels in children diagnosed with PA and its relationship with their body composition. Methodology: Exploratory cross-sectional study that evaluated 15 children with PA and 15 matched controls (C). Anthropometric data were measured: height, weight, waist circumference (WC) and triceps skinfold. Fasting blood glucose(G), insulin(I), 17OHP, total cholesterol, LDL, HDL, triglycerides, DHEA-S, 25(OH)D and irisin levels were determined. Results: The levels (mean±SEM) of triglycerides [99±14, 8mg/dl (C); 68±9, 1mg/dl (AP)] and 25(OH) D [26±0, 9ng/ml(C); 30.2±1.6ng/ml (AP)] were significantly different between the groups. WC above p90 and G/ I<7 were found in 6.7% of the C group versus 33.3% and 20% of the PA group, respectively. Conclusions: PA children presented a lower G/I and a higher waist WC compared to the C group, suggesting an increased risk of metabolic disease. Irisin levels were not different between the groups. The significantly higher levels of TGs from the C group may be related to their reduced levels of 25(OH) D, which may also have masked differences in irisin levels. This study suggests that determination of vitamin D levels may be necessary to evaluate metabolic data, based on the significant frequency of this hormone insufficiency/deficiency in the pediatric population.

Development of an educational programme to facilitate critical thinking of student nurses in Namibia 制定教育方案,促进纳米比亚护士学生的批判性思维

The article focused on the development process of an educational programme to facilitate the development of critical thinking in the students. The study was conducted in four phases, beginning with the needs assessment in phase 1 through which the researcher determine the need of the students in terms of critical thinking. The deductive data analysis of phase 1 served as basis of the conceptual framework for the development of an educational programme. Phase 2 focused on the development of the conceptual framework and it was achieved by utilized a specific educational approaches and philosophical framework were employed. Phase 3 and 4 were conducted simultaneously. Phase 3 focuses on implementation while phase 4 focuses on evaluation. A quasi – experimental design was implemented which focused on active participation by the participants in the programme.
