DESIGN OF AN ARTICULATING NON-INVASIVE JOINT DISTRACTOR FOR METACARPOPHALANGEAL JOINT OF THE THUMB 用于拇指掌指关节的无创关节牵张器的设计

There is no specific distraction device available for the metacarpophalangeal joint (MCPJ) of the thumb. Joint distraction is required to facilitate the implantation of MCPJ spacers. In addition, expanding the joint space is essential for MCPJ arthroscopy. For these reasons, a novel device has been designed to create some space in the MCPJ. This articulating joint distractor uses a finger trap to apply the distraction force to the thumb and a strap to fixate the hand. There is the possibility to flex the thumb while maintaining the distraction force. In a first cadaveric experiment, we have determined that the force required to create 2 mm of distraction is approximately 30N. A prototype of the articulating joint distractor has been built and was tested during a second cadaveric study. As a result of the second cadaveric experiment, the desired functions of the prototype have been validated. By adjusting and optimizing the design to minimize the risks found partly during the experiment, the final design of the non-invasive MCPJ distractor has been realized.

STETHAID: AN ELECTRONIC STETHOSCOPE CONNECTED TO IOS MOBILE APPS FOR AI-ASSISTED AUSCULTATION Stethaid:连接ios移动应用程序的电子听诊器,用于人工智能辅助听诊

Auscultation is a critical component of a cardiopulmonary examination; however, several studies have shown that many physicians lack proficiency in it. With the development of artificial intelligence (AI)-augmented auscultation tools, physicians could improve their auscultation skills and provide accurate diagnoses, even approaching the expertise of seasoned clinicians. Although a few AI auscultation platforms have been created, none have been adopted widely in clinical settings. Our goal is to develop a comprehensive digital auscultation platform, termed StethAid, to supplement the value of auscultation. StethAid is a comprehensive auscultation platform that comprises of an electronic stethoscope, mobile applications, and website portals. The StethAid stethoscope enables streaming and recording of heart and lungs sounds. It features 100 levels of sound amplification, digital filtering, active noise cancellation, and wireless connectivity. The StethAid stethoscope is similar to FDA-approved stethoscopes in its frequency response. Our mobile apps deliver various auscultation tools, as well as an AI suite and remote auscultation capabilities. The AI suite includes deep learning-based automated wheeze detection from auscultated lungs sounds. StethAid has been rigorously validated technically and clinically. StethAid could assist physicians in making more informed decisions, potentially leading to improved patient outcomes.

BENCH TESTING OF A TUNABLE ANKLE-FOOT ORTHOSIS WITH ADJUSTABLE STIFFNESS AND NEUTRAL ANGLE 具有可调刚度和中性角度的可调踝足矫形器的台架试验

Cerebral palsy is the most common childhood motor disability that effects 0.3% of children in the US and two-thirds of them have gait impairment. An ankle-foot orthosis (AFO) is commonly prescribed as a gait assistance. The function of an AFO is maximized when its stiffness and neutral angle are optimally prescribed to address each user’s issue. However, the prescription process is not standardized and is subjective to the prescribing clinician. Only one-third of AFOs improved the user’s gait while another one-third negatively affected the gait. A tunable AFO is developed to allow clinicians to test a range of stiffness and neutral angle of a passive AFO, making the prescription more likely to have a positive effect. The design of tunable AFO has a weight of about a typical plastic AFO and a dimension designed for children. It covers 90% range of stiffness and neutral angle of prescribed AFOs. It can withstand the torque applied during walking. Mechanisms of adjusting both parameters operate fast and are controlled by a microprocessor. A benchtop prototype was built to test the adjustment mechanisms. Both mechanisms were validated.

An interprofessional team embarked on the design and prototyping of an improved powered air-purifying respirator for healthcare. They used 3D modeling to move their ideas from a list of key expectations to a virtual prototype. The design thinking concept of rapid prototyping and empathy interviewing ensured this design met the needs of the stakeholders who would use the product. Several significant changes in the design resulted from the rapid prototyping interviews. This process was enhanced by using a virtual reality software to allow the team to “see” the product and comment on the design integrating both VR headsets and videoconferencing for feedback and collaboration. The design project is now ready for physical prototyping and further human factors analysis.

DESIGN OF A WEARABLE ULTRASOUND PATCH WITH SOFT AND CONFORMAL MATCHING LAYER 一种具有软适形匹配层的可穿戴超声贴片的设计

Ultrasound is a safe, noninvasive diagnostic technique used to measure internal structures such as blood vessels and the velocity of blood flow in the human body. The ability to continuously measure blood flow in major cerebral arteries would enable the early detection of medical problems such as stroke. However, current ultrasound technology consists of rigid, hand-held probes that are arduous to use, sensitive to movement, and are primarily designed for intermittent, instead of continuous use. Here, we describe the design of a wearable ultrasound patch for continuously measuring blood flow velocity through the middle cerebral artery (MCA) that can be assessed from the temple region of the head. The wearable ultrasound patch is composed of an array of piezoelectric elements that are wired together using flexible electrical conductors and encapsulated in an elastic substrate. To improve ultrasound energy transfer, a soft and conformal composite matching layer is introduced. The matching layer consists of gallium-based liquid metal (LM) microdroplets dispersed in a silicone elastomer. The acoustic impedance of the matching layer can be tuned by varying the volume loading of LM. The wearable ultrasound patch will provide new opportunities to continuously measure blood flow velocity and ultimately enable early detection of medical problems such as stroke.

A RECONFIGURABLE, ADDITIVELY MANUFACTURED VIBROTACTILE STIMULATION DEVICE FOR CHRONIC PAIN 一种用于慢性疼痛的可重构、增材制造的振动触觉刺激装置

This paper presents an experimental chronic pain rehabilitation therapy utilizing vibration-based stimulation of the hand, wrist and lower arm, which has shown promise in reducing pain ratings in chronic pain patients (n=5) over a six-week treatment period. Based on the clinical need and practical use discoveries of that first trial, this paper outlines the development of a second-generation device that activates specific nerves through vibrotactile stimulation applied to the skin. Several major steps in this process were the iterative design process, physical prototyping, conceptual modeling, and benchtop testing. Furthermore, this next generation device was tested with epilepsy patients undergoing invasive monitoring (stereotactic electroencephalography, sEEG) to collect electrophysiological measurements via implanted electrodes in the brain. Early findings showed differential modulation of delta power in response to 2 Hz median nerve stimulation, but not during 12 Hz median nerve stimulation or 2 Hz ulnar nerve stimulation, hence indicating differential encoding of vibration features depending on frequency and spatial parameters. These patient trials also served to collect feedback for the continued development of the device to improve aspects such as ease of use, patient comfort, and ideal application of the vibration stimulus. Testing and development are ongoing with the enhancement of vibration isolating structures, more comfortable contact points, more durable additively manufactured structures, and embedded force sensors in the device.

Based on the clinical need and practical use discoveries of that first trial, this paper outlines the development of a second-generation device that activates specific nerves through vibrotactile stimulation applied to the skin. Several major steps in this process were the iterative design process, physical prototyping, conceptual modeling, and benchtop testing. Furthermore, this next generation device was tested with epilepsy patients undergoing invasive monitoring (stereotactic electroencephalography, sEEG) to collect electrophysiological measurements via implanted electrodes in the brain. Early findings showed differential modulation of delta power in response to 2 Hz median nerve stimulation, but not during 12 Hz median nerve stimulation or 2 Hz ulnar nerve stimulation, hence indicating differential encoding of vibration features depending on frequency and spatial parameters. These patient trials also served to collect feedback for the continued development of the device to improve aspects such as ease of use, patient comfort, and ideal application of the vibration stimulus. DEVELOPMENT AND TESTING OF A MULTIFUNCTION GASTRIC FEEDING TUBE CAPABLE OF VITAL SIGN MONITORING 一种可监测生命体征的多功能胃饲管的研制与试验

Infants born extremely prematurely present significant clinical and population health challenges. Advances in clinical monitoring can potentially drive improvements in survival and long-term outcomes in this vulnerable population. In this study, a multi-function catheter with ECG, pressure, and temperature sensors is developed and the performance and ideal lead location are tested in a rat model. The ECG signals, transdiaphragmatic pressure, and core body temperature of the rat were recorded from a modified multi-electrode gastric feeding tube with one pressure sensor at the tip and another pressure sensor and temperature sensor at 6 cm from the trip. The ECG signals were obtained from esophageal electrodes in multiple locations and eventually, optimal electrode locations were identified at 4 and 6 cm from the tip of the feeding tube. Reliable pressure signals at a pressure range of 0-0.2psi (0-14 cm H2O) were obtained from pressure sensors placed above and below the diaphragm. A core temperature of ~41°C was recorded from the temperature sensor which was elevated relative to the rectal temperature measurements due to the experimental setup used. Our multifunction catheter proved to provide a reliable, strong, high resolution and low noise ECG signal from the esophageal electrodes in an animal model equivalent in size to the preterm infant. In addition, continuous pressure and temperature recordings can simultaneously be extracted, with all 3 sensors contained within a less than 3 mm diameter tube as is routinely used in this population. Integration of these multiple components into a feeding tube, which is already universally used in this population purely for nutrition, will provide significant advances in vital sign monitoring while reducing risk to vulnerable preterm infants.

DESIGN OF BICORPORAL PUMP FOR THE TREATMENT OF HYDROCEPHALUS 治疗脑积水的双腔泵的设计

With the current design for shunt systems used to treat hydrocephalus, tissue from the brain ventricle can cause blockages that obstruct cerebrospinal fluid (CSF) flow through the proximal catheter. Previous work demonstrated that the reversing of flow through the proximal catheter can restore flow through a blocked catheter. Periodic backwash through the proximal catheter can prevent obstructions and maintain adequate drainage. This project aims to design a pump that can periodically reverse the flow of CSF to backwash the proximal catheter. To reduce the complexity of the implanted components, a simple diaphragm pump can be externally actuated. Actuation of the pump can be achieved using an electromagnet located outside of the body; a metal cap placed on the diaphragm can be magnetically manipulated to pump CSF and backwash the proximal catheter. Testing demonstrates the feasibility of this pump design with an achieved flow rate of 3.4 to 4 mL/min. This bicorporal pump used as part of a backwashing system may favorably change the shunt failure rate and provide a more robust treatment for hydrocephalus.
