During the COVID-19 pandemic antibiotic use considerably increased being partially justified by the fear of bacterial infection. Antibacterial resistance (ABR), due to increased and unjustified antibiotic use, is a major threat to the economy and global health. In pediatric practice, antibiotics are the most common prescribed substances, both in the community and hospital setting. Unjustified use, inappropriate doses and prescription duration promote antibacterial resistance and increase mortality. The majority of current health problems in children are viral infections, even the rates of infection are higher as in adults, fact that determines more frequent diagnostic uncertainties. More so, respiratory infections in children have a greater potential of excessive and incorrect use of antibiotics, that justifies protocol based evaluations, risk assessment and targeted treatment. The diversity and magnitude of clinical presentation of SARS-CoV-2 infection, along with early and long-term complications and sequelae, also noticed in pediatric practice, created the premises of increased use of antibiotics, aggravating the ABR. This is a challenge for the clinician during a period when development of new antibiotics is not a priority of the pharma industry anymore. Research of mechanisms that contribute to ABR, innovative therapies, expansion of genetics and implementation of antibiotic stewardship, together with stimulating pharma industry for developing of new substances, may have the potential of decreasing antibacterial resistance in an era of medical and economic uncertainties generated by the COVID-19 pandemic.

Diabetes during pregnancy is more and more frequent worldwide because of the rising incidence of its known risk factors, such as high body mass index, sedentary lifestyle, and hypercaloric diets. It can be either a preexisting condition, or it can be first diagnosed during pregnancy, usually between the 24th and the 28th week of gestation, when it receives the name of gestational diabetes mellitus. Both preexisting diabetes mellitus and gestational diabetes mellitus may have severe maternal and newborn consequences, especially if there is insufficient control of the maternal glycemic levels and progression of diabetes complications, such as microvascular disease and nephropathy. Renal malformations represent a large part of the pathology referred to the pediatric surgeon. Most of them are diagnosed during pregnancy. We present one case of complete ureteral duplex with upper pole obstructive megaureter managed in a private hospital using minimally invasive methods for both diagnosis and treatment due to good teamwork. We do not want to demonstrate that other investigations are not leading to the same diagnose, we want to highlight an alternative to the classic approach. We hope that our experience is a starting point for further research and that in future radiation and general anesthesia in children will be avoided as much as possible for renal investigations.

Gastroschisis is a congenital anterior abdominal wall defect that occurs in 4 to 5 in 10,000 births, consisting in the evisceration of bowel in the amniotic cavity without a membranous covering. Gastroschisis cases can be detected during the first trimester using ultrasonography or at the beginning of the second trimester with a detection rate of 100%. Although an isolated condition in most of the cases, further complications such as malrotation, atresia, stenosis, perforation or volvulus can be associated. Periodic monitoring is required to assess the progress of the pregnancy with measurements of the level of amniotic fluid, fetal biometry and bowel diameters at every ultrasound examination. Gastroschisis is most frequently associated with oligohydramnios, intrauterine fetal growth restriction, increased bowel wall thickness and bowel dilatation. The outcome of the disease is favorable with a survival rate of around 90% for born infants. The long time on ventilator and multiple intestinal surgeries that delay the moment of oral feeding with a long-term use of total parental nutrition and short bowel syndrome are the most important comorbidities. Delivery is recommended at 38 weeks of gestation, preserving caesarean delivery for complicated gastroschisis. Although a complex pathology, gastroschisis is associated with normal intellectual development and few to none long-term complications.

Background. The incidence of pediatric differentiated thyroid carcinoma (DTC) is increasing even though that this is a rare disease. It is known that there are important differences between adults and children especially in clinical presentation and genetic alterations. The only guideline for the diagnostic and therapeutic approach for the children with thyroid nodules and differentiated thyroid carcinoma is the American Thyroid Association (ATA) guideline published in 2015, but there could be improvements that we need to highlight in order to reduce the apparition of comorbidities and to improve the quality of life. There is also no national guideline in Romania for the treatment of this pathology in children. Materials and methods. This study is a review of the literature published in the last 5 years concerning the diagnostic and treatment of the pediatric thyroid cancer. Results. The management of the children with the suspicion of thyroid nodule should be done in specialized centers in a specialized, multidisciplinary team, with high volume surgeons. The diagnostic approach should include genetic testing. The surgical management could be less radical in certain cases. The pre-surgical evaluation and intraoperative laryngeal nerve monitoring should reduce the apparition of comorbidities. Conclusion. A European and also a national guideline for the management of pediatric thyroid nodules and thyroid carcinoma is mandatory to reduce the apparition of comorbidities and to improve the quality of life of the patients.

The clinical case presented a challenge in clarifying the final diagnosis – high-risk embryonal rhabdomyosarcoma.

Goldenhar syndrome, also known as oculo-auriculo-vertebral syndrome, represents a rare congenital complex of anomalies, involving the development of the first and second brachial arches during the first embryonary stage. In addition to the craniofacial disorders, Goldenhar syndrome can include central nervous system, heart, skeleton or kidneys pathologies as well as vertebral defects. We present the case of a Goldenhar syndrome whose first pathologic sign was tetralogy of Fallot, with associated cardiovascular disorders (atrial septal defect type ostium secundum, and single umbilical artery) and bilateral microtia, with malformation of auditory canal, mandibular hypoplasia and a single epibulbar dermoid. The Goldenhar syndrome was diagnosed after birth, the article emphasizing the difficulties of prenatal diagnosis.

Background. Acute pediatric poisonings represent an emergency pathology that associates a significant risk of adverse outcome or death. The increased incidence reported by antitoxic centers is a public health problem and requires implementing prevention strategies. Material and method. The six-years study targets children diagnosed with acute poisoning complicated by cardiogenic shock in a pediatric toxicology unit, performing a descriptive analysis of the epidemiological characteristics of the group. Aim. To provide statistical data that can represent the basis of preventive measures carried out by health services. Results. The incidence of severe intentional poisonings complicated by cardiogenic shock is almost twice as high as accidental ones. A strong correlation was identified between intentional poisonings and adolescent girls. Conclusions. The detailed epidemiological data allows targeted intervention measures to prevent suicide attempts and decrease the incidence of accidental exposures among the pediatric population.

Acute pediatric poisonings represent an emergency pathology that associates a significant risk of adverse outcome or death. The increased incidence reported by antitoxic centers is a public health problem and requires implementing prevention strategies. Material and method. The six-years study targets children diagnosed with acute poisoning complicated by cardiogenic shock in a pediatric toxicology unit, performing a descriptive analysis of the epidemiological characteristics of the group. Aim. To provide statistical data that can represent the basis of preventive measures carried out by health services. Results. The incidence of severe intentional poisonings complicated by cardiogenic shock is almost twice as high as accidental ones. A strong correlation was identified between intentional poisonings and adolescent girls. Conclusions.
