Does exergaming promote neurofunctional changes in Parkinson´s disease? A pilot clinical study 运动是否促进帕金森病的神经功能改变?初步临床研究

Abstract Introduction Previous studies have demonstrated beneficial effects in people with Parkinson´s disease trained with exergames. However, to the best of our knowledge, none of them evaluated whether these effects are sustained by neurofunctional changes. Objective To evaluate neurofunctional effects of a training, by means of functional magnetic resonance imaging, in people with Parkinson´s disease. Methods This study was a blind, randomized, and controlled pilot clinical trial with crossover design. The participants were submitted to an evaluation including cognitive performance and functional magnetic resonance imaging before and after the WiiTM or control trainings. Trainings were applied for 10 days, in two consecutive weeks. Participants starting with WiiTM training were then moved to the control training and vice versa. A wash-out period of 45 days between the trainings was respected. Results Memory, executive and visuo-spatial functions, and attention were significantly improved compared to baseline (p < 0.05). No differences were observed in cognition compared to the control training. Though not significant, results of functional magnetic resonance imaging analyses suggested that WiiTM training could promote improvements on the brain functional connectivity especially in areas involved in motor execution, planning, visual, memory and somatosensory functions. Conclusion In people with Parkinson´s disease, an intensive WiiTM training improved cognitive performance that underlined neurofunctional changes in areas involved in cognitive processing.

Influence of the selective insufflation technique on cerebral blood flow in preterm infants 选择性充气技术对早产儿脑血流的影响

Abstract Introduction The intervention of respiratory physio-therapy in neonatal units is in continuous development, having its own care characteristics related to the weight and gestational age of the newborn, respecting the immaturity of the organs and systems and the diseases of this patient. Through techniques, the objective is to optimize the respiratory function, assisting in the clearance of secretions, and the restoration of lung volumes. Objective To verify if the respiratory physiotherapy technique of selective insufflation alters the cerebral blood flow in premature infants under 34 weeks of gestational age. Methods This is an uncontrolled clinical trial, conducted in a Neonatal Intensive Care Unit of a level III hospital, between January 2019 and March 2020, with participation of premature newborns under 34 weeks of gestational age. All were submitted to transfontanellar Doppler ultrasonography to assess cerebral blood flow measurements, mainly the resistance index, before and after the application of the selective insufflation respiratory physiotherapy technique. Results Sixty-two newborns were included, with a mean gestational age of 29.3 ± 2.2 weeks and birth weight of 1,259 ± 388 grams. The resistance index did not change significantly (RI before: 0.55 ± 0.07; after: 0.54 ± 0.07; p = 0.06) before and after the intervention and no studied variables such as, gender, gestational age, weight, Apgar score or SNAPPE II score had an influence on cerebral blood flow measurements. Conclusion The selective insufflation technique did not alter cerebral blood flow in premature newborn infants under 34 weeks gestational age.

Effect of walking with blood flow restriction in elderly women with osteoporosis/osteopenia 老年妇女骨质疏松/骨质减少伴血流量限制行走的影响

Abstract Introduction The preservation of bone mass in elderly women is associated with better levels of practice of systematic physical exercises. Aerobic training combined with blood flow restriction seems to be a new alternative that determines this process, but knowledge gaps are still observed when referring to exercise associated with blood flow restriction (BFR) and adaptations on bone variables. Objective To analyze the chronic effects of aerobic training with and without BFR on bone mineral density and bone biomarker osteocalcin concentrations in older women. Methods Thirty women were randomized into the following groups: walking on a treadmill at low intensity with BFR; moderate treadmill walking with no BFR; only BFR (no exercise) for 20 minutes, twice a week, for 24 weeks. Bone mineral density was measured before and 24 weeks after intervention. Blood serum osteocalcin concentrations were measured before, 12 and 24 weeks after intervention. Results There were no differences between groups in bone mineral density (femoral neck, p = 0.31; total femur, p = 0.17; lumbar spin, p = 0.06) and osteocalcine (W(2) = 0.27; p = 0.87) ouctomes after 24 weeks of intervention. Conclusion There was no difference between walking training, blood flow restriction only, or walking+blood flow restriction on bone mineral density and osteocalcin concentrations after 24-weeks of intervention in older women with osteopenia/osteoporosis.

Is there a difference in balance between continent and incontinent women? 大小便失禁和大小便失禁的女性在平衡方面有区别吗?

Abstract Introduction Urinary incontinence is defined as any involuntary loss of urine. An imbalance in the transmission of forces between the bladder and urethra, associated with deficient support of the pelvic floor muscles, contributes to an alteration in balance in women. Objective To compare balance between continent and incontinent women. Methods This was a cross-sectional study with 13 women divided into incontinent (age: 41.50 ± 9.13 years) and continent (age: 35.29 ± 4.99 years) groups. Balance assessments were performed using a force platform and electromyography: standing, with eyes open (BI_OA); standing, with eyes closed (BI_OF); standing on foam, with eyes open (ESP_OA) and closed (ESP_OF); and standing with unipedal support, with eyes open (UNI_OA). Statistical analysis was initiated after resampling of the original data using the bootstrap technique, with the α value set at 5% (p < 0.05). Results In the BI_OA task, no significant differences were found between the groups. In the BI_OF task, incontinent women showed greater displacement in the anteroposterior axis (p < 0.001), and continent women showed greater displacement in the mediolateral axis (p = 0.008). In the ESP_OA task, incontinent women showed greater displacement in both the COP_X (p = 0.003) and COP_Y (p = 0.001) axes; in the ESP_OF task, continent women showed greater displacement in the COP_X (p < 0.001) axis. In the UNI_OA task, greater anteroposterior displacement was observed among incontinent women (p = 0.008). Conclusion Continent women showed greater displacement in the mediolateral axis in the tasks with eyes closed, and incontinent women showed greater displacement in the anteroposterior axis in the BI_OF, ESP_OA, and UNI_OA tasks.

The impact of post-COVID multicomponent rehabilitation covid - 19后多组分康复的影响

Abstract Introduction Post-COVID-19 syndrome is characterized by chronic fatigue and myalgia, among other symptoms, which can limit activities of daily living. Physical therapy protocols with multicomponent exercises combine strength, resistance, balance and gait, producing sig-nificant improvements in functional mobility. Objective Evaluate whether multicomponent rehabilitation is effective in improving functional mobility and quality of life in individuals with post-COVID-19 syndrome. Methods Randomized controlled trial with 59 volunteers, divided into an intervention (IG) and control group (CG), all of whom underwent initial assessment (T0). The IG performed 24 sessions over 12 weeks and the CG did not undergo training. Functional mobility was analyzed using the 6-minute walk test (6MWT) and the Berg balance scale, while the SF-36 questionnaire assessed quality of life. Results In the 6MWT, the IG covered 464.40 + 81.26 meters (T0) and 518.60 + 82.68 meters (T4). The IG Berg scale scores were 48.00 + 4.00 (T0) and 51.90 + 4.26 (T4). In terms of quality of life, the IG obtained mean scores of 96.26 + 10.14 (T0) and 102.60 + 5.53 (T4). None of these measures was statistically significant. Conclusion Individuals who underwent the multicomponent phys-iotherapy protocol showed statistically nonsignificant increases in the variables studied.

Results In the 6MWT, the IG covered 464.40 + 81.26 meters (T0) and 518.60 + 82.68 meters (T4). The IG Berg scale scores were 48.00 + 4.00 (T0) and 51.90 + 4.26 (T4). In terms of quality of life, the IG obtained mean scores of 96.26 + 10.14 (T0) and 102.60 + 5.53 (T4). None of these measures was statistically significant. Abstract Introduction Cervical cancer is caused by a persistent infection with some types of human papillomavirus (HPV), and its treatment entails chemotherapy, radiotherapy and surgery. This may cause different kinds of dysfunction in the pelvic floor. Hence, physiotherapy plays an important role in the evaluation and treatment of urogynecological dysfunctions occasioned by the oncological treatment. Objective To develop a digital technology in application format to help physiotherapists in the assessment of pelvic dysfunctions after cervical cancer. Methods A digital technology in application format was developed after approval from the Ethics in Research Committee of the Tropical Medical Center. The instrument was developed by a physiotherapist with expertise and experience in the area. The researchers held meetings to finalize the creation of the checklist, and the application was programmed using the Android Studio Environment. In the end, the application was evaluated by two physiotherapist experts in the field. Results The application created displays five sections addressing the symptoms of urinary dysfunction, sexual function, anal function, pain and alterations such as: lymphedema, vaginal stenosis and vaginal fibrosis. Conclusion The use of the application may help physiotherapists in the assessment of pelvic dysfunctions after cervical cancer.

Functional performance indicators associated with hypertension in older people 老年人高血压相关功能表现指标

Abstract Introduction During aging, changes occur in the human body that increase the probability of arterial hypertension which can potentiate deleterious effects on functional performance. Objective To analyze the association of functional performance indicators with hypertension in older people. Methods An epidemiological, population-based, cross-sectional survey was conducted with 209 older adults (58.40% women). Functional performance was assessed by the following tests: handgrip strength; chair stand test; arm curl test; timed up and go (TUG); sit and reach test; and step in place test (SPT). Diagnosis of arterial hypertension was self-reported. Results The prevalence of hypertension was found to be 58.90% (males: 51.70%; females: 63.90%). It was also observed that the hypertensive individuals of both sexes had worse performance in SPT and TUG (p < 0.05). Furthermore, it was found that each additional second for TUG increased by 11% the probability of hypertension in men (PR:1.11; 95%CI: 1.03-1.20) and by 7% in women (PR: 1.07; 95%CI: 1.04-1.12), while each step performed in the SPT decreased the probability for the outcome by 2% (men and women = PR: 0.98; 95%CI: 0.97-0.99). Conclusion TUG was found to be positively associated with hypertension, while SPT was inversely associated with hypertension.

Brief Lower Body Functional Performance Questionnaire (Brief-LBFPQ) for independent older adults 针对独立老年人的简易下半身功能表现问卷(Brief- lbfpq)

Abstract Introduction The literature still lacks evidence about patient-reported outcome measures to fast screen the reduced physical performance of the lower body in older adults to be applied in any clinical setting as primary health care or without specific instruments in prevention campaigns, or even easy to be applied by phone. Objective: To develop a brief questionnaire to screen the lower body functional performance in community-dwelling older adults and to validate this new questionnaire with objective clinical tests. Methods A convenience sampling of 221 community-dwelling older adults was included in this cross-sectional study. The validity between Brief-LBFPQ and objective tests such as gait speed, Timed-Up and Go test (TUG), 5-Time Stand-to-Sit test (5TSST), and step test were assessed by multinominal logistic regression. Internal consistency was determined using Cronbach’s alpha and Test-retest reliability was determined using intraclass correlation coefficient (ICC) for numeral scale and Cohen’s Kappa for ordinal scale. Results Brief-LBFPQ was significantly associated with objective tests. All eight items from Brief-LBFPQ presented an absolute agreement with ICCs values above 0.7. Kappa values of Brief-LBFPQ items ranged from 0.6 to 0.83, showing substantial agreement and perfect agreement. Conclusion Brief-LBFPQ could be very useful in general clinic settings as it provides earlier screening of functional impairment in independent older adults, and consequently may allow an earlier intervention approach.

Abstract Introduction: Smartphone use has become a popular social communication phenomenon worldwide. Its excessive use can compromise daily routines and habits, which is associated with sleep disorders, stress, anxiety and pain. Therefore, the university student stands out, as they has a lifestyle in which it is necessary to reconcile daily activities with curriculum activities, aggravating psychosocial factors. Objective: To investigate whether smartphone addiction influences sleep quality, anxiety, depression and pain in university students. Methods: We carried out an analytical cross-sectional study. For data collection, the following self-administered questionnaires were used: Smartphone Addiction Inventory (SPAI-BR), to assess smartphone dependence; Pittsburgh Sleep Quality Index (PSQI), to evaluate sleep quality; Hospital Anxiety and Depression Scale, to assess anxiety and depression (HADS), where it was subdivided into HADS-A for anxiety and HADS-D for depression; and Numeric Rating Scale (NRS) to determine physical pain intensity. The sample consisted of 301 university students studying physiotherapy and physical education at the State University of Northern Paraná (UENP). They were divided according to the score obtained in the SPAI-BR between the “regular” group (up to 6 points) and “predisposed” to smartphone dependence (7 or more points). Results: The comparisons were statistically significant in favor of the regular group: the predisposed group obtained a higher score for the questionnaires used with an average NRS of 2.37 points, average HADS-D of 9.05 points and average HADS-A of 6.01 points. Differences between groups were statistically significant: NRS, p = 0.018; HADS-A, p = 0.001; HADS-D p = 0.001; and PSQI, p = 0.001. Conclusion: The university students analyzed in this study classified as predisposed were more prone to being addicted to their smartphone, and they were more likely to have anxiety, with a worse quality of sleep and with a greater intensity of pain.

Effect of unilateral bag use on plantar pressures and static balance in women 单侧袋使用对女性足底压力和静态平衡的影响

Abstract Introduction: The increasing insertion of women into the labor market has created a need to adapt handbags, with different sizes and weights, which consequently can overload the musculoskeletal system. Objective: To evaluate the effect of using a unilateral bag on plantar pressures and static balance in women. Methods: Cross-sectional study, carried out in Fortaleza in 2018. 258 women aged between 18 and 59 years who used a unilateral bag participated in this study. Two questionnaires were applied, targeting sociodemographic variables, life habits, characteristics of bag use and level of physical activity. Height, scapular symmetry, body and bag mass were also measured. A baropodometer was used to assess plantar pressure and static balance with and without the unilateral bag. Independent and paired t-tests were applied to verify the influence of the bag on the variables of interest, using the SPSS Statistics program (version 23.0). Results: On the side where the bag was carried, an increase in lateral mass distribution (LMD), foot pressure and surface area, and a decrease in the distance from barycenter (p < 0.05) were observed. On the opposite side, a decrease in LMD and an increase in barycenter were detected (p < 0.05). In static balance, no differences were observed in the anteroposterior and side-to-side oscillations with bag placement (p > 0.05). Conclusion: The unilateral bag causes alterations in plantar pressures and ipsilateral barycenter alongside the use of the bag, being a risk factor or aggravation for dysfunctions of the musculoskeletal system and for the occurrence of pain.

Objective: To evaluate the effect of using a unilateral bag on plantar pressures and static balance in women. Methods: Cross-sectional study, carried out in Fortaleza in 2018. 258 women aged between 18 and 59 years who used a unilateral bag participated in this study. Two questionnaires were applied, targeting sociodemographic variables, life habits, characteristics of bag use and level of physical activity. Height, scapular symmetry, body and bag mass were also measured. A baropodometer was used to assess plantar pressure and static balance with and without the unilateral bag. Independent and paired t-tests were applied to verify the influence of the bag on the variables of interest, using the SPSS Statistics program (version 23.0). Results: On the side where the bag was carried, an increase in lateral mass distribution (LMD), foot pressure and surface area, and a decrease in the distance from barycenter (p < 0.05) were observed. On the opposite side, a decrease in LMD and an increase in barycenter were detected (p < 0.05). In static balance, no differences were observed in the anteroposterior and side-to-side oscillations with bag placement (p > 0.05).
