Purpose. The arterio-venous anastomosis (AVA) continues to be the "gold standard" for vascular access in patients undergoing haemodialysis. This retrospective study aims to analyze the causes that led to End Stage Renal Disease (ESRD), the type of native AVA, their localization, and their patency. Material and Methods. The study included the last 100 consecutive and random AVAs constructed in our clinic. 68 men of average age 62.3 (+/-10.8) years and 32 women of average age 59.9 (+/-15.8) years were operated. The main causes that led to ESRD were: Glomerulonephritis - 33%, Hypertension- 24% and Diabetes- 20%. The overweight patients were 33% and the smokers - 30%. Results. AVA has been successfully constructed in 82% of the patients on the left hand and in 18% on the right hand. A distal latero-terminal radio-cephalic fistula (DLTRC) was constructed on only 8% of the patients. The most common type of anastomosis was an L.T. radio-cephalic in the forearm - 58%, followed by an L.T. brachio-cephalic fistula - 19% and latero-lateral, radio-cephalic in the forearm - 14% of the patients. The early fistula patency was - 80%, one-year cumulative patency was 94%. Conclusion. The analysis of our results shows that the relative share of the overweight and diabetes population is increasing today. In practice, solutions for the construction of primary forearm anastomosis are increasingly common.

Purpose: To investigate the influence of self-assessed working environment factors and organizational communication on professional satisfaction among dentists in Bulgaria. Material/Methods: A self-administered anonymous questionnaire consisting of 37 items about socio-demographic characteristics, motivation, work environment and economic factors was mailed to a group of 1427 dentists from 107 cities and villages in Bulgaria. Job satisfaction was assessed by using a three-point scale – satisfied, partly and dissatisfied. To search for significant associations statistical package, IBM SPSS Statistics 25.0 was used. The significance level was set at p<0.05. Results: A total of 436 dentists provided duly completed questionnaires (response rate=30.5%). Type of clinical patient-centered activities, availability of dental assistants, working conditions (dental equipment and supplies), and non-clinical activities (sanitary and hygienic duties, document workflow, dental practice management, financial planning, etc.) were significantly associated with job satisfaction (p<0.05). Working hours, proper planning of the working schedule, and dentist-patient interactions were not found to be related to levels of professional satisfaction (p≥0.05). Conclusions: Dentists working under unsatisfactory conditions, without a dental assistant employed, those working only in their specialty and performing sanitary-hygienic and administrative activities, were most likely to be dissatisfied with their jobs.

Effective doctor-patient communication is considered an important prerequisite in providing high-quality medical care. In modern healthcare, a new model of patient-centered collaboration and partnership is established. A general practitioner’s communication culture and skills are crucial parts of his/her qualification and activities and have a direct influence on the doctor-patient relationship and form a patient’s trust in the GP. Our study aimed to analyze the problems arising in the process of doctor-patient communication. Material/Methods: An anonymous inquiry with a questionnaire was conducted between September, 1 and September 30, 2018, in the Pleven region, Bulgaria with 1053 respondents visiting their GPs during this period. The data we collected through empirical research is presented and evaluated, based on a general analysis of the communication act. The results obtained were processed with Microsoft Office Excel 2007. The quantitative analysis was performed with SPSS 17.0. Results: The analysis elucidated weaknesses and unsolved problems in doctor-patient communication. A particularly serious problem identified was that of verbal communication of the physicians with their patients: the ability to ask questions and listen, explain, and guide patients on proper behavior and lifestyle. Conclusions: There are problems concerning doctor-patient communication. Verbal communication is a major problem, which could be attributed to insufficient training. We recommend improving and updating the communicative skills training programs for physicians and other health care professionals in primary care.

Purpose: The aim of the study is to determine the pain during intraosseous anesthesia (IA) with the Quicksleeper and the post-operative state of the perforation site. Materials and methods: The subjects of the study were 58 patients who underwent successful intraosseous analgesia, indicated with treatment of caries or irreversible pulpitis of mandibular first and second molars. The Visual Analogue Scale was used to assess the pain during the perforation of the compact bone and during the infiltration of anesthetic solution into the cancellous bone. All 58 patients were followed up at the 24th hour and on the 7th day after the manipulation, and the following parameters were evaluated: condition of the perforation site and presence of palpation pain. Results: Only 10 patients registered mild pain during perforation of compact bone and 8 – during anaesthetic infiltration. At 24 hours after IA, 12 patients registered mild pain, 6 patients registered moderate pain at the perforation site. On the 7th day, we established mild pain in 4 patients. After IA a reactive inflammatory process was registered in 27% of the cases at 24 hours, while on the 7th day, the inflammatory reaction was registered in only 6% of the cases. Conclusion: The intraosseous system Quicksleeper allows painless perforation of the compact bone and infiltration of the anesthetic solution into the cancellous bone due without risk of serious post-operative complications at the site of anesthesia.

Background: The auriculotemporal nerve is one of the main branches of the mandibular nerve. It provides sensory innervation to the auricle, the scalp in the temporal region, the temporomandibular joint and part of the dura mater, the parasympathetic innervation of the parotid salivary gland, as well as the external surface of the tympanic membrane, and the sympathetic innervation of the sweat glands and of the blood vessels passing through the parotid gland. Its interruption during surgical interventions or trauma can results in its most common disease - Frey's syndrome. This article is an overview and is based on 23 foreign scientific works - articles published in refereed and renowned scientific publications and anatomy atlases by world-renowned and proven specialists in their field. Review Results: The auriculotemporal nerve is a sensory nerve which includes secretory parasympathetic vasomotor sympathetic fibers coming from the otic ganglion. It is a branch of the mandibular nerve (V3). Its five terminal nerve branches are front auricular branches, branches for the external auditory canal, branches for the temporomandibular joint, branches for the parotid gland and superficial temporal branches. Frey's syndrome occurs when the auriculotemporal nerve is cut or injured proximal to the site of separation of the branches for the parotid gland or are severed themselves. The clinical picture characteristic of Frey's syndrome includes redness, sweating, warmth and itching in the area of innervation of the nerve and less often pain. Conclusion: Knowledge of the anatomy of the auriculotemporal nerve and Frey's syndrome are essential for understanding its nature and stand on the basis of its prevention.

Purpose: Urinary incontinence leads to adverse life changes and psychological stress among patients. The purpose of this report is to follow the effect of implementing a specific exercise program in women with urinary stress incontinence. Material: The study included 30 women who completed a specialized kinesitherapy program over 16 weeks. It included exercises to strengthen the back, abdominal and gluteal muscles, Kegel exercises and balance exercises. Results: A one-hour Pad test was used to monitor the effect of applied kinesitherapy. Results are reported as means with standard deviations with 95% confidence intervals. Conclusions: The obtained results show that specialized exercises have a beneficial effect on the tone of the pelvic floor muscles and reduce the symptoms of urinary stress incontinence.

Introduction: Articular cartilage damage is quite a common problem within the general population, especially in the younger and more athletic populations. These defects, if left untreated, would lead to post-traumatic osteoarthritis. Taking into consideration the high frequency and socioeconomic burden of osteoarthritis, the treatment of osteochondral lesions presents a unique challenge for surgeons. Materials/Methods: Several hypotheses were formulated to discover whether mosaicplasty could be used in order to successfully treat osteochondral lesions. Furthermore, open mini and arthroscopic surgery were tested to determine whether they could achieve the same result as mosaicplasty. A sample of 45 patients from the general population volunteered for the study, of whom 13 patients underwent open mosaicplasty, 17 mini-open and 15 patients arthroscopic mosaicplasty. Assessment of the knee articular cartilage reconstruction was done during each visit, utilising Lysholm and the VAS scale. A histological examination was also performed. Results: A significant difference was observed between pre- and post-surgical Lysholm score, as well as between pre- and post-surgical VAS score. Additionally, histological analysis confirmed the success of osteochondral autograft transfer and integration into the recipient's knee. Conclusion: The study demonstrated that several mosaicplasty techniques could be used in articular cartilage reconstruction. Moreover, the histological examination further confirmed the viability of the autograft.

A 55-year-old man presented with low backache, paraesthesia, and spasticity of both lower limbs. He had urinary retention and constipation. Investigations revealed a vascular intradural cauda equina tumor. MRI scan demonstrated an enhancing mass at the third and fourth lumbar vertebral levels. There were multiple dilated and tortuous veins draining from both poles of the tumor. At surgery, we found a well-defined tumor with an orange hue and fleshy consistency was encountered arising from the filum terminale, comprising a few adjacent nerve roots. The tumor was excised en bloc. The filum terminale was also been resected. A sporadic hemangioblastoma arising from the filum terminale should be considered as a rare cause of back pain and sciatica. Total excision of the tumor offers a cure.

Background: The coronavirus is one of the common pathogens among humans and animals that causes various respiratory diseases Methods: Alternative, non-parametric and correlation analysis were used. Logical units of observation: Two hundred patients suffered from COVID-19 in the period from 01.09.2020 until 01.09.2021. Results: In our study, the majority of respondents who had recovered from COVID-19 reported having one or more persistent symptoms. Only 1% had no residual complaints. With regard to the dating of the complaints that appeared, 74% or the main part of our respondents indicated a period of 1-3 months, and the fewest complaints were observed in the 9-12 months after recovery. As age increases, cardiac complaints also increase p=0.001 (χ2=79.51). The correlation relationship is straight and significant p=0.001 (r=0.619). Conclusion: Complex rehabilitation is part of the overall treatment plan and includes special physical and rehabilitation activities, health care, healthy behavior, education and prevention of post-infection complications.

Purpose: The aim of this report is to measure and analyse the isometric and isokinetic (concentric) peak torque (PT) of knee extensors and flexors of the involved and uninvolved limb of twin professional football players over one year post anterior cruciate ligament reconstruction (ACLR). To calculate the unilateral and bilateral asymmetry indexes and compare the athlete's PT values with sport-specific and position-specific data. Case description: Strength measurements of the knee extensors and flexors were conducted on two ACLR football players (twins) on the involved and uninvolved limbs in isometric and concentric modes of muscle contractions, and unilateral/bilateral asymmetry indexes were calculated. Results: The peak torque isometric data indicated that the strength characteristics of the involved leg extensors in both patients were significantly lower compared to the uninvolved limb and were indicators of higher secondary injury risk. The first athlete's concentric peak torque values showed relatively good return-to-sport results, while the second football player returned to sport having not met the recovery criteria. Both athletes require additional specific muscle strength exercises on the extension and flexion muscle groups to reach the ACL reconstruction recovery targets. Conclusions: In this case report, the possibilities of isokinetic dynamometric testing in an isometric and concentric mode of football players to more accurately assess the extent of the muscle strength deficiency and asymmetry are demonstrated. This more precise information allows to the creation of correct training and therapeutic program to recover the knee muscle groups' strength and unilateral/bilateral strength balance to avoid reinjury.
