After the pandemic and at the beginning of 2023, our team carried out a survey among active athletes about the risks associated with smoking. The purpose of the study is to assess smoking as a risk factor in athletes using a survey method. Methods: The respondents were asked to participate anonymously in a survey consisting of 14 questions. We surveyed 50 active athletes (35 men and 15 women). Results: Forty-three of the participants were aged between 18 and 22 years old, and 7 were aged between 23 and 27 years old. Results: 30% of the surveyed athletes are smokers, 40% are non-smokers and 30% used to smoke in the past; 66% are familiar with the risk of smoking; 42% would support a ban on smoking in public; 22% report that cigarettes influence athletic achievements; 80% of the athletes consider that they are smoking due to stress; 34% think that only a health problem could make them quit smoking. Conclusions: The fact that 66% of the athletes are aware of the risks associated with smoking gives us hope in the combat against smoking. The effect of cigarettes on the human body should be emphasized. The survey showed that there are insufficient campaigns to raise awareness about the harmful effects of smoking which is key to quitting cigarettes.

Global coronavirus disease 2019 (COVID-19) has posed major challenges to clinical laboratories, from initial diagnosis to patient monitoring and treatment. Some routine laboratory markers reveal a range of changes in patients with COVID-19. Among the most frequently observed are: increased levels of C-Reactive Protein (CRP), Lactate dehydrogenase (LDH), D-Dimer neutrophilia with lymphopenia and decreased platelet count. The aim of our study is to present the changes in the values of CRP, LDH, D-Dimer, number of Thrombocytes (PLT), Lymphocytes (Ly) and Neutrophils (Neu) during hospitalization and at the discharge of patients with Sars-Cov-2. Materials and methods: The object of the study are clinical laboratory results of 106 patients who underwent hospital treatment for Covid -19 in the "St. Ivan Rilski" St. Zagora, divided into two groups. The first group (Group I) is in the period November-December 2020, the second group (Group II) patients were hospitalized in April 2021. The values of CRP, LDH, D-Dimer, number of platelets, Ly, Neu from the day of the patient's admission to the hospital and from his discharge were examined. Results and discussion: In 100% of the examined patients in the first group CRP with values at the time of admission to the hospital (TAH) 88.70 ± 63.132 mg/L. LDH was increased in three of the groups in more than 90% of the patients, with the highest values of the enzyme being recorded in Group II at the end of hospitalization, 802.33 U/L (± 557.836). In the studied groups, we found significantly more patients with elevated values in both groups at discharge (Group I 49.12% and Group II 46.94%). The highest number of patients with thrombocytopenia was observed in Group II at the time of admission to the hospital, 26.53% (n=13). Conclusion: Quantification of these and other laboratory parameters could significantly aid the detection of at-risk patients and reveal disease severity. They represent objective and standardized criteria guiding therapy.

Introduction: The daily practice shows that the patients prefer treatments that require fewer visits and low cost. The technology of conventional complete dentures follows several subsequent stages – clinical and laboratory. The search for alternative methods has led to the more and more successful development of digital technologies that could also be used for complete dentures. The conclusions of their use include higher quality, simplification of the laboratory technologies, and creation of functionally fit prosthetic structures. Purpose: The purpose of this research is to systematically review the up-to-date information available in the scientific databases in regard to the implementation of CAD/CAM technologies for the complete prosthesis. Materials and methods: A search was carried out in the period from January 2022 to October 2022 in the electronic databases PubMed, Google Scholar and ScienceDirect, using the following keywords: CAD/CAM, digital prosthesis, complete dentures, planning of complete dentures, edentulous patients, in English, German, Turkish and Bulgarian language. Conclusion: The individual anatomic and morphological particularities, age and functional changes occurring after the complete loss of teeth create conditions for the prosthesis that cannot be unified according to the scientific literature. Most probably, this is the reason why there is no straight protocol and straight technology for producing complete dentures using the digital method.

Introduction: Hypodontia and hyperdontia are two dental phenomena with deviations in the number of teeth - reduction and increase of tooth germs. The aim of the study is to compare the frequency of manifestation of hyperdontia and hypodontia, their distribution by tooth groups among Bulgarian orthodontic patients. Materials and methods: The prospective clinical-epidemiological study covered 2676 patients: 1705 (63.7%) female and 971 (36.3%) male. They were detected and diagnostically proven with supernumerary and missing teeth. The findings were distributed according to their localization to make a comparative analysis. Results: The patients established with hypodontia were 5.79% with average age of 15.17±5.34 years, and hyperdontia in 3.13% of patients with average age of 11.80±4.65 years. Statistically credible, hyperdontia was more frequently observed in males, while hypodontia was significantly more frequently observed in females. The hypodontia was the most commonly observed in the mandibular distal segment - second premolars (46.5%), next affected was the maxillary lateral (37.4%), followed by the maxillary distal segment (32.3%), and the least was the mandibular frontal segment (12.9%). The supernumerary teeth found in the maxillary frontal segment were incisors and mesiodens (91%) compared to the maxillary distal segment (9%) was significant. In the mandible, the difference between the frontal (42.3%) and distal segment (57.7%) was without statistical significance. Conclusions: Knowledge of the clinical characteristics of hypodontia and hyperdontia are important for targeted early diagnosis and treatment. Both of these specific problems have an effect on an individual's function and psychosocial well-being.

Purpose: Recently, quantitative dermatoglyphic examinations have been intensively carried out for screening purposes in oncology. Material/Methods: The present study covered 82 women aged between 36 and 80 years with clinically, histologically and mammographically confirmed breast cancer and 60 healthy women aged between 31 and 79 years from the region of Varna, Bulgaria. Two dermatoglyphic methods were applied: i) dactyloscopy of finger ridge count and finger indexes and ii) palmoscopy - palmar ridge count, palmar maximal atd, adt and dat angles, and palmar main lines. Statistical data processing was performed by means of variation analysis, regression analysis, χ2 test and one-way ANOVA using the SPSS software package, version 19. Results: The results from the dactyloscopic and palmoscopic examinations revealed several statistically significant differences in these quantitative dermatoglyphic patterns between breast cancer females and healthy ones. Conclusion: We could conclude that the purposeful examinations of the quantitative dactyloscopic patterns such as total finger ridge count and finger indexes and quantitative palmoscopic traits such as palmar ridge count, maximal adt, atd and dat angles and main palmar lines play a promising role for the cost-effective and trustworthy breast cancer screening in Bulgaria.

In our practice, we are increasingly seeing men aged from 37 to 45 years' old who have permanent low normal testosterone levels. Aim: We set out to investigate whether there is an association between permanent low normal testosterone levels and negative change in seminal fluid parameters in young men. Patients and Methods: For the period from January 2013 to December 2015 at the Andrology office at Hospital "St. Sofia" we examined 73 men aged 37 to 45 years with normal or elevated body mass index, permanent low normal testosterone level, and negative change in seminal fluid parameters. In order to compare the results we obtained, at the very beginning of the study we selected a control group of 20 healthy men of the same age. Results: We obtained, although within reference ranges, significantly lower values for total testosterone in the 73 men with negative change in seminal fluid parameters, compared with those in the control group without seminal damages (p<0.001) We found a high correlation relationship between the level of testosterone and the results of the first (r = 0.614, p<0.001) and second spermograms (r = 0.662, p<0.001). Conclusions: 1. Our study shows that in a number of men at a young age, some decrease in normal testosteronе secretion occurs, with a concomitant negative change in seminal fluid parameters, which is remarkably different from the same parameters in their peers with a high normal testosterone level. 2. We identify permanent low normal testosterone, overweight and obesity as predictors, signaling a possible negative change in seminal fluid parameters. 3. We can say that if obesity plays some role in seminal damages, the mechanism in most cases is most likely related to the sustained permanent low normal testosterone level, as a result of increased adipose tissue.

Background: Blindness (anopia) is a functional loss of vision, which could be due either to ophthalmological or neurological conditions. Blindness may be congenital or acquired. The main causes of blindness in adults are cataracts, uncorrected ametropia (myopia, astigmatism), glaucoma, macular dystrophy, cortical ischemic strokes, etc. In less than 10% of the patients, the reason for cortical blindness is bilateral occipital strokes, one of them hemorrhagic. Case Description: In the current case report, we present a 75-year-old patient with complaints of headache and visual loss. The patient has a history of a left PCA (posterior cerebral artery) distal ischemic stroke. The CT scan reveals a hypodense cerebral lesion in the left medial occipital cortex and subcortex (lingual gyri and cuneus) and a hyperdense cerebral lesion in the right medial occipital cortex and subcortex (lingual gyri and cuneus). The neurological examination shows only bilateral visual loss without motor deficits. The patient has visual anosognosia and confabulation in the setting of obvious visual loss and cortical blindness(Anton-Babinski syndrome, also known as ABS or Anton syndrome). However, performing cerebellar tests (dysdiadochokinesis, finger-to-nose test, heel-to-shin test, cerebellar ataxia) was not done because of the visual loss. After being consulted by a neurologist and neurosurgeon with the preliminary diagnosis of brain hemorrhage, the patient was admitted to the Neurology Clinic of UMHAT D-r "Georgi Stranski", Pleven, Bulgaria. Conclusion: Occipital infarction must be considered in all cases with sudden onset of isolated visual loss. Early diagnosis and treatment of stroke reduce mortality and morbidity. The prognosis depends on the extension of the visual cortex damage.

Prosthetic treatment of a patient with manifested nausea reflex could be a dramatic experience. There is a risk of aspiration or ingestion of foreign objects, and the dentist may have great difficulties during the procedures. Purpose: The purpose is to report the efficiency of homeopathic medicines in the case of prosthetic treatment with removable dentures of a patient with a gag reflex and several medical conditions. There is a risk of aspiration of foreign bodies. This leads to compromises in the final treatment quality and difficulty wearing a denture because of gagging. Homeopathy is a safe and natural alternative that finds application in dentistry. Material/Methods: A case of a female patient is reported. Age of 63 with accompanying diseases. The patient has an exaggerated gag reflex during intraoral examination and needs prosthetic treatment with removable dentures. The gag reflex was assessed by the Classification of Gagging Problem Index (CGP), and examination of reflex reaction with a sterile swab and monofilaments were done before and after the reception of homeopathic medicines. Results: During the first and second visits, the results of the CGP index were recorded in the patient's clinical record. The results of the tests with the monofilaments and the sterile swab before and after taking homeopathic medicines were filled in tables and were compared. Conclusions: A satisfactory, harmless and long-lasting result in the reduction of the nausea reflex is achieved in the reported case by homeopathic medicines. All clinical stages of removable dentures' fabrication were possible, and the patient tolerated the new prostheses pretty well.

Revision total hip arthroplasty (RTHA) is a challenging procedure when we speak about acetabular defects and recurrent instability. There are multiple reasons like the number of previous revisions, implant position, bone quality, patient characteristics and etc. Managing acetabular defects with porous trabecular metal augments has excellent survivorship, but when addressing instability, there are some unsolved issues. Management options include the use of large femoral heads, constrained acetabular liners, and dual mobility cups (DMC). These cups have increased jump distance and reduced likelihood of dislocation while not relying on locking mechanisms, therefore are the preferred option in RTHA. We present single-center series of 13 patients undergoing revision THA with a specific DMC cemented into a newly implanted or well-fixed trabecular metal (TM) revision shell. All patients underwent cementation of a single specific cemented DMC design (Novae Stick, SERF, Decines, France) in a Trabecular Metal Revision Shell (Zimmer Biomet, Warsaw, IN, US). The mean follow-up is 15,2 months, and there were no dislocations of the implanted DM cups. The mean HHS improved significantly from 35.9 (range 15–80) preoperatively to 80.1 (range 64-93) postoperatively (p = 0.001). The results of this small clinical series demonstrate that cementing a DMC in a well-fixed TM revision shell leads to no dislocation or dissociation of the cement interface and acetabular construct loosening in the short term.

Tumors of the peripheral nervous system are classified according to the specific origin of cell differentiation. Schwannomas (neurilemmomas) are benign nerve sheath tumors slowly growing, often solitary, arising from the nerves' Schwann cells that are not generally associated with malignant degeneration. The cause is unknown. The authors present a case of a 37-year-old woman with a lump formation located at the anterior aspect of the knee joint, inducing a burning-like sensation around the knee and up the whole hip. The pain increased through motion of the knee joint and during palpation. Prior to these complaints, the patient does not recall any trauma or degenerative knee conditions.
