The authors developed a new portable transcutaneous blood glucose monitoring system using non-invasive collection of suction effusion fluid (SEF) from human skin. A ion sensitive field effect transistor (ISFET) sensor was employed to measure glucose concentration in a very small quantity of the SEF. The system is self-contained for portable usage during up to 6 hrs monitoring. This system may be the first blood glucose monitoring equipment which does not use blood sampling.<>

Fractal analysis for the adherent platelets under flow conditions 流动条件下粘附血小板的分形分析

The kinetics of platelets aggregation on protein-coated surfaces under flow conditions was studied using a PC-based imaging system. The fluorescence-labeled platelets were in whole blood flowing through a chamber. The platelets deposited onto the protein-coated surface in a time-dependent manner. The application of the fractal dimension to characterize the distribution of platelets deposition provided a measure of the degree of correlation of the signal over time.

Electrosurgical sealing of large canine vessels (3-5 mm) can be feedback controlled by thermocouple-measured temperatures at the surface of the vessel. The duty cycle of a 500 kHz constant voltage source is regulated by means similar to pulsed width modulation, temperatures in different tissues were repeatable. Also, histologic analysis reveals thermal damage in the tissue and sealing in the intimal layer.<>

Time-frequency analysis of the electrogastrogram: evaluation of STFT, adaptive spectral analysis and exponential distribution 胃电的时频分析:STFT的评价,自适应谱分析和指数分布

The aim of this paper is to compare the performance of the short-time Fourier transform (STFT), adaptive spectral analysis and exponential distribution (ED) methods in estimating the changes of both amplitude and frequency of the gastric slow wave with time, and detecting gastric dysrhythmic events from the electrogastrogram (EGG). Results from both computer simulations and real applications show that the adaptive spectral analysis method is more accurate in the detection of gastric dysrhythmia with brief duration, whereas STFT method is more appropriate for the estimation of signal amplitude. The performance of the ED method appears to be in between the STFT method and adaptive spectral analysis method.

Early detection of infection and rejection in lung transplantation 肺移植感染和排斥反应的早期发现

A paperless electronic diary/spirometer instrument has been tested in a home monitoring program for subjects who have undergone lung transplantation. The objective of the program is to provide early defection of infection or rejection so that therapeutic intervention can be instituted at the earliest sign of potential clinical problems. This study reports on the early detection capability of the home monitoring program, focusing on example cases of chronic rejection (OB) and infection (CMV). Declines in spirometry and increases in respiratory symptoms appeared in the home data 30-90 days before infection or rejection diagnoses were made in clinic. Thus home monitoring can provide a method for the early detection of infection or rejection after lung transplantation.<>

A distributed system for medical consultation and education 分布式医疗咨询教育系统

Pathology is an image intensive discipline which utilizes sensed data to render diagnostic decisions and to engage in a broad spectrum of research ranging from the molecular to the macroscopic. The interaction with images is generally personal, subjective, and limited to some degree by geography. Given the nature of the diagnostic process and the medical impact of an incorrect assessment, improvements in it might be realized if the process were less subjective and open to multiple viewers. Digital computer networks transcend many of the barriers associated with analog modes of communication, but while the technology continues to evolve, significant obstacles persist which prevent individuals located at disparate imaging facilities from interacting effectively. Particularly problematic in this regard is the fact that there are presently no true standards regarding the architecture, operating system, and graphic user interface (GUI) of interacting machines. The authors' goal was to develop an inexpensive means by which digital information and computer resources could be shared among distant clinical and research facilities irrespective of these parameters. They present a prototypic network of heterogeneous computer architectures which has been configured to assist investigators located at remote sites to archive, display, manipulate and share medical information such as health-related images and clinical reports. Since the inception of the network in November, 1994, the network has managed more than one thousand interactive imaging sessions and its medical databases are accessed at a rate of more than 300 queries/day by users around the globe. The network is presently being evaluated by a consortium of imaging sites throughout the health care community to explore its potential role in broader range digital consultation and distance learning applications.

New equipment for necrotic burn treatment using excimer laser irradiations 准分子激光治疗坏死性烧伤的新设备

The authors developed a new equipment for heavily burned skin (burn eschar) treatment using excimer laser ablation. Although auto-skin graft after necrotic tissue removal (debridement) is the only way to save a burned patient, the procedure of the debridement has not been well-established. The authors have an idea to use excimer laser ablation for debridement. Since the burn eschar may have a high protein concentration with less water content compared with normal skin, the use of the excimer laser irradiation may induce efficient ablation. The authors found that the burn eschar can be eliminate efficiently with a sufficient ablation selectivity from normal skin using a KrF excimer laser (/spl lambda/=248 nm). In order to develop and attain fast and wide area elimination of burn eschar, the authors designed a manipulator for laser delivery. They think this new equipment might be useful for the treatment of burned patients.

Time-frequency analysis of 24-hour heart rate fluctuation for the characterization of very low-frequency component 24小时心率波动的时频分析用于表征极低频成分

A smooth pseudo Wigner-Ville distribution was employed to investigate both time and frequency characteristics in a very low-frequency component (less than 0.04 Hz) of 24-hour heart rate fluctuation. In some cases of heart failure the authors could see remarkable and very low-frequency oscillations of the Wigner-Ville distribution, which are related to Cheyne-Stokes respiration. However the Wigner-Ville distribution of normal subjects did not have such oscillations, but more random spectral characteristics instead. Furthermore, the authors calculated 1/f/sup /spl alpha// regression slopes of average spectrum to be a crude estimate of fractal dimensions. The slopes are more negative in heart failure patients than normals (-1.866/spl plusmn/0.888 vs. -0.957/spl plusmn/0.180 p<0.005), which associates a loss of dimensionality of heart rate fluctuation in heart failure.

Template generation for neural waveform analysis in extracellular recordings 细胞外记录中神经波形分析的模板生成

A fully automated system for sorting the action potential waveforms of multiple neurons in extracellular recordings has to generate a template for each neuron by averaging waveforms produced by that neuron. This requires a multidimensional clustering approach that can also segment waveforms from the continual recording, and average them with appropriate alignment. The performance of the sorting process is dictated by the quality of the templates. Confusion errors between classes and misalignment errors within classes deteriorate the templates generated by available algorithms. An algorithm that the authors developed was evaluated with simulations and physiological data.
