The unilateral sagittal split osteotomy may not be a particularly popularized method to access and remove impacted mandibular third molars (M3Ms). However, in unusual cases of deep impaction, it can provide a surgical option that minimizes bone removal and may reduce the risk of iatrogenic mandibular fracture and inferior alveolar nerve (IAN) injury, depending on the position of the third molar relative to the neurovascular bundle. The unilateral sagittal split osteotomy may present an alternative method for removing impacted mandibular third molar teeth in certain situations.

The rehabilitation of a combined mid-facial defect using a magnetretained intra- and extra-oral prosthesis: a case report 使用磁固位口内和口外假牙修复合并颌面中部缺损:病例报告

There are several treatment options available to restore function and appearance in patients with combined mid-facial defects. Owing to the size of the defect and the associated size and weight of the prostheses, adequate retention can be challenging to achieve. Retention of a facial prosthesis can be improved using several techniques, such as medical grade adhesives, attachments, or osseo-integrated implants. For patients with a history of radiotherapy, surgical rehabilitation may be less favourable than non-surgical approaches owing to the risk of osteoradionecrosis and a reduced blood supply in the surgical fields. This clinical case report highlights the conservative rehabilitation of a combined mid-facial defect using a magnet-retained intra-oral and extra-oral prosthesis. A thorough medical and clinical assessment will highlight factors that will influence the treatment planning process when managing a combined mid-facial defect.

Following the ‘2017 World Workshop on the Classification of Periodontal and Peri-Implant Diseases and Conditions,’ hard and soft tissue deficiencies have been debated in the literature with keratinized mucosa at the forefront. In this case report, the present author investigated whether an apically positioned flap and free gingival graft to increase keratinized mucosa and vestibular depth improved oral hygiene and patient comfort around an implant-retained upper complete denture. Using an apically positioned flap in combination with free gingival graft to augment the soft tissues around implants with hard and soft tissue deficiencies, increased both the keratinized mucosa and vestibular depth to correct the soft tissue deficiencies. This case report highlights the importance keratinized mucosa and vestibular depth has at the patient and site level in improving oral hygiene and patient comfort. This case report also supports the growing evidence that the ideal periodontal phenotype around implants is to have keratinized mucosa ≥2 mm and vestibular depth ≥4 mm. Many children struggle or refuse to take oral medications. The refusal of medication occurs for many reasons including learned and pre-existing anxiety, sensitivity to tastes and textures and sensory–motor/coordination difficulties (for tablet swallowing). Oral analgesics play a vital role in managing the odontogenic pain of children. This is especially true when they have limited access to appropriate dental care services, are pre-cooperative or lack the ability to be compliant for conventional dental treatment. Practical advice around the facilitation of analgesic administration for odontogenic pain is lacking for parents of children who may not accept conventional preparations.

The number of cardiac transplants carried out each year within the UK is increasing. With this rise in numbers, it is important that dental professionals are aware of both pre- and post-operative considerations for these patients. This paper considers the practical elements of managing these patients, outlining important aspects of history taking, treatment planning, and safe pain and anxiety management options. The unilateral sagittal split osteotomy may present an alternative method for removing impacted mandibular third molar teeth in certain situations.

A nasopalatine duct cyst in an orthodontic patient: a case report 一名正畸患者的鼻腭管囊肿:病例报告

This case report describes the presentation of a nasopalatine duct cyst in a 27-year-old orthodontic patient. The patient presented with pain in the anterior hard palate and a Class II division 2 occlusion with a complete overbite. The cyst was discovered upon further radiographic investigation. Following discussion at a joint orthognathic clinic, the decision was made to leave the cyst under active monitoring and review the patient clinically and radiographically after 12 months. Nasopalatine duct cysts are common, therefore awareness of their clinical and radiographic features and treatment options is essential.

A detailed and honest social history is vital when piecing together any clinical diagnostic puzzle. This case presentation highlights this, leading to a surprising diagnosis for both patient and clinician. Additionally, access to cone beam computed tomography (CBCT) has vastly widened diagnostic accuracy within the dental field, and this case emphasizes the importance of thorough training to identify, and interpret, incidental findings. Injuries obtained under the influence of drugs might be missed without in-depth social history and appropriate imaging training.
