A novel approach for the placement of zygomatic implants: the zygomatic bone zone index 植入颧骨种植体的新方法:颧骨区指数

This article explores the considerations and factors that implant dentists may use to decide whether the placement of zygomatic implants is necessary, and if so, the potential considerations in planning for them within the zygomatic bone. A novel treatment planning tool has been presented, the Zygomatic Bone Zone index, to improve the surgical placement of zygomatic implants, to help reduce complications and to allow for further placement within the zygomatic bone in the future. CPD/Clinical Relevance: It is important for clinicians to be aware of treatment modalities that might be of benefit to some patients.

Narcissism, pride, envy, lust and dentistry 自恋、骄傲、嫉妒、欲望和牙医

Platforms such as Instagram have normalized self-adulation and this has affected many patients, and some dentists, in different ways. Some posts demonstrate excessive pride in the appearance of the mouth or face, and seem designed to induce envy in others. The resultant emotional impacts on envious followers can increase demands to have a similar sort of appearance, which can lead to over-treatment, or to potentially dangerous procedures being carried out on their mouth and face regions. This article explores the nature of narcissism and the increasing likelihood of it being ‘normalized’ as one significant factor in the increased demand for, and the delivery of, some questionable dental, oral and/or facial treatments. We outline how the narcissistic traits in some dental healthcare professionals might lead them to undertake elective, biologically risky, and irreversible procedures, which can invoke ‘the law of unintended consequences’ for patients and/or themselves. The terms ‘destructodontics’ ‘relapsodontics’ and ‘alignerodontics’ are suggested as being appropriate descriptions for some dentistry, which is carried out mainly for appearance reasons, but which can produce long-term biological and other problems. CPD/Clinical Relevance: The normalization of narcissism, as demonstrated by some posts on Instagram and other platforms and in dubious advertising has many worrying implications and possible consequences for patients and the dental profession.

A case of a mandibular dentigerous cyst in a paediatric patient 一例儿科患者的下颌齿状突起囊肿病例

Cystic lesions of the jaw in the paediatric population have been infrequently documented in the literature. The majority of these lesions are considered to be developmental in nature, as opposed to the commonly noted inflammatory lesions in the adults. Although asymptomatic, these lesions are associated with a high level of anxiety for parents. Early diagnosis and management help to allay anxiety. We present a case of 6-year-old boy who had a rapidly growing lucent lesion of the mandible that needed urgent intervention. The lesion was excised, analysed histopathologically, and confirmed as a dentigerous cyst. The management of paediatric cystic lesions can be difficult and vary significantly from those of the adult population. This can be amplified because cross-sectional imaging is seldom used in children owing to the risk associated with radiation. We present our experience of dealing with rapidly progressing lesion and the lessons learned in the process. Supernumerary nines: Rare after all? an overview and case series 超常九:毕竟罕见吗?

Supernumerary molars are rarely reported in literature. A literature review was undertaken to identify the prevalence of supernumerary nines following multiple cases attending at local oral and maxillofacial clinics. The review found a prevalence range of 0.06–0.96% for supernumerary molars, with proliferation of epithelial remnants of dental lamina (induced by pressure or ‘induction factors’) being the most accepted aetiology. This article explores supernumerary molars and their associated comorbidities, such as disruption and damage to other teeth. The case studies illustrate management techniques. CPD/Clinical Relevance: An awareness of supernumerary teeth and their management is of clinical benefit.

A maxillary midline diastema is often seen in childhood as part of physiological development, but those persisting after the establishment of the permanent dentition may be a functional and aesthetic concern for which patients seek treatment. The association between an enlarged maxillary labial frenum and a maxillary midline diastema is commonly reported in the literature. However, the aetiologic role of an enlarged frenum is likely to represent only a proportion of diastema cases, and many diastemas exist without the presence of an abnormal frenum. This article provides an overview of the maxillary labial frenectomy and its role in closure of the midline diastema, providing management recommendations for practitioners. Restorative management of the posterior tooth that has undergone a pulpotomy 后牙髓核切除术后的修复管理

Pulpotomy is increasingly being recognized as a treatment option for the management of permanent teeth with extremely deep caries. These teeth can present with or without symptoms of pulpitis. Traditionally, irreversible pulpitis has been managed with pulp extirpation and root canal treatment. Contemporary research has shown that pulpotomy may be suitable to manage permanent teeth diagnosed with irreversible pulpitis. This article discusses the options and decision-making process of how to restore posterior permanent teeth that have undergone a pulpotomy. In addition, the article highlights diagnostic and material considerations relating to the role of pulpotomy in managing deep caries. CPD/Clinical Relevance: Clinicians should be aware of the use of pulpotomy in management of extremely deep caries or partial irreversible pulpitis in permanent teeth.

Acromegaly is a rare and underdiagnosed acquired disorder. It is commonly caused by an underlying pituitary adenoma that often requires surgical excision. Diagnosis is normally preceded by ~5–10 years of excessive growth owing to hypersecretion of growth hormone. This can have long-term medical complications and result in a reduced life expectancy. Acromegalic patients present to a number of healthcare professionals prior to diagnosis, including general dental practitioners. As changes in occlusion are often a patient's primary concern prior to acromegaly diagnosis, it is important that all dentists are aware of the presentation and differential diagnoses of such disorders. CPD/Clinical Relevance: Dentists should be aware of the initial presentation of acromegaly to facilitate early diagnosis.
