Bitewings are a fundamental tool for treatment decision-making in young patients as they provide essential information on caries depth, the presence of a dentine bridge and pulp retraction, radiographic signs of pulp necrosis and presence of a permanent successor. The article updates the practitioner on the use of bitewing radiographs for primary molars and provides hints and tips for maximizing success in children. This three-part series of articles challenges some popular myths about supposedly ‘ideal’ treatment plans and is designed to provoke reflection and stimulate debate. It explains the concept of ‘satisficing’, as opposed to ‘maximizing’, in dentistry, and illustrates how subconscious bias and self-interests might lead supposed experts to promote arbitrary aspirational standards and confuse them with what the law expects (the Bolam Test standard), and what is genuinely in the best interests of an individual patient. It is argued that sound, patient-centred pragmatic planning and treatment is equally valid, with wider applicability than routinely defaulting to a self-serving ‘maximalist’ approach. CPD/Clinical Relevance: The ‘satisficing’ concept has wide and profound application across many fields of dentistry.

Guided implant surgery: principles and practice 引导植入手术:原理与实践

For dentists who were trained without computers and planning software, the introduction of any sort of digital technology can be met with reservation. Clinicians had to weigh the benefits of a new technique versus the cost and time involved in making it an effective investment. In low- and middle-income countries, the introduction of guided implant surgery is still in a nascent state. The procedures that fall into this category merge patients' data, both clinical and radiographic, to better visualize and guide access to the target site. Guided implant surgery is a minimally invasive, reproducible technique that substantially reduces the risk of injuring critical anatomical structures. Previously published data on guided implant surgery are limited and contain outdated methodology. In this review, the authors introduce the benefits, general principles and updated clinical workflow for guided dental implant surgery. CPD/Clinical Relevance: Guided implant surgery is a reproducible technique that results in predictable implant placement.

Gingival recession affects more than half of the population and has a multifactorial aetiology. Despite this, the knowledge and awareness of the factors that predispose sites to recession remains limited, with most people associating it with poor oral hygiene. As patients retain their teeth for longer, the risk of recession is likely to grow, thus increasing the need to establish a greater understanding of this very common condition. This two-part series provides a contemporary overview of the condition with the first article discussing its prevalence, aetiology and classification, and the second covering the different management options. CPD/Clinical Relevance: It is important to establish the aetiology of gingival recession to manage the condition effectively.

Autotransplantation of an impacted migrated mandibular canine using platelet-rich fibrin and physio-dispenser system: a report of two cases 使用富血小板纤维蛋白和物理分配器系统对撞击移位的下颌犬进行自体移植:两个病例的报告

Canine impaction and migration is conventionally managed by fixed orthodontic treatment with surgical exposure; however this is not always possible, thus requiring clinicians to explore other treatment options. Autotransplantation of the tooth has been recognized as an alternative treatment option. This article reports on two cases where a migrated canine was asymptomatic, and accidentally diagnosed during routine examination. The procedure undertaken was autotransplantation of the canine using a physio-dispenser system and a platelet-rich fibrin (PRF) membrane to promote bone and soft tissue healing and regeneration. After a 2-year follow up, the autotransplanted canines showed no signs or symptoms of clinical or radiological failure. On radiographic examination, a patent periodontal ligament space was also appreciated. CPD/Clinical Relevance: Use of a physio-dispenser system and PRF during autotransplantation of a migrated and impacted canine may improve the clinical outcome.

Preservation of the primary teeth until exfoliation is one of the most important aspects of preventive and interceptive dentistry. Premature loss of the second primary molar before the eruption of the permanent first molar may lead to space loss that results in tooth size/arch length discrepancy. Early loss of a second primary molar is a challenge to paediatric dentists if the first permanent molar has not erupted. A space maintainer of the intra-alveolar type assists in controlling the eruption of the permanent first molar in these circumstances. This report describes the case of space maintenance in a 3-year-old girl for whom the primary second molar was extracted owing to a poor prognosis. A pre-formed intra-alveolar space maintainer, which in this case was a Roche's distal shoe type, was given and followed up for 12 months. CPD/Clinical Relevance: This pre-formed distal shoe space maintainer gives the advantage of reduced chairside time, more stability and strength, and is better accepted by the child.

Premature loss of the second primary molar before the eruption of the permanent first molar may lead to space loss that results in tooth size/arch length discrepancy. Early loss of a second primary molar is a challenge to paediatric dentists if the first permanent molar has not erupted. A space maintainer of the intra-alveolar type assists in controlling the eruption of the permanent first molar in these circumstances. This report describes the case of space maintenance in a 3-year-old girl for whom the primary second molar was extracted owing to a poor prognosis. A pre-formed intra-alveolar space maintainer, which in this case was a Roche's distal shoe type, was given and followed up for 12 months.
