Fissure sealants (FS) are micro-invasive procedures that have proved to be effective in caries prevention in caries-free, first permanent molars. Within high-caries risk individuals, fissure sealants arrest the caries process in early caries lesions stages. Composite resin (CR) sealants have been recognized as the gold standard FS material; however, new evidence has emerged in relation to GIC sealants in their applicability and longevity. This review provides evidence-based information supporting the effectiveness of GIC sealants in caries prevention in comparison to CR sealants, by assessing their properties, methodology and utility in healthy and anomaly-affected teeth. CPD/Clinical Relevance: This article will inform clinicians about the effectiveness of GIC sealants and CR sealants in caries prevention.

‘Satisficing’ in dentistry: who decides? who benefits? 牙科中的 "满意服务":谁决定谁受益?

This series of articles challenges some popular myths about supposedly ‘ideal’ treatment plans, and is designed to provoke reflection and stimulate debate. It explains the concept of ‘satisficing’ (as opposed to ‘maximizing’) in dentistry, and illustrates how subconscious bias and self-interests might lead supposed experts to promote arbitrary aspirational standards and confuse them with what the law expects (the Bolam Test standard) and what is genuinely in the best interests of an individual patient. It is argued that sound, patient-centred pragmatic planning and treatment is equally valid, with wider applicability than routinely defaulting to a self-serving ‘maximalist’ approach, often on spurious grounds. CPD/Clinical Relevance: The ‘satisficing’ concept has wide and profound application across many fields of dentistry; this first part explains the basic principles and how and why it is relevant to various aspects of practical dentistry and also to our understanding of professionalism.

Patient-support techniques for treating patients with learning disabilities 治疗学习障碍患者的患者支持技术

There are many techniques that can, with simple adjustments, be used for patients with learning disabilities. A number of techniques may be familiar to dentists as those which are applied to anxious or paediatrics patients. There is a lack of focused articles clearly describing specific techniques that can be used for individuals with learning disabilities and how these can be adapted to help enable dental assessment and treatment. This article aims to inform general dental practitioners of patient-support techniques that may be used or adapted for individuals with learning disabilities. CPD/Clinical Relevance: Dental professionals should be able to make reasonable adjustments to their practice to better support individuals with learning disabilities who access dental care.

Theoretical considerations for root canal re-treatment 根管再治疗的理论依据

When considering performing root canal re-treatment, we must first consider why the original treatment failed. Failure of root canal treatments can occur when ideal technical standards for control of the root canal infection are not achieved. Sometimes, extra-radicular infection may prevent a successful treatment outcome. Teeth with post-treatment peri-apical infection can be managed by either non-surgical or surgical root canal re-treatment, both of which aim to restore health to the peri-apical tissues and prolong the survival of the tooth in the oral cavity. CPD/Clinical Relevance: Examples of different clinical circumstances that were considered for root canal re-treatment are presented.

Pollen food syndrome (PFS) is an IgE-mediated food hypersensitivity reaction that typically results in immediate-onset oral and oropharyngeal symptoms in individuals who are sensitized to pollen. Symptoms are often mild, but systemic allergic reactions, including anaphylaxis, occur rarely. Using a cases series of six patients who were referred to our oral medicine department and for whom a diagnosis of PFS was made, we illustrate the clinical features of PFS and discuss the possibility that oral mucosal lesions may be a feature. Patients with PFS (or indeed other food hypersensitivities) may present to dental professionals in the first instance, therefore, it is important to be able to recognize key signs/symptoms, to give advice and to investigate/refer appropriately. CPD/Clinical Relevance: Awareness of the clinical features of PFS, including oral mucosal lesions, is of value to dental clinicians

Incidental finding of a horizontal root fracture when considering a tooth for root canal treatment 在考虑对牙齿进行根管治疗时意外发现牙根水平断裂

A 44-year-old female patient was referred to the restorative department at Manchester University Dental Hospital for root canal treatment of the maxillary right lateral incisor, which appeared to have a sclerosed root canal. The tooth had a history of trauma when the patient was 6 years old. A cone beam computed tomography scan of the tooth revealed an incidental finding of a transverse root fracture that was otherwise not visible on the peri-apical radiograph. This highlights the diagnostic challenge of peri-apical radiographs and emphasizes the value of three-dimensional imaging in endodontics. It also highlights the importance of timely referral and management of traumatic dental injuries to reduce the risk of complications in the future. CPD/Clinical Relevance: This case report emphasizes the value of three-dimensional imaging in diagnosis.

A cone beam computed tomography scan of the tooth revealed an incidental finding of a transverse root fracture that was otherwise not visible on the peri-apical radiograph. This highlights the diagnostic challenge of peri-apical radiographs and emphasizes the value of three-dimensional imaging in endodontics. It also highlights the importance of timely referral and management of traumatic dental injuries to reduce the risk of complications in the future.
