La transfusion sanguine chez le chien en Tunisie, enquête auprès des vétérinaires libres-praticiens

En Tunisie, la transfusion sanguine chez le chien est pratiquée en absence de tout cadre légal qui protège le vétérinaire et garantit une bonne pratique transfusionnelle. Étant donné que la situation actuelle de la pratique transfusionnelle est inconnue en Tunisie, nous avons opté pour une enquête d’évaluation multicentrique descriptive avec collecte transversale et unique des données. Le questionnaire anonyme conçu avec Google forms a été effectuée en ligne puis adressé à 200 vétérinaires libre-praticiens par e-mail en juin et juillet 2023. Environ un tiers (29,5 %) des vétérinaires ont accepté de participer à l’étude et ont répondu au questionnaire. D’après eux, 35,6 % des vétérinaires ont déjà transfusé au moins une fois un chien malade. Il existe des incohérences entre les pratiques actuelles et les règles de bonnes pratiques, aucun des vétérinaires pratiquant la transfusion ne procède au dépistage des infections transmissibles par le sang, aucun n’effectue le groupage sanguin et le crossmatch. Des efforts sont nécessaires pour promouvoir et organiser la transfusion sanguine chez le chien en Tunisie.
In Tunisia, blood transfusion in dogs is practiced without any legal framework protecting the veterinary practitioner or guaranteeing the conditions for good transfusion practice. Given that the current situation of transfusion practice is unknown in Tunisia, we opted for a descriptive multicenter evaluation survey with a single cross-sectional data collection. An anonymous questionnaire was designed with Google forms and sent to 200 veterinary practitioners by e-mail between June and July 2023. Approximately one third (29.5%) of veterinarians agreed to take part in the study and completed the questionnaire. Among them, 35.6% of veterinarians had already transfused a sick dog at least once. Traitement par orchiectomie et ablation scrotale d’un granulome scrotal chez un Berger allemand de 3 ans

A client presented a 3-year-old dog to the clinic with the primary complaint of recurrent scrotal enlargement and a request for castration. The dog had a history of infertility, recurrent scrotal enlargement and otitis externa. Microbial culture of scrotal subcutaneous fluid revealed heavy growth of Staphylococcus aureus. The scrotal tissue was histopathologically confirmed to be a scrotal granuloma associated with hypospermatogenesis and focal areas of epididymal tubular hyperplasia. The final diagnosis made was scrotal granuloma from chronic S. aureus infection. There is scarce literature on scrotal granuloma associated with S. aureus infection and infertility in dogs as against several cases in humans. There is often an indiscriminate preference for removing the testes and scrotum of dogs presenting chronic disease conditions of the testes and scrotum, unlike in humans. The case was managed by the removal of the testes and scrotum (orchiectomy and scrotal ablation). The dog's fertility could have been salvaged by testicular sperm extraction and in-vitro fertilisation but, the technology was not available in the dog's current location. This case reveals a need for Theriogenologists in developing countries to develop or adopt skills in testicular sperm harvest and in-vitro fertilisation of spermatozoa from hypo-spermatogenic patients.
