Objective: To present an epilepsy surgery case report from the Comprehensive Epilepsy Program (CEP) at Austin Health (Heidelberg, Victoria, Australia). In this case, multimodal imaging-guided stereo EEG allowed for a “super-focal” resection adjacent to eloquent cortex with excellent postsur-gical outcome. Methods: Patient records at the Austin Health were reviewed. Clinical details and investigations were summa-rized. The patient underwent video EEG monitoring, MRI at 3 and 7 Tesla, MEG, PET, SPECT (ictal and interictal), function-al-MRI (EEG-fMRI and resting state functional connectivity) prior to proceeding to invasive monitoring using stereo-EEG and electrical stimulation. Results: All investigations were reviewed at the Comprehensive Epilepsy Program Meeting at Austin health and a plan was formulated for a very limited parietal corticectomy adjacent to eloquent cortex. The patient underwent surgery without suffering a neurological deficit, and had an excellent outcome (Engel class 1B) at 12 months. Conclusion: In selected cases, excellent outcomes can be achieved using multimodal imaging guided stereotactically complaints and address the mood issues to reduce burden of memory dysfunction in patients with epilepsy. Objective: We aimed to study the cognitive outcome of 12- to 18-year old children of women with epilepsy (WWE). Methods: Children under fol-low-up in Kerala Registry of and evaluated with Wisconsin Card Sorting Test (WCST), trail test Rey auditory verbal learning test Rey–Osterrieth Objectives: Depression is a major comorbidity in persons with epilepsy ( p WE). The effect of antidepressants sertraline (SRT) and escitalopram (ESC) in combination with sodium valproate (SV) and levetiracetam (LEV) on seizure and cognitive impairment in rats was studied. questionnaire. A video electroenceph-alography and CT brain (MRI in focal seizures) in all patients.

Status Epilepticus in Neonates
Q4 Medicine Pub Date : 2018-04-01 DOI: 10.1055/s-0038-1667213
A. Saini, K. Vinayan
Abstract Seizures are a reflection of acute brain injury in the neonatal period, and status epilepticus reflects a state of high seizure burden. Neonatal status epilepticus has been reported in 8 to 43% of newborns with seizures. There is no separate definition for neonatal status epilepticus, and it has been commonly defined as a continuous seizure lasting at least 30 minutes and/or a series of seizures whose total duration exceeds 50% of a given epoch. The causes of status epilepticus in the neonatal period mirror the causes of neonatal seizures. It is symptomatic, usually associated with hypoxic–ischemic encephalopathy, inherited metabolic disorders, infections, and cerebral hemorrhage. Management guidelines are not separately outlined for neonatal status epilepticus and are commonly derived from the recommendations for neonatal and infantile seizures. The presence of neonatal status epilepticus significantly increases the odds for development of cerebral palsy, global developmental delay, and epilepsy later in life. Further research is needed into the role of current antiepileptic drugs in causing neuronal injury and use of neuroprotective agents during neonatal status epilepticus.

Abstract Introduction Prevalence of sexual and reproductive dysfunction in men and women with epilepsy (MWE and WWE) ranges from 20 to 60% in different studies. Epilepsy and antiepileptic drugs both contribute in varying degrees to the reproductive endocrine dysfunction. Aims and Objectives The aim of this study is to assess the alteration in sex hormone levels in MWE and WWE on enzyme-inducing antiepileptic drugs (EIAEDs). Materials and Methods This is a cross-sectional case-control study of 29 MWE and 26 WWE on EIAEDs. Control group were age- and sex-matched healthy volunteers. Serum sex hormones such as total testosterone (TT), dehydroepiandrosterone sulfate (DHEAS), estradiol (E2) and progesterone, gonadotropins such as luteinizing hormone (LH), follicular-stimulating hormone (FSH), and sex hormone–binding globulin (SHBG) levels were estimated. Free testosterone (FT) and bioactive testosterone (BAT) were calculated. Gonadal efficiency in men was estimated by TT/LH and in women by E2/SHBG and LH/FSH ratios. Results There was a significant decrease in TT/LH, FT, BAT, DHEAS, and progesterone along with a significant elevation of SHBG and E2 in MWE on EIAEDs. There was also significant elevation of gonadotropins such as LH, FSH, and prolactin (PRL) in MWE. A significant decrease in progesterone, DHEAS, and E2/SHBG along with a marked elevation in SHBG, LH, FSH, and PRL was found in WWE. Conclusion This study clearly demonstrates that EIAEDs have an adverse effect on reproductive health in both MWE and WWE.
