Context

Integrative care is the association with conventional treatments of quality and evidence based complementary therapies used in supportive care. More than half the patients use this approach during cancer treatment. To facilitate access to this type of care, for patients whose WHO performance status is 2-3 and/or presenting a grade symptom 3, an integrative care outpatient hospital was set up almost two years ago. How does this innovative unit function and what are its preliminary results?

Method

A questionnaire for each complementary practice was sent to the different members of the healthcare team (two doctors, four nurses, a psychologist, a dietitian, a beautician) and analysed.

Results

Fourteen questionnaires were studied (some caregivers practicing several techniques): assessment nursing interview, coordinating medical consultation, homeopathy, acupuncture, auriculotherapy, micro nutrition, dietetics, aromatherapy, reflexology, psycho-oncological interview, social-aesthetics, mindfulness, hypnosis, virtual reality and therapeutic meal. All caregivers have experience in oncology and have the relevant diplomas in their particular technique. The feedback from both caregivers and patients is very positive. The benefits obtained in regard to symptoms and quality of life are convincing and in line with the results of the various existing studies. The main indications are fatigue, pain (especially neuropathic pain), digestive and anxiety- depressive disorders.

Conclusion

This day of care is greatly appreciated by patients and caregivers alike. The assessment questionnaires are very positive. A care charter governs the activity of the integrative caregiver and helps to prevent any possible deviation. A qualitative study is about to be carried out to assess our practice.

Cet essai concerne une série de 1022 patients du métro parisien suivis par homéopathie au cours de plusieurs décennies. Il cherche à définir dans quelle mesure les études observationnelles peuvent être utilisées pour évaluer l’impact médico-social, la validité théorique et les effets de la thérapeutique homéopathique, discipline originale, à cheval sur l’empirisme et l’expérimental, point de rencontre entre la maladie étiquetée et celle vécue par le malade. Pour ce faire, toutes les informations notées concernant chaque patient et tous les effets survenus après chaque prescription doivent être systématiquement recherchés et classés. Cette somme vise à être intégrée dans une banque de données et devra être à la disposition des chercheurs. On tente ici de situer la position de l’homéopathie en médecine et d’analyser l’intérêt et les limites des études observationnelles. Puis, on définit la méthode employée au sein du service médical de la RATP. Ensuite, on classe les consultants selon leur origine géographique, leur sexe, leur âge, leurs consultations dans le temps et leur fonction dans l’entreprise. Enfin, on quantifie les consultations : nombre, sex-ratio, par années, par consultant et par motifs et types de pathologie selon la classification CIST 2.

This study concerns a series of 1022 patients from the Paris metro treated by homeopathy over several decades. It attempts to define the extent to which observational studies can be used to evaluate the medical-social impact, the theoretical validity and the effects of homeopathic therapy, an original discipline, straddling the empirical and the experimental, a meeting point between the labelled illness and the illness as it is experienced by the patient. To do this, all the information noted about each patient and all the effects occurring after each prescription must be systematically collected and classified. This then needs to be integrated into a databank and must be made available to researchers. The aim here is to determine the position of homeopathy in medicine and analyse the benefit and limits of observational studies. The method used within the RATP medical service will then be defined. Next, the patients are classified according to their geographical origin, gender, age, their consultations over time and their role in the company. Finally, the consultations are quantified: number, sex ratio, by year, by patient and by motive and type of pathology according to the CIST 2 classification.

Through a clinical case of schizophrenia, we will specify the type of hallucinations corresponding to Alumina, thus enabling us to propose 4 index additions to the “Hallucinations” sections of the homeopathic repertories. Alumina has been the main and the most effective homeopathic medicine prescribed, in combination with neuroleptics. The beneficial collaboration of the psychiatrist and the homeopathic doctor led to the complete cure of a young schizophrenic man with a follow-up of more than 20 years.
