La psicotraumatologia in Psicologia pediatrica: duplicità o integrazione del trauma nelle emergenze pediatriche? Un inquadramento teorico operativo 儿童心理学中的精神创伤学:在儿童紧急情况下,创伤的双重性还是整合性?理论操作框架

The study is the expression of the work carried out by the Italian Society of Pediatric Psychology (S.I.P.Ped.) and presents the theoretical and operational framework of the child and adolescent trauma in Pediatric emergency, which is defined in Pediatric Psychology through the approach of the Psychotraumatology, In this sense, the fundamentals of this approach are: - the type of trauma that develops while the event that caused it takes place and not after it has ended. This time can persist and could cause an atypical development. - the nature of the trauma is explained as an “aggregate” between organic trauma and psychic trauma. In this sense, the study underlines how this aggregate gives life to a “psychoneurobiological displacement”. - the Traumatic Developmental Disorder as an often irreversible damage, which becomes the primary focus compared to the Post Traumatic Stress Disorder, with which it is in a comorbidity relationship. The paper then presents the procedural model of actions for the integrated care between physician and pediatric psychologist, including through the presentation of pediatric conditions, and provides a time sequence from the here and now in which the Aggregate was determined to the time of hospitalization and/or care.

Kidney Ultrasonography After First Febrile Urinary Tract Infection in Children: A Systematic Review and Meta-analysis The clinical utility of renal ultrasound after the first febrile urinary tract infection (UTI) is much debated, and guideline (LG) recommendations often differ. The objective of this study was to determine the prevalence of urinary tract abnormalities detected by ultrasound after the first febrile UTI in children. Medical research databases were used to search for articles on studies on the use of ultrasound in children with first UTI. Primary outcome was the prevalence of urinary tract abnormalities and clinically important abnormalities detected on renal ultrasound. Twenty-nine studies were included, with a total of 9.170 children studied. Of the 27 studies that reported the sex of participants, the average percentage of males was 60% (range, 11% -80%). The prevalence of abnormalities detected on renal ultrasound was about 22%, while the prevalence of clinically important abnormalities was about 4%. The most detected abnormalities were hydronephrosis, pyelectasis, and ureter dilatation. Urinary tract obstruction was identified in 0.4% of cases, and surgery was performed in 1.4% of cases. No studies reported parent-reported outcomes. The results suggest that 1 in 4-5 children with first febrile UTI have a urinary tract abnormality detected on renal ultrasound and 1 in 32 an abnormality that changes their clinical management. Given the considerable heterogeneity of the studies and the lack of comprehensive outcome measurement, the authors suggest the need for well-designed prospective longitudinal studies to fully evaluate the clinical utility of renal ultrasound after the first febrile UTI.

Si riportano alcune evidenze scientifiche documentate sulle correlazioni tra utilizzo degli schermi e sviluppo del bambino, in particolare nei primi anni di vita e poi nella prima infanzia, derivanti dalle principali pubblicazioni disponibili nel contesto italiano ma anche da alcune recenti ricerche svolte in ambito internazionale. Si indagano le principali motivazioni dei genitori rispetto alle abitudini di utilizzo dei dispositivi digitali nell’ambito familiare e l’importanza per i genitori di avere delle indicazioni su questo tema. Si ribadisce il ruolo fondamentale che il pediatra di famiglia è chiamato a svolgere anche in questo campo, informando i genitori circa gli effetti che gli schermi possono avere sui bambini e promuovendo un utilizzo consapevole e attivo degli stessi.

Mastitis in neonatal age is an inflammatory process generally caused by an infection of the mammary gland, often sustained by a “Staphylococcus aureus”. It is important to recognize and treat the infection with parenteral antibiotic therapy to avoid possible complications. In case of late diagnosis, inadequate therapy, or abscess, surgical treatment may be necessary. The clinical course of three cases of neonatal mastitis is described here.

The possible causes of vaginal bleeding in a girl are many, from “simple” infections to pathologies of a neoplastic nature, not forgetting situations of suspected sexual abuse and the possible discovery of a foreign body. It is important that the pediatrician knows how to correctly approach this situation and propose a reasoned diagnostic procedure in close collaboration with the gynecological specialist.
