Dupilumab, a new therapeutic possibility for drug-resistant eosinophilic esophagitis: a phase 3 trial To date, therapy for eosinophilic esophagitis is based on proton pump inhibitors (PPIs), topical corticosteroids, elimination diets, and esophageal dilation in cases of stenosis. Only for topical corticosteroids are there high-grade evidence, and treatment choices are not codified, but tailored based on patient characteristics and clinical practice. Dupilumab, a human monoclonal antibody already approved for therapy in selected cases of diseases with type 2 inflammation (severe asthma, moderate-to-severe atopic dermatitis, chronic rhinosinusitis with nasal polyposis), was tested in this phase 3 study in patients aged ≥12 years with a diagnosis of eosinophilic esophagitis documented by endoscopic biopsy, at a dose of 300 mg 1 time per week or every 2 weeks, compared with placebo. The two coprimary outcomes (histologic remission and change in symptom score) at 24 weeks were achieved only for dupilumab administered 1 time per week, however, histologic remission was similar for both schemes: 59 and 60 percent of cases. On the basis of these data, although there was no comparison with currently used therapies, dupilumab was also approved in Europe for the treatment of eosinophilic esophagitis in adolescents and adults.

The Covid-19 pandemic has acted as a detrimental trigger for the malaise of pre-adolescents and adolescents, who, in a context of continuous and rapid social changes, are going through difficult times, featured by an epidemiological wideranging increase in accesses to mental health services, so high that it is likely to become a ‘new normality’. This article reviews the last three years of Italian medical history and shows the epidemiological data collected at national level and at the Giannina Gaslini tertiary care children’s hospital in the metropolitan city of Genoa.

The association between persistent neonatal pulmonary hypertension (PPHN) due to premature closure of the ductus arteriosus of Botallo (DA) and NSAIDs intake in the third trimester of pregnancy is known in the literature. We describe the case of a full-term newborn with PPHN and right ventricular hypertrophy due to early DA closure following maternal treatment at 32 and 37 weeks of pregnancy with repeated doses of ketorolac.

The group tool in mental health has a long tradition developed during the Second World War with the groups of traumatized soldiers created in England. In Pediatric Oncohematology the group does not have a very long history behind it; some experiences of support for younger patients, but above all for adolescents are reported. In 2020 a group with adolescent patients was founded at the Pediatric Oncohematology Service of the “Vito Fazzi” Hospital in Lecce, born as an experiential group and grown over time in numbers and in the perspective not only of elaborating the common experience but also of working together on supportive projects for children and above all for adolescents at diagnosis. After more than two years of weekly work, the group has changed and is now working on a new balance which, without renouncing the pleasure of helping children and peers, does not forget its mandate to process the experience.
