Language production is a developmental stage whose delay in acquisition often causes great concern to parents, and requests for assessment appear increasingly early and numerous. This phenomenon appears even more pronounced for bilingual children in whom not only families but also the contexts in which they are included often cause alarm. One goal of this article is to create a space for reflection on the timing of intervention and expected language development in monolingual and bilingual children. We will start with the definitions of Primary Language Disorder (PDL), bilingualism, and what may be false beliefs that are increasingly ingrained in common thinking.

The concept of human rights was enshrined in several documents, Conventions, and Declarations; in this article, the attention is focused: on the Declaration on Human Rights and Climate Change initiated prior to the 2015 COP 21, on the Children’s Climate Risk Index dell’UNICEF that provides the first comprehensive view of children’s exposure and vulnerability to the impacts of climate change and on the current guidance for governments (General Comment n. 26) of the Committee on the Rights of the Child explaining how children’s rights are related to the environment and climate change, and what governments must do to protect them.

Child maltreatment and neglect are still a major public health problem worldwide. Dentists are in a privileged position to recognize any childabuse and/or neglect because most of the lesions are found precisely in the facial area, involving teeth and oral cavity. Next to the pediatrician, who regularly examines the child as he grows up and occupies a privileged position in recognizing family situations at greater risk and in supporting the family, the dentist has an important diagnostic support function, having a specific competence for the recognition of non-accidental injuries and neglect with oro-facial localization. As indicated in the Ministry of Health’s guidelines for the management of dental trauma, it is desirable that, in the near future, standardized multidisciplinary intervention strategies be promoted, within which both professions, aware of their respective sentinel roles in protecting abused children, can actively collaborate in establishing detection and reporting protocols.

Secondary prophylaxis of rheumatic fever recurrence with intramuscular benzylpenicillin seems to be the most widely used therapy in young Italian patients. Its duration may vary from several years to decades. Pain and subsequent discomfort related to benzylpenicillin administration are poorly detected and addressed in our country. We present here a survey in which 49 Italian families participated, with children receiving benzylpenicillin therapy every 3 to 4 weeks. Procedural pain, treatment setting, information and communication emerge as critical issues for families. The long course of treatment of these young patients needs to be more explored, reviewed, and likely reorganized, with the aim of limiting and managing, with every applicable measure, the criticality of pain and supporting proper information and communication, equally important aspects of care.

The column on pediatric nutrition edited by the Nutrition working Group of the Pediatric Cultural Association continues in this issue. The group monitors 38 of the most highly qualified international scientific journals based on EBM criteria to disseminate the results of the most relevant articles on pediatric nutrition. On these pages, the main articles published in the monitored journals will be summarized briefly. All articles and editorials published and deemed worthy of attention are listed divided by topic, with a brief commentary. This issue is based on the systematic monitoring of publications from June to July 2023. We hope the service may be useful to the readers of Quaderni ACP.

Nonostante le diagnosi di celiachia siano in continua crescita, sia nella popolazione pediatrica che in quella adulta, la parte sommersa del cosiddetto “iceberg” è ancora molto rilevante. Su come portare alla diagnosi chi ancora non sa di essere intollerante al glutine, il dibattito tra i sostenitori dell’ approccio per case-finding e i sostenitori dello screening universale resta tuttora molto aperto. Un recente studio multicentrico italiano, partendo dalla determinazione degli HLA-DQ2/-DQ8, ripropone il tema dello screening in una popolazione infantile in età scolastica. È un passo in avanti, anche se molti aspetti di questo problema restano ancora in attesa di definizione.

Cortisone is no more effective than saline in sleep-disordered breathing: results of an RCT Sleep disordered breathing (SDB) affects about 12% of children. An Australian multicentre, randomised, double-blind, placebo-controlled clinical trial conducted in third-level centres, intends to verify the efficacy of intranasal mometasone in improving the symptoms and quality of life of children with SDB compared with an intranasal saline solution. 276 subjects aged between 3 and 12 years with SDB were recruited. The results did not show any difference between the two treatments. In fact, both groups recorded an equal percentage of improvement in symptoms, which could be due in both groups to the treatment or to the natural evolution of the picture. This leads the authors to state that a proportion of children with SDB could be managed in the primary care setting.
