Covid-19 nelle cure primarie pediatriche in Italia 意大利初级儿科护理中的新冠肺炎

Introduction: Very few studies have been published on the clinical history and contagiousness of not hospitalized Covid-19 pediatric patients. Therefore, to get deeper in this topic, between October 2020 and March 2021, we carried out an observational study in the pediatric primary care setting in Italy. Methods: The study enrolled patients with Covid-19 disease (cases); patients with suspicious symptoms but negative swab; close contacts of Covid-19 cases. The follow-up was 28 days for patients with Covid-19 disease, 14 days for the other subjects enrolled. Results: 1947 patients aged 0-13 were enrolled: 465 with Covid-19 disease; 549 with suspicious symptoms but negative swab; 933 close contacts of Covid-19 cases. 130 subjects with Covid-19 were asymptomatic at the time of diagnosis. Among the others, in children aged 0-2 the most frequent symptom was rhinitis (41%) followed by fever (38%) and cough (31%). In children aged 3-13 the most frequent symptom was headache (30%) followed by rhinitis (23%), asthenia (18%), fever (16%), cough (11%). No symptoms alone were sufficient to induce or rule out the suspicion of the Covid-19 disease diagnosis. A multiple logistic regression analysis, conducted for the purpose, allowed to build a score capable of inducing disease suspicion in patients older than 2 years of age, based on: fever, rash, rhinitis, headache and asthenia. Conclusions: This study compared subjects with SARS-CoV-2 infection with unaffected subjects with similar symptoms and confirmed that it is impossible to clinically distinguish Covid-19 from other similar respiratory diseases in pediatric age. The obtained score, potentially useful and valid during the study period, could not be applied in other epidemiological scenarios.

Farmaci e allattamento: ancora un dilemma? 药物和母乳喂养:又进退两难了吗?

It is widely believed that many medications are dangerous for infants if taken by nursing mothers. So, they often choose to give up the drug in order to continue breastfeeding or, conversely, to discontinue breastfeeding to take the medication. This occurs even for medications considered as compatible by scientific evidence. The lack of coherent official information on the use of medications during lactation is probably one of the main reasons inducing an excess of prudence, based on presumption of risk rather than on evidence. In addition, literature suggests that breastfeeding training has a crucial role to increase healthcare professionals’ knowledge, attitudes and practices and to improve professional support for breastfeeding, even in case of maternal use of medications or natural products.

Il disturbo dello spettro autistico (ASD) è caratterizzato da difficoltà sociocomunicative associate a comportamenti, attività e interessi ristretti e ripetitivi che conducono a un maggiore coinvolgimento in attività solitarie, come la visione della televisione. La letteratura scientifica sottolinea tanto i rischi connessi a un uso sistematico di questo strumento, quanto i suoi potenziali vantaggi se adeguatamente integrato nella pratica clinica e nell’esperienza quotidiana. In questo articolo descriviamo le difficoltà tipiche di fruizione dei cartoni animati da parte dei bambini con ASD e, a partire da queste, suggeriamo alcune linee guida per la realizzazione di programmi televisivi a loro destinati, affinché possano divenire oggetto di scambio e condivisione con i pari, facilitando i processi di socializzazione.

Although the risk of psychopathological problems for adolescents with Specific Learning Disabilities (SLD) is well-acknowledged, few studies focused on this population after two years of Covid-19 pandemic. The study has two main goals: to investigate strengths and difficulties in the emotional, social and behavioral areas in a group of adolescents with SLD and to analyze the concordance between the mothers’, fathers’ and adolescents’ reports. The study involved 4 adolescents with SLD and 7 typically developing (TD) peers aged 11-14 years. The Strengths and Difficulties Questionnaire (SDQ) administered on-line. In contrast with the main pre- Covid studies, the SLD adolescents did not show more psychological problems than TD peers. On the contrary a higher percentage of adolescents with TD fell into the clinical range. We also observed a low parents-adolescent concordance. Clinical and educational implications derived from these results are discussed.
