Multidisciplinary Treatment for Glioblastoma 胶质母细胞瘤的多学科治疗

60 Gyの放射線療法,テモゾロミドによる化学療法であ るが,その他,さまざまな治療法が近年開発されてきた. 手術は maximal safe resectionが推奨されているため,術 中ナビゲーションや術中MRI,5‒aminolevulinic‒acid(5‒ ALA)術中蛍光診断や覚醒下手術などが行われ,神経機 能温存のためのさまざまな支援技術が存在する.また摘 出後,カルムスチン脳内留置用剤の留置や光線力学療法 が行われる.放射線治療は強度変調放射線治療(IMRT: intensity modulated radiation therapy)が多くの施設で行 われるようになったが,60 Gyの 30回分割照射だけでな く,高齢者には線量を落として短期間で終了する治療法 も標準的になりつつある.化学療法は標準治療のテモゾ ロミドを基盤として,血管新生阻害剤であるベバシズマ ブを時機をみて併用する.その他の治療としては,2018 年より保険適応となった交流電場療法である tumor‒ treating field(TTF)や,2021年に承認された G47Δのウ
