Irisin — the future of ischemic stroke therapy? 鸢尾素——缺血性中风治疗的未来?

Irisin is a recently discovered hormone, synthesized mainly by the muscles. Expression of irisin and its precursor named FNDC5 was also found in the heart, kidneys, liver, pancreas, adipose tissue, and brain including cortical neurons, hippocampus, cerebellum, hypothalamus, and spinal cord. The purpose of this study is to review the latest research on the properties of the irisin and its cytoprotective effect against neuronal damage and to draw attention to its possible clinical use in the treatment of stroke. Notch pathway activity increases after ischemic damage, stimulating the repair of the affected brain area. Irisin activates the Notch pathway which inhibits the activity of microglia, secretion of inflammatory factors, and finally leads to reduction of the brain edema. Studies revealed that irisin increases levels of brain-derived neurotrophic factor (BDNF), leading to enhancement of survival and migration of the neurons, and protecting nerve cells from damage during the ischemic stroke. It was also found that irisin maintains mitochondrial integrity, reduces oxidative stress, and exerts a protective effect on the blood-brain barrier. Irisin entails a neuroprotective effect, reducing the extent of the infarcted area and the degree of brain damage. Stimulation of the irisin expression by physical activity or its exogenous administration remains the subject of research that raises hope for development of the new therapeutic options for diseases, especially ischemic stroke.

Comparison between unfractionated heparin (UFH) and fondaparinux on platelets and D-dimer level in COVID-19 patients with hypercoagulation 普通肝素(UFH)和磺达肝素对新冠肺炎高凝患者血小板和D-二聚体水平的比较

Introduction: This study aimed to compare the clinical effects between UFH and fondaparinux in COVID-19 patients with hypercoagulation. Material(s) and Method(s): This was a prospective cohort study. Samples were taken consecutively from hospitalized COVID-19 patients with hypercoagulation who received UFH or fondaparinux based on the standardized guidelines. A total of 71 patients met the inclusion criteria. Patients were evaluated for platelet and D-dimer values before and after administration of UFH or fondaparinux. Result(s): Although there was no difference in D-dimer reduction between the two groups (p = 0.44), fondaparinux showed a greater reduction, 26% against 22% for UFH. While on platelets, there was a significant difference (p = 0.04) between fondaparinux and UFH. Fondaparinux showed a reduced thrombocytopenia impact, as seen by an increase in pre-and post-therapy platelets of up to 50%, compared to 16% in UFH. In regard to the incidence of Heparin-Induced Thrombocytopenia (HIT), there was no significant difference between post-UFH therapy and post-fondaparinux therapy (p = 0.361). Conclusion(s): Fondaparinux did not reduce platelet levels as much as UFH, but there was no difference between the fondaparinux group compared to the UFH group in the effect of decreasing D-dimer levels and the sign of HIT. Standing waves in a patient with systemic scleroderma 系统性硬皮病患者的驻波

Standing waves are a very rare angiographic image with an unclear etiology. Many theories have been developed to explain this phenomenon, but none have been confirmed. We present an interesting case of a 57-year-old woman, whose angiography showed standing waves along the entire length of the superficial femoral artery. On admission, the patient's general condition was fairly good. No significant abnormalities were observed in the P-A chest feet and hands X-ray, as well as in the ultrasound of the abdominal cavity and pelvis. On March 8, the patient underwent angiography documenting the presence of regular "goose trachea-like" 'notched walls' over the entire length of the right superficial femoral artery. Angiography was then followed by an ultrasound exam of femoral arteries, however, no femoral artery wall changes were seen. The diagnosis was standing waves. Moreover, extensive diagnostics for systemic disease, as well as the overall clinical picture allowed for the diagnosis of systemic scleroderma. Standing waves are a rare phenomenon, harmless to the patient's life and health. Researchers have put forward many theories to explain its unclear etiology. The presence of standing waves has also been reported with the coexistence of diseases such as Buerger's, Tolos-Hunt's, or rheumatic heart disease. Moreover, it is worth mentioning the possibility of misdiagnosing standing waves as FMD. To summarize, standing waves are a harmless phenomenon. Despite its rare occurrence, it is worth remembering not to misdiagnose it and consequently, implement unnecessary diagnostics and treatment and cause superfluous anxiety in the patient.
